Periodontal disease occurs when bacteria from the mouth form on the teeth in a substance called plaque. The plaque makes it's way under the gumline and sets in motion a vicious circle, which can eventually lead to tooth loss. The bacteria in the subgingival plaque will secrete toxins. These toxins damage the periodontal tissues and can decrease the attachment. However, the bacteria will also elicit an inflammatory response from the animal's gingival tissues. White blood cells and other inflammatory mediators will leak out of the periodontal tissues and into the periodontal space (between the gum or bone and the tooth). The white blood cells will release their enzymes to destroy the bacterial invaders, but will also damage the attachment of the tooth. As this progresses, the pocket will get deeper and deeper. This will weaken the bone in the area, and if it is in the lower jaw it can weaken it to the point of causing a pathologic fracture . This is most common in older small breed dogs. The end stage of this disease is tooth loss, however the disease has caused problems well before this. The inflammation in the gingiva that allows the body's defenses to attack the invaders also allows those invaders to gain access to the body. The bacteria from the mouth can enter the bloodstream and be carried to the bodies. Studies have shown that these bacteria will be filtered out by the kidney and liver, and can cause microabscesses on these organs. This leads to a decrease in function of these vital organs over time. In addition, it has been suggested that these bacteria can become attached to the heart valves and cause a disease called endocarditis. In addition, the body must deal with these bacteria on a daily basis, leading to a state of chronic disease.

    Dr. Edwin S. Rosenberg received his degree in dentistry from the University of Witwatersrand in South Africa, where he practiced Periodontics, Periodontal Prosthesis and Fixed Prosthesis. He received a degree in Periodontics from this University. In 1974, Dr. Rosenberg came to the United States, where he earned certificates in Periodontics and Periodontal Prosthesis from the University of Pennsylvania's School of Dental Medicine.
    A Diplomat of the American Board of Periodontology, Dr. Rosenberg is internationally recognized as one of the leaders in the field of Periodontics, Periodontal Prosthodontics, and Implant Dentistry. He has written over 160 books, research abstracts, and book chapters that have been widely published throughout the world. He also frequently lectures in the United States and internationally at professional and scientific congresses and courses.
    He was awarded a full professorship at the University of Pennsylvania School of Dental Medicine, with tenure, where he served for over ten years as Director of the Postdoctoral Program of Periodontics and Periodontal Prosthesis. Dr. Rosenberg served as the first Director of the Branemark Implant Center at the University of Pennsylvania.
    He was the former Chairman and Professor of Periodontics in the Department of Periodontics, and Director of the Dental Implant Center, both at Temple University's School of Dentistry.

If that is not the case, your dentist and hygienist will want to help you learn ways to improve the health of the tissues before moving forward with definitive restorative care. Depending on the stage of disease present, treatment may be as simple as very thorough scaling and polishing of tooth surfaces and an improved home care routine, or as complex as surgical procedures to save your teeth. However you and your dentist have decided to proceed, the process will begin with you having a complete understanding of your present condition followed by time with the hygienist to start the initial therapies. If the course of treatment is to be completed without surgery, it is likely that your hygienist will finish it. If surgery is indicated, then a referral to a periodontist will likely follow. When a referral takes place, your dentist and the periodontist will have detailed discussion regarding the course of treatment you have started. Together the three of you will work to achieve your goals and objectives. In extensive restorative cases, it may be necessary for your dentist to prepare teeth and place provisional restorations prior to you seeing the periodontist. These provisional (sometimes called "temporary") restorations allow the dentist and periodontist to work to achieve the best end result for your circumstances.

It has been estimated that at least 80% of the adult populations have some form of gum disease. Periodontics deals with the diagnosis, prevention and control of periodontal diseases. One of the most obvious sign of periodontal disease is bleeding gums . Other signs of periodontal disease which may be present are red and swollen gums, loose teeth, bad breath, periodontal pockets, drifting of teeth may occur during the more advanced stage of the disease. As periodontal disease is usually painless, an individual may not be aware he has periodontal disease until the condition has become quite serious.
    Signs of gum disease The main cause of periodontal disease is bacterial plaque . Bacterial plaque is made up of different types of bacteria and their by-products which by virtue of their toxicity may cause damage to the gums and teeth if allowed to accumulate for a sufficient length of time. It is difficult to see plaque by the naked eye unless the palque has become very thick.     NASA Langley Technology Revolutionizes Dentistry
    Today's adults over 35 years old are at risk for periodontal disease, the single most destructive dental disease today causing tooth loss. Periodontal disease is the loss of connective tissue between teeth, the supporting tissue and bone. A tremendous effort is being dedicated to developing tests for detection of periodontal disease. NASA Langley Research Center in Hampton, Va., with the United States Navy, is developing an instrument to detect the onset of periodontal disease. The function of this instrument is to identify and map the upper boundary of the periodontal ligament and its variation over time as an indicator of the presence of periodontal disease.
    Benefits
    This instrument will allow for a display of real-time data and images to determine the presence of the disease. The electronic nature of the instrument allows for automatic storage and retrieval of patient data for tracing and comparison purposes. Diagnostic data collected with this instrument is presented in a user-friendly manner and will give the dentist the capability of automatically storing the data and visual comparison to previous data through special software. The greatest benefit of this new technology is that it would result in minimal-to-no patient discomfort.
    About the Technology
    This instrument uses an ultrasonic sound wave to interact with the tooth structure and with the periodontal structures to map the conditions present within the periodontal pocket (see graphic). This technology will speed up the process of identifying periodontal disease and provide more comprehensive data from which the physician can diagnose the condition of the periodontal structures. Instead of a metal probe being inserted between the tooth and thegum, an acoustic wave will scan the tooth periphery for the presence of abnormalities.

    What Is Gum Disease?
    Gum disease or periodontal disease, a chronic inflammation and infection of the gums and surrounding tissue, is the major cause of about 70 percent of adult tooth loss, affecting three out of four persons at some point in their life. What causes gum disease?
    Bacterial plaque - a sticky, colorless film that constantly forms on the teeth - is recognized as the primary cause of gum disease. Specific periodontal diseases may be associated with specific bacterial types. If plaque isn't removed each day by brushing and flossing, it hardens into a rough, porous substance called calculus (also known as tartar).Toxins (poisons) produced and released by bacteria in plaque irritate the gums. These toxins cause the breakdown of the fibers that hold the gums tightly to the teeth, creating periodontal pockets which fill with even more toxins and bacteria. As the disease progresses, pockets extend deeper and the bacteria moves down until the bone that holds the tooth in place is destroyed. The tooth eventually will fall out or require extraction.     Periodontics, Prevention, and Geriatrics includes the disciplines of periodontics, dental hygiene, geriatrics, behavioral sciences, oral health, and community dentistry.
    Periodontics is concerned with establishing and maintaining health for the structures that provide support for the teeth. The educational program is designed to help students develop the ability to instruct patients in preventing periodontal diseases, diagnosing and treating patients with gingivitis and moderate periodontitis, and recognizing and refering patients who require specialist care. The dental hygiene program is a component of this department and is concerned with providing a wide range of preventive oral health care services.

    The graduate program in Periodontics has three major objectives: (1) to teach the scientific basis of specialty practice; (2) to insure a high level of competence in the diagnosis of oral diseases, treatment planning both periodontics and implant, comprehensive therapy, and follow-up of patients with periodontal diseases; and implant treatment; and (3) to develop future leaders not only in the specialty of Periodontics, but also in the larger dental arena, with due emphasis on research and clinical aspects.
    Strong emphasis is placed on developing scholarly ability and technical proficiency, while maintaining a sense of professional responsibility and social obligation.
