Postpartum Depression and the "Baby Blues"
Are changes in mood common after childbirth? After having a baby, many women have mood swings. One minute they feel happy, the next minute they start to cry. They may feel a little depressed, have a hard time concentrating, lose their appetite or find that they can't sleep well even when the baby is asleep. These symptoms usually start about three to four days after delivery and may last several days. If you're a new mother and have any of these symptoms, you have what is called the "baby blues." "The blues" are considered a normal part of early motherhood and usually go away within 10 days after delivery. However, some women have worse symptoms or symptoms last longer. This is called "postpartum depression." What is postpartum depression?

POSTPARTUM DEPRESSION Books on postpartum depression What is Postpartum Depression?
Many new mothers experience "baby blues", a low-grade depression that can last up to 2 weeks after childbirth. Postpartum depression, however, is dark, intense, and long-lasting. If months have gone by since you gave birth, and you are still persistently sad, irritable, having problems sleeping, or you're "just not yourself", you may very likely have postpartum depression. The most common symptom of PPD is insomnia (inability to sleep). Your ob/gyn, primary care physician, or a mental health professional such as a psychologist or psychiatrist can diagnose and treat postpartum depression, or refer you to someone who can. The important thing is to seek help and don't blame yourself. Having postpartum depression does not make you a "bad" or inferior mother in any way. Postpartum depression is thought to be caused by low levels of the hormone cortisol. Cortisol is normally released during times of stress. It raises blood sugar levels and regulates your blood pressure. Cortisol levels rise right before childbirth, then fall dramatically afterward. So, postpartum depression may caused by a "cortisol crash". However, not every woman becomes depressed after childbirth, so this theory is still being questioned.

by Brenda Lane, ICCE, CD After the baby comes, many new parents are simply relieved that they got through it!  However many mothers are unprepared to face one of their biggest challenges - how to manage postpartum depression. Risk Factors for Postpartum Depression/Anxiety Prenatal or previous depression Life stress Lack of social support Marital dissatisfaction Unwanted pregnancy History of sexual abuse Traumatic birth experience High expectations of birth/parenting Physical problems with baby Problems with health providers "Wait a minute.  We never talked about this in childbirth class.  And I didn't have any depression before, so I skipped that chapter in my book," one mother says.   Classes often limit postpartum discussion to mom's physical recovery and taking care of the baby.  But rarely do parents have the opportunity to prepare themselves for such an emotional roller coaster.

What is post partum depression?
Knowing the difference between Baby Blues and Post Partum Depression can prevent anguish for new mothers.
Giving birth is one of the most emotionally rewarding experiences in a woman's life. However, with the birth of a baby comes a time of many changes that some women find overwhelming. The mother's hormones have to adjust to her body not being pregnant any longer and thus it is par for the course for a new mother to experience a wide spectrum of emotions. This mild form of depression is what is referred to as "Baby Blues." It is also quite common for a new mother to feel a bit of a let down after her baby's birth. The baby has been such a physical and emotional part of the mother's body for nine months and now suddenly the baby is a separate being. Some mothers also find themselves being jealous of the baby for taking away all of the attention that she received when pregnant. These feelings are absolutely normal and nothing to be ashamed of! Up to 80% of all new mothers experience the baby blues to some degree. The symptoms vary greatly and can start anywhere from a few hours after giving birth to a few days. As the mother's hormones begin to settle down and return to normal, the blues often go away on their own after only a couple of weeks. If you feel that the symptoms aren't improving and you are concerned about it, there isn't any reason why you have to wait for the whole 6 weeks until your follow up appointment to call your doctor. bodyOffer(28960)

Postpartum depression: More than 'baby blues'
By Christy Oglesby CNN ATLANTA, Georgia (CNN) The recognition of postpartum depression as a legitimate health problem has been an evolutionary process about 50 years in the making. But the number of support groups and developing treatments for the disorder attest to its validity and seriousness, doctors and therapists said. It was about half a century ago that physicians began recognizing patterns in the depression that some women experienced after childbirth, said Paul Appelbaum, president-elect of the American Psychiatric Association. The seemingly unexplained crying, mood swings and irritability, or "baby blues" that about 70 percent of women experience after childbirth is not clinical depression and generally stops after two weeks, he said. But about 10 to 20 percent of new mothers are diagnosed with postpartum depression, said Appelbaum, who specializes in the disorder. And 1 or 2 out of 1,000 will experience psychosis with that depression, he explained. A part of that psychosis might include auditory hallucinations. SYMPTOMS These may be some of the symptoms of postpartum depression:
  • Inability to sleep or sleeping a lot Change in appetite Extreme concern and worry about the baby or a lack of interest or feelings for the baby Feeling unable to love the baby or your family

      • This state peaks 3-5 days after delivery and lasts from several days to 2 weeks.
      • A woman with the blues may cry more easily than usual and may have trouble sleeping or feel irritable, sad, and "on edge" emotionally.
      • Because baby blues are so common and expected, they are not considered an illness.

After giving birth , about 70-80% of women experience an episode of baby blues , feelings of depression anger anxiety and guilt lasting for several days. In about 10% of new mothers , this develops into the more severe postpartum depression (also postnatal depression ), a form of major depression requiring treatment. While the precise causes for postpartum depressions are unknown, several factors have been suggested: In severe cases, postpartum psychosis can develop, characterized by hallucinations and delusions . This happens in about 0.1 - 0.2% of all women after having given birth. Postpartum depression can happen after the birth of any child, not just the first one. Sometimes a preexisting mental illness can be brought to the forefront through a postpartum depression.

    What Is Postpartum Depression?
Postpartum mental illness is gradational, ranging from the blues, a mild reaction to the changes that have taken place in the parent's life; to chronic depressive syndrome, an illness mimicking normal depression; to severe and life-threatening psychosis. It is likely that the majority of women who give birth, between 50 and 80%, experience the "baby blues." Women with the baby blues can experience sadness, anxiety, weeping, gain or loss of appetite, insomnia, and fatigue. They are often overwhelmed by all that is taking place in their lives and say they are not themselves. The baby blues usually sets in during the first few days after the birth of an infant and only lasts 10 to 12 days. New mothers with the blues are usually reacting to the severe change in their lives having a newborn can bring. They are easily treated by reassurances that they are doing a fine job at mothering and with additional assistance caring for their babies. In postpartum psychosis, symptoms can include extreme confusion, fatigue, agitation, alterations in mood, feelings of hopelessness and shame, hyperactivity and rapid speech or mania. The sufferer has bizarre hallucinations or delusions about herself or her baby. Although postpartum psychosis is rare, experienced by only 1 in 1000 new mothers, it can be life-threatening to the woman and to her children.
