What To Do If Prostate Cancer Strikes: A Helpbook for Patients
Why There Is Hope
Unlike breast cancer, which has recently become a celebrated cause, prostate cancer is a silent scourge. Many resources are available for men with prostate cancer, but the topic is difficult for them to discuss. Many men do not get the best treatment for their cancer because they do not act early enough. In 2004, the American Cancer Society estimates that there will be about 230,900 new cases of prostate cancer in the United States. About 29,900 men will die of this disease. (source: American Cancer Society But there is hope. More cancers are caught early and new treatments are making it possible for men to live long and healthy lives following their diagnosis. By taking a few simple steps, men diagnosed with prostate cancer can take charge of their lives and overcome much of the fear and anxiety that accompany a cancer diagnosis. So, even if you have recently been diagnosed with prostate cancer, take heart. You will have many decisions to make, but you will have plenty of company. Ultimately, there is a good chance your cancer will be cured or controlled. Most men die with, not of, prostate cancer.

No One Answer for Testing or Treatment
by John Henkel Russ Ingram didn't sense pending calamity when he reported for a company physical seven years ago. After all, he was in good shape and, at 39, still very much a robust young man with no signs of health problems. During part of the exam, however, the doctor noticed that Ingram's prostate was enlarged. While this can indicate a tumor, often it signals a common benign prostate condition, usually in men much older than Ingram. But a visit to a urologist produced the grim news that his condition was not benign. He had prostate cancer. "I was devastated," says Ingram. "Due to my age, I didn't think there was anything to worry about. It caught me totally off guard. I didn't even know where the prostate was."

Prostate Cancer Education and Support Your trusted online source for Prostate Cancer Information Prostate Cancer Clinical Trials Defined
Phase I Trials Phase II Trials Phase III Trials Participation ... Trials Listing by Phase Clinical trials are organized studies conducted to answer specific questions about new treatments or new ways of using an already known treatment. Every study is conducted in a manor that answers very specific questions to try to increase medical knowledge and find new and better ways to treat patients. New cancer treatments must prove to be safe and effective in scientific studies with a certain number of patients before they can be made widely available.
Us TOO seeks for its members greater access to trials. It is estimated that only about 5% of eligible patients participate in clinical research. Patients take part in clinical trials for many reasons. Usually they hope for benefits for themselves. They may hope for a cure of their disease, a longer time to live, a way to feel better. Often they want to contribute to a research effort that may help others. Patients should choose if they want to take part in a study only after they understand both the possible risks and benefits. All patients in clinical trials are carefully monitored during and after participating in the trial.
Through clinical trials, researchers learn which approaches are more effective than others. A new treatment is always first studied in the laboratory. A new drug will be considered if it is found that it changes cells, or parts of cells, in a way that suggests that it will destroy cancer or help the body handle the side effects of cancer treatment. Based on what researchers learn from laboratory studies, and sometimes earlier clinical studies and standard treatments as well, they design a trial to see if a new treatment will improve on current treatments.

Prostate Cancer Risk Management
There is no organised screening programme for prostate cancer but an informed choice programme, Prostate Cancer Risk Management , has been introduced.
Why isn't there a national screening programme for prostate cancer?
All screening programmes cause some harm. This could include false alarms, inducing anxiety, and the treatment of early disease which would not otherwise have become a problem. When considering population screening programmes the benefits and harms must be carefully assessed, and the benefits should always outweigh the harms. Until there is clear evidence to show that a national screening programme will bring more benefit than harm, the NHS will not be inviting men who have no symptoms for prostate cancer screening. In 1968, Wilson and Jungner of the World Health Organisation developed ten principles which should govern a national screening programme. These are:
  • The condition is an important health problem Its natural history is well understood It is recognisable at an early stage Treatment is better at an early stage A suitable test exists An acceptable test exists Adequate facilities exist to cope with abnormalities detected Screening is done at repeated intervals when the onset is insidious The chance of harm is less than the chance of benefit The cost is balanced against benefit
  • To date, prostate cancer screening fulfils only the first condition. See the Health Technology Assessment Programme's monograph

The American Cancer Society plans to advise men who are at high risk for the disease to begin yearly screenings at age 45. Those at high risk include men with a family history of the disease and African-American men.

The prostate is a gland found only in men that produces the liquid component of semen. Prostate cancer is the second most common cancer in men (after lung cancer), though it mainly affects older men. ... The prostate is about the size of a walnut and is located just below the bladder. It surrounds the tube that carries urine from the bladder and out through the penis (the urethra). See the diagram, below.
    The male urinary tract
    Changes to the prostate
Normally, cells grow and multiply only when the body needs them to. Cancer develops as a result of a breakdown in this process, leading to cells growing in an uncontrolled way. The mass of excess cells forms a tumour (growth), which may be benign or malignant. The tumour is described as malignant if it is able to invade other healthy tissue. The peripheral (outer) zone of the prostate is the area most susceptible to developing cancer.

    Worried About Prostate Cancer?
    Wipe Off That Milk Mustache!
When it comes to preventing prostate cancer, the science is clear: Men who steer clear of milk have significantly lower rates of the deadly disease. According to the World Cancer Research Fund and the American Institute for Cancer Research, 11 separate human population studies have tied dairy consumption to prostate cancer. Take a look for yourself:
The American Cancer Society points out that "about one-third of the 500,000 cancer deaths that occur in the United States each year is due to dietary factors…Although the committee recognizes that no diet can guarantee full protection against any disease, we believe that our recommendations offer the best nutrition information currently available to help Americans reduce their risk of cancer." The Society's top two recommendations are: "1. Choose most of the foods you eat from plant sources"; and "2. Limit your intake of high-fat foods, particularly from animal sources." Results of the landmark Physicians' Health Study of 20,885 doctors showed that men who consumed at least 2-1/2 servings of dairy foods daily were about 30 percent more likely to develop prostate cancer than men who averaged less than half a serving per day.

Things you need to know about prostate cancer
Prostate cancer is the most common non-skin cancer in American men. Between 1950 and 1997, the age-adjusted rate of new cases of prostate cancer more than doubled, probably owing to more widespread screening for the disease. (Age-adjusted rates show what the level of incidence would be if there were no differences in the age composition of the population from year to year.) The rate has recently begun to decline. Estimated new cases, 2001: 198,100 Estimated deaths, 2001: 31,500 Trends: 1990-1997
    • annual change in new cases: -2.1 percent annual change in deaths: -2.2 percent
    Five-year survival: 1989-1996
    • all stages: 93.1 percent localized/regional: 100.0 percent distant: 32.6 percent
Along with the rise and subsequent fall in the rate of prostate cancer diagnoses, there has been a rise and fall in prostate cancer death rates. The reasons for the decline through the 1990s are unclear. Such factors as more effective treatments and screening methods offer a possible explanation. Research is ongoing to help explain the changing rates in recent years. Prostate cancer is still the second-leading cause of cancer deaths in men, behind lung cancer. Age is the most important and best-studied risk factor for prostate cancer. The disease is extremely rare in men under age 40, but risk increases greatly with age. More than 75 percent of cases are diagnosed in men over age 65.
