    COLUMBUS, Ohio, Oct. 30, 1998 Most mothers and mothers-to-be are seriously uninformed about five basic infant nutritional practices, according to a national survey conducted by The Institute of Pediatric Nutrition. Most of the 323 women surveyed are uncertain about a range of nutritional issues including: when to introduce cow's milk into the infant's diet, whether fruit juice has any nutritional value, and how to deal with a low milk supply during breastfeeding. Additionally, the survey shows that mothers misinterpret normal spitting up as an allergic reaction and mistakenly believe iron-fortified formula causes constipation misconceptions that can lead to improper feeding decisions. An analysis of survey results shows that only five percent of mothers and mothers-to-be were able to provide accurate answers indicating they understood all of the following: Cow's milk should not be introduced when the baby starts eating solids.

    Prepared for the students of SUNY-Health Science Center at Brooklyn.
    These "NUTRITION NOTES" were prepared at the request of Dr. Laurence Finberg, formerly Chairman and Professor of Pediatrics of the Children's Medical Center of SUNY-Health Science Center of Brooklyn. "I think," he said, "you should tell medical students what they need to know about the scientific basis of pediatric nutrition."
    The NOTES provide a beginning for understanding
    a foundation for learning.
    They will help you understand how a problem unfolds and what language should be used to describe the problem, how to develop a preliminary plan of action and obtain more information. Do you need more? Yes, you do. There is an abundant literature in clinical nutrition that is available in the texts cited, other books, and in readily available journals to be found in libraries, and more recently, on the Internet.

    IN LATER LIFE

    A final concern for consideration in these Nutrition Notes is somewhat new to pediatric nutrition, which is the subject of the prevention of the degenerative diseases of later life by appropriate nutrition in childhood.
    One example is the attempt to prevent coronary heart disease (CHD). It is now clear that cholesterol levels at the point where there is arterial damage have some influence on the occurrence of coronary occlusion. It is also clear that fatty streaks appear in the aortas of children on high fat diets by 10 years of age or so. Such lesions however, are not seen in coronary arteries until much later. What is not clear is whether dietary control in childhood will influence what happens in later life, except in those unusual families where there is a genetic defect producing hypercholesterolemia.
    There remain differences of opinion on the importance of monitoring cholesterol levels in all children. Part of the reason lies in the fact that this measurement is a very crude surrogate for what one really wants to know. The other part of the objection is that having measured it, it is not clear (except in families with genetic defect) that anything beyond the already recommended diet is of any value. In another decade, and perhaps sooner, our tools will be better and our course of action more clearly delineated.

    The Pediatric Nutrition emphasis is a residency-training program outside the Division of Nutrition that is offered to individuals who desire increased knowledge and additional clinical experience in pediatric nutrition. Registered dietitians who are interested in improving their pediatric skills and U of U CMP students who have completed their second year of study may apply to a 6-12 week pediatric practice experience provided at Primary Children's Medical Center. Primary Children's Medical Center is a 240-bed tertiary care pediatric hospital located next to the University of Utah campus and University Hospital.     Researchers have known for about 25 years that atherosclerosis or hardening of the arteries begins in childhood. But it has taken time to find markers for artery disease and to see if the markers important for problems in adults also are significant in children. Both studies looked at C-reactive protein, a marker of systemic inflammation that is produced in the liver. High levels of C-reactive protein are a known predictor of heart disease, cardiovascular problems, and type 2 diabetes in adults. The first study, published in the January issue of Obesity Research, looked at obesity, levels of fasting insulin and C-reactive protein in the blood. Obesity is associated with higher than normal levels of insulin and C-reactive protein in older children and adults but little is known about these associations in very young children. The researchers found that greater body mass index, even in very young children, is associated with higher fasting insulin levels and that fasting insulin is associated with C-reactive protein level. The researchers saw no observable relation of fasting glucose level with the body fat measurements, indicating that glucose equilibirium was maintained despite higher fasting insulin levels among the children with above average weights. But the findings that the children with higher body mass index had higher fasting insulin levels indicate that insulin resistance, a potential warning sign for type 2 diabetes, begins much earlier in life than previously thought.

The reason for that is to allow infants to develop the motor skills necessary to clearly indicate hunger or satiety. There is some evidence that spooning food into infants who lacks the skills to show if he or she is no longer hungry may increase the likelihood that the child will learn to overeat. This becomes more important issue in light of theincreasing prevalence of obesity in childhood. e. A relatively high fat intake should continue throughout infancy to ensure adequate caloric intake and to provide substrate for central nervous system growth. Because of its low caloric density and high protein and electrolyte load, low- fat milk is not recommended by the AAP until age 2 years. a. Most of food aspiration occurs in children 3 years of age and younger. Two-thirds of the recovered food items are clearly inappropriate food for infants and young children (such as nuts, popcorn, and candy), but one fourth of the items are vegetables and fruits, suggesting a continued need for supervision to prevent choking.
