New guidelines give specifics for pneumonia care
An update by the Infectious Diseases Society of America offers more detailed drug treatment recommendations for community-acquired pneumonia, while keeping an eye out for antibiotic resistance.

Washington As the nation braces for an especially virulent flu season, revised guidelines to aid physicians in the management of a well-known and dangerous flu complication community-acquired pneumonia were released by the Infectious Diseases Society of America. The IDSA guidelines are updated every few years to, in part, try to stay ahead of the rapidly evolving resistance developed by many infectious agents to even the most advanced and effective antibiotics. A Toronto nurse takes precautions The deadly pneumonia, which is spreading across the world, could be more contagious than previously thought, an expert has warned. Hong Kong, which has been hit hard, has established quarantine camps in an effort to try to contain the spread of the disease. And at Singapore airport, nurses monitored passengers for signs of the illness. Seven were sent to hospital because they showed symptoms of the pneumonia. The illness, dubbed Severe Acute Respiratory Syndrome (SARS), has so far killed 62 people worldwide, and infected 1,800. Initially it was thought that the illness could only be spread by close contact. But Dr Hitoshi Oshitani, a World Health Organization (WHO) expert, told the BBC that this was not necessarily the case. He said: "In most cases infection occurs by close person-to-person contact, but there are several cases now that we cannot explain by this model of transmission." He stressed that SARS was more contagious than the deadly Ebola virus.

More Headlines The mysterious pneumonia-type disease that has struck Hong Kong and spread around the world has caused considerable alarm. It has killed at least 10 people worldwide and infected over 500 more. In an indication of how serious a threat this could be, the World Health Organization took the unusual step of issuing an alert that travelers should avoid Hong Kong and Southern China. Medical experts in Hong Kong now think that they have identified the pathogen that causes the disease. While it's too early to say that things are under control, the disease's spread may be slowing. One lesson that should be learned from this episode is how important a free press is. The disease almost certainly originated in China. Numerous deaths from a mysterious respiratory disease occurred in the southern coastal provinces of Guangdong and Fujian over the last several months. Indeed, the first cases of what's likely the same disease showed up in Guangdong in mid-November. By early January, panic had spread to some Guangdong cities.

    Pentamidine is a treatment for PCP pneumonia. Pneumocystis carinii pneumonia, commonly referred to as PCP, can affect the lungs as well as other parts of the body, including the skin and internal organs. People who are at the greatest risk of contracting this type of pneumonia are those who have less than 200 T4 cells. Symptoms of PCP include shortness of breath, dry cough, and fever. PCP seems to occur very frequently in women as a first or second AIDS-defining illness. Diagnosis in women is often delayed and the illness may be severe by the time it is detected. Pentamidine is used as an inhaled aerosol for prevention of PCP. Fujisawa Pharmaceuticals, who makes Nebupent, has a Reimbursement Service . Call (800) 366-6323 for more information. Side effects
    Anxiety, headache, bronchospasm, cough, and chills are some side effects you may experience from treatment with aerosol pentamidine. IV pentamidine can cause leukopenia, thrombocytopenia, and anemia. You must discontinue ddC until you are stable on pentamidine.

    Sample Tracks: Ballad Of Carol Lynn Don't Wanna Know Why This album has received a lot of slack from those who expect more from Whiskeytown, and the press is half-justified. Pneumonia is a remarkably mediocre album; the kind which could inspire an old pop star like Elton John to sing Adams' praises and eventually join him onstage at a Toronto concert. It's here we're delivered (for the most part), the most ineffectual alt.country created by an independent band today. Perhaps that's why they broke up and left this album as their swan song. "The Ballad of Carol Lynn" is good starter, and raises hopes kept afloat by the next song "Don't Wanna Know Why". "4 Reasons to Lie" swells with repetitious guitar plucks quite nicely, only to have the album then choke up at mid-section. The saving grace here, for some, is the Elton Johnesque "Mirror, Mirror". Completely un-country and as gaudy as sequin socks with white pants, all Adams' influences however wide-ranging, are laid bare with this track. I can imagine "Mirror, Mirror" being despised by purists, but it's catchy as hell. "Paper Moon" and "What the Devil Wanted" also shed some quirky sparkle, but after that the rest of Pneumonia is a write off. The only thing left for diehard Whiskey fans is the last page of the liner notes, which simply state, "Thank you and goodnight."

