Superficial Thrombophlebitis
Superficial thrombophlebitis is an inflammation of a 'surface' vein, usually on a leg. A small blood clot may form in the vein, but is usually not serious. The condition usually settles and goes within 2-6 weeks. Treatments can ease the pain and discomfort. Complications occur in a small number of cases.
What is thrombophlebitis?
  • Phlebitis means inflammation of a vein.
  • Thrombo(sis) means a blood clot in a vein
A vein is a blood vessel that takes blood towards the heart. If a vein becomes inflamed, a blood clot commonly forms inside the inflamed portion. So, the term thrombophlebitis is used to mean an inflamed vein, with or without a small blood clot inside the vein. (However, thrombophlebitis is commonly just called phlebitis.) What is superficial thrombophlebitis? The superficial veins are the ones that you can often see or feel just under the skin. Most bouts of thrombophlebitis occur in a leg vein, but any superficial vein can be affected. A typical site is in a varicose vein in a leg. Varicose veins are common, particularly in pregnant women. Superficial thrombophlebitis is not usually serious, but complications sometimes occur (see below). Note: this leaflet does not deal with inflammation or thrombosis of deep veins which are larger, pass through the muscles in your arms and legs, and you cannot see or feel. People often get confused between thrombophlebitis and deep vein thrombosis, but they are quite different. A deep vein thrombosis (DVT) is more serious, and is dealt with in another leaflet.

Phlebitis And Thrombosis
Phlebitis is an inflammation of a vein. Thrombosis is the formation of a blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, blood vessel is called thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. Symptoms of phlebitis may include:
  • a hard and cord-like vein that is very sensitive to pressure or touch a surrounding area that is red and warm to the touch a slight fever or swelling of the extremity
The primary danger in thrombophlebitis is that the clot may move and travel to a vital organ, such as the lung, causing serious damage. If you have symptoms of thrombophlebitis or deep vein thrombosis your healthcare provider may order a doppler ultrasound examination (a test that uses sound waves to locate clots); a venogram in which dye is injected into a vein, usually in the foot. (X-rays are then taken to visualize the circulation and any blockages that may be present.) Lung scans may be ordered if there is concern that blood clots have traveled to the lungs.

Contributing risk factors Other conditions that may be present Treatment recommendations The term phlebitis refers to an inflammation of a vein, usually in the leg, frequently accompanied by blood clots that adhere to the wall of the vein. When the affected vein is close to the surface, the condition is called superficial phlebitis. This condition usually resolves on its own without further complications. However, when phlebitis occurs in a deep vein, it is called deep vein thrombosis (DVT). DVT is a life-threatening condition because a clot could dislodge from the vein and lodge in the lungs. If a swollen, painful vein does not disappear within 2 weeks, consult a physician. Symptoms of superficial phlebitis include pain, swelling, redness, and warmth around the affected vein. The vein feels hard to the touch because of the clotted blood.
Deep vein thrombosis is more difficult to diagnose. It can occur without any symptoms until the clot reaches the lungs. However, in about half of cases, there are warning symptoms including swelling, pain and warmth in the entire calf, ankle, foot, or thigh (depending on where the involved vein is located). Als sehr störende Nebenwirkung kann eine Phlebitis mit Bläschenbildung proximal und distal der Einstichstelle (auch ohne Extravasation) auftreten. Die Bilder zeigen diese Komplikation 7 Tage nach der Infusion von 50 mg Vinorelbin (in 250 ml NaCl 0,9% intravenös infundiert über 20 Minuten). Solche Zwischenfälle können vermieden werden, wenn man sich an die Vorschläge hält, wie sie zum Beispiel bei Anita Margulies et al. [1] beschrieben sind.
Vinorelbin is semisynthetic catharanthus alkaloid displaying activity on an extremely wide variety of tumors, such as multiple myeloma, bronchial carcinoma and breast cancer. In metastasizing breast cancer it has even been used as frontline therapy. As a very troublesome side effect, phlebitis with formation of small vesicles proximal and distal to the puncture site (with or without extravasation) may occur. The illustrations show ths complication 7 days after infusion of 50 mg vinorelbin (infused iv in 250 ml 0,9% NaCl). These incidents can be avoided by following the suggestions of e.g. Anita Margulies et al. (Onkologische Krankenpflege, 2nd ed. Berlin: Springer 1997; p. 898).
