Abstract: Psychiatric services, more than most medical care services, are sensitive to cultural norms. How psychiatric distress is expressed varies culturally as do the expectations of help from professionals and relatives. This thesis focuses on the encounter between a multicultural population in a Stockholm suburb and the general psychiatric outpatient services.
The basis of the thesis is a register study. An entire year's cohort of patients (N= 839) was divided into eight ethnic groups according to country of birth. All ethnic groups had access to the psychiatric services. Diagnoses given at the clinic varied largely with ethnicity: patients of Greek origin were more likely to receive a diagnosis of somatoform disorder (OR= 8.8) and African patients more likely (OR= 5.6) to receive a diagnosis of psychosis (except schizophrenia) compared with the rest of the patient group. Swedes were about twice as likely as the rest to receive a personality disorder diagnosis (OR= 2.2). Trauma and its consequences are underestimated in psychiatric services, illustrated by the fact that no patient had received a diagnosis of posttraumatic stress disorder (PTSD).

The formal study of childhood stress resulting from natural disasters is an important and growing science. Because natural disasters are unpredictable, intermittent, and multifaceted occurrences, it is difficult to conduct a systematic study of how such disasters affect victims. Moreover, the individual's age, gender, ethnicity, exposure to the trauma, methods of coping, and the social environment are mitigating factors in the development of dysfunction, such as posttraumatic stress disorder (PTSD), and recovery. The opportunity to study children's behaviors in response to natural disasters is a rare and daunting task. Such moments, however, provide an opening for researchers to gain insight into the effects of dire events on children. The potential exists for understanding the short-term and long-term effects of traumatic stress on children, as well as the coping mechanisms and their associated symptoms that youngsters use after a disaster. Ultimately, clinicians could use this information to predict which individuals are in greatest need and thus prevent their symptoms from emerging and treat those exhibiting primary and secondary effects. In this study, we explored individual characteristics, exposure to trauma, methods of coping, and the social environment of a cohort of fourth-grade students who lost their school and their homes to floodwaters following Hurricane Floyd.

Unlike in adults, acute stress disorder is not a sensitive predictor of PTSD in children
Philadelphia Clinicians seeking to predict which acutely traumatized children are in need of more extensive follow-up should not rely solely on assessment of acute stress disorder (ASD) as a diagnostic tool. Research at The Children's Hospital of Philadelphia found that only one in five children who developed posttraumatic stress disorder (PTSD) would have been identified during the immediate post-trauma period using only ASD diagnostic criteria. The study appears in the April issue of the Journal of the American Academy of Child and Adolescent Psychiatry. "Based on previous findings that have found a strong connection between ASD and future PTSD in adults, we set out to demonstrate sensitivity and specificity in predicting child PTSD among children with significant ASD symptoms," said lead author Nancy Kassam-Adams, Ph.D., associate director of behavioral research, TraumaLink at Children's Hospital. "Only when we used symptom subsets like dissociation or arousal did we find sensitive predictors for PTSD." The study population included 243 children admitted to The Children's Hospital of Philadelphia for traffic-related injuries between July 1999 and October 2001. The children had been injured in a traffic crash in which the child was a motor vehicle passenger, a pedestrian or a bicyclist. The researchers assessed ASD symptoms within one month of the injury in 243 children. They assessed PTSD at least three months after the injury in 177 of the 243 children assessed previously.