Flu-ish Dear Flu-ish, The symptoms of pneumonia typically include fever, chills, and shortness of breath, as well as a cough with yellow-green phlegm or occasionally blood. Chest pain with breathing can also be a symptom of pneumonia, which occurs as a result of inflammation of the membrane lining the lungs and chest cavity. Pneumonia is diagnosed through a physical exam, when a physician listens to a patient's chest with a stethoscope. Further testing includes a chest x-ray, or examination of a sputum culture. Pneumonia, which is an inflammation of the lungs due to infection, is caused by viruses or bacteria in most cases. However, pneumonia can also result from organisms like yeast, fungi, or protozoa in people with immunodeficiency disorders. This is commonly seen in people with AIDS who develop pneumocystis pneumonia, where the pneumonia is due to a protozoan. The two main types of pneumonia that exist are lobar pneumonia and bronchopneumonia, though other types of pneumonia do exist such as Legionnaires Disease. Lobar affects one lobe of one lung initially, while in broncho the airways (bronchioles) experience initial inflammation which is then spread to one or both lungs. Treatment for pneumonia depends on the causative agent, and therefore ranges from antibiotic to antifungal medication. Mild cases can be treated at home, but more severe cases may require hospitalization.

People who are at the greatest risk of contracting this type of pneumonia are those who have less than 200 T4 cells. Symptoms of PCP include shortness of breath, dry cough, and fever. PCP seems to occur very frequently in women as a first or second AIDS-defining illness. Diagnosis in women is often delayed and the illness may be severe by the time it is detected. Studies are ongoing to determine the usefulness and safety of combined treatment with primaquine and clindamycin. Side effects
    The combination of primaquine and clindamycin is generally well tolerated. Nausea, vomiting and stomach cramps occasionally occur. Other side effects can be anemia, headache, visual disturbance and intense itching. Dosage
    Combination therapy with oral primaquine at 15 or 30 mg daily with clindamycin given intravenously or orally at 1.8-3.6 g in 3 or 4 divided doses daily is effective in treating PCP.

    Dapsone is an effective antibiotic used for prophylaxis against PCP pneumonia. Pneumocystis carinii pneumonia, commonly referred to as PCP, can affect the lungs as well as other parts of the body, including the skin and internal organs. People who are at the greatest risk of contracting this type of pneumonia are those who have less than 200 T4 cells. Symptoms of PCP include shortness of breath, dry cough, and fever. PCP seems to occur very frequently in women as a first or second AIDS-defining illness. Diagnosis in women is often delayed and the illness may be severe by the time it is detected. Dapsone alone is probably less effective than Bactrim/Septra for prophylaxis. Dapsone with trimethoprim is an effective treatment of PCP. Dapsone with pyrimethamine is being studied for the prevention of toxoplasmosis. Side effects
    Anemia, allergic reaction such as widespread rash, and fever. Dosage
    Dapsone and ddI should not be taken together. The antacid used in the formulation of ddI may prevent dapsone from being absorbed. Some people do use ddI and dapsone in combination, but take them at least 2 hours apart to reduce the risk of getting PCP. Dapsone is taken in pill form. The usual dose is 50 mg twice a day, or as a single dose of 100 mg per day. In such high dose, anemia may be more frequent. How long it may take to work
    Because dapsone is a preventive treatment, you will continue treatment as tolerated to prevent you from getting active PCP or other opportunistic infections that you may be susceptible to.
