Phlebitis
When blood clots inside a vein or artery, at least two things take place: partial or complete obstruction to the flow of blood within the vessel, and inflammation. The veins of the lower legs are the usual sites of abnormal clots, and the reaction is commonly referred to as phlebitis - inflammation of a vein. Several factors may be responsible. Injury to the inside of the vein may disrupt the delicate lining layer of tissue, thereby triggering the clotting mechanism. Also, years of blood pooling in the legs from the effects of gravity, pregnancy or constraining garments may stretch the veins, injuring the lining and pulling apart the valves that direct the blood back to the heart. Acute symptoms may include redness, swelling, fever, pain in the calf and warmth of the leg. It may be painful to bend the foot upward. On a more chronic basis, swelling becomes prominent. Skin changes may include rashes, peeling and permanent darkening. Blistering with serious infection can also occur. In the most severe cases, so much tissue may be involved that amputation is necessary. Although the initial exam may strongly suggest phlebitis, further tests may be recommended. Confirmatory tests are usually of two types: invasive and noninvasive.

Basically it means a clot in the vein. As the venous system is divided in deep veins and superficial veins one can have clot in each of the systems. Only very rarely clotting can occur in both of the systems in the same time. Superficial vein thrombosis or SVT is also called superficial thrombophlebitis/phlebitis. The "itis" denotes that the vein is engulfed in an inflamatory process: the skin around the vein is read and painful. Swelling can be present as well. Unfortunately, many physicians treat this condition with antibiotics. This is completely unnecessary as in most of cases there is no bacterial infection. The main cause for the condition are varicose veins. Blood stagnates in those venous pools and will clot easily especially during inactivity. Injecting varicose veins can lead to superficial phlebitis as well. In the absence of varicose veins one should look for other reasons such as malignancy elsewhere in the body, gout, Buerger's disease. or an inherited tendency to clot. ( See FAQ The treatment of the condition is simple: evacuation of clot if the patient comes early to the physician, local compression with ace bandages or surgical stockings, and ambulation. The only medication to be given is ibuprofen (Motrin/Advil) 5-7 days that will ease the pain and calm the inflamatory reaction. Occasionally SVT can be associated with DVT. Only in these cases is anticoagulation treatment necessary. If the underling condition for the superficial phlebitis was varicose veins after the inflamatory process has cooled off, surgery for the varicose veins is indicated.

Phlebitis

This condition is also known as inflammation of the veins caused by infection or injury. Two types of phlebitis can occur. The more common one is inflammation of the superficial or surface veins of the legs; the other type involves inflammation of the deep veins of the legs. Both result in an irritated, itchy, painful and tender feeling in the vein and the tissues beneath it.
Detailed Description
Phlebitis can also increase the chances of clot formation and result in a pulmonary embolism. Phlebitis in the deep veins can result in deep vein thrombosis (DVT). This condition results when blood circulation in the veins slows down or in blood that clots easily. Phlebitis can also result from vein injury in those with impaired circulation. It can develop following varicose vein damage in the legs. Prolonged use of an intravenous needle, particularly during illegal drug use, can also cause vein injury. Phlebitis can develop in people with pancreatic cancer, breast and ovarian cancers, or lupus.
Treatment
Phlebitis can be diagnosed from a physical examination; an ultrasound may also be necessary to detect if phlebitis has infiltrated the deep veins, resulting in DVT. With phlebitis in the superficial veins, treatment includes anti-inflammatory drugs, application of a heat source, and wearing elastic compression stockings.

Treatment of Phlebitis at Mayo Clinic in Jacksonville
(Synonyms: thrombophlebitis, thrombosis) When a clot and associated inflammation occur in a vein, the disorder is called thrombophlebitis. Often the name is shortened to phlebitis. A minor inflammation may be called thrombosis. Phlebitis may affect either a deep or a superficial vein. It can be caused by prolonged bed rest after an operation, paralysis, cancer or the use of the female hormone estrogen. Sometimes prolonged sitting during a long car or airplane ride will cause thrombosis of a vein. When the clot and inflammation occur in a readily visible vein near the surface of the skin, the diagnosis is superficial thrombophlebitis. The hard, tender clot can usually be felt and seen, and the affected area feels uncomfortable. Superficial thrombophlebitis rarely leads to serious complications. Treatment consists of applying heat to the area, elevating the limb and using anti-inflammatory medications. When phlebitis occurs in a vein deep in the leg (or, more rarely, in the arm), the condition is called deep-vein thrombosis. The condition can be diagnosed using ultrasound scans or X-rays taken after dye is injected (venogram). The principal danger of deep-vein thrombosis is pulmonary embolism. Several episodes of deep-vein thrombosis could lead to the development of a permanent obstruction and persistent leg swelling.

    Phlebitis: the inflammation of a vein. Cause unknown. May occur in acute or chronic infections or following operations or childbirth. Embolism: the obstruction of a blood vessel by foreign substances or a blood clot. Diagnosis depends upon predisposing factors. Embolism usually due to blood clots. Thrombosis: the formation, development, or existence of a blood clot or thrombus within the vascular system. This is a life-saving process when it occurs during hemorrhage. It is a life-threatening event when it occurs at any other time because the clot can occlude a vessel and stop the blood supply to an organ or a part. The thrombus, if detached, becomes an embolus and occludes a vessel at a distance from the original site; for example a clot in the leg may break off and cause a pulmonary (lung) embolus. Consult the doctor immediately.
