PedNSS
Pediatric Nutrition Surveillance System
2002 Pediatric Nutrition Surveillance Report * (PDF - 540K)
Background and Problem Description
In the late 1960s, the United States Ten-State Nutrition Survey characterized the nutritional status of children from low-income families as being less than satisfactory. These findings generated concern about the nutritional status of low-income populations in the United States. Specifically, calories, calcium, iron, vitamins A and C were less likely to be consumed in adequate amounts by low-income black and Hispanic children. In response, CDC began working with five states in 1972 to develop a system for continuously monitoring the nutritional status of specific high-risk population groups. Data already being collected in participating programs, such as growth measurements and hemoglobin concentrations, were used to form the basis of the PedNSS. An important use of the PedNSS data is monitoring the Healthy People 2010 objectives. These objectives are designed to serve as a goal for monitoring progress towards improving the health status of the nation. Examples of those that are relevant to the health of infants and children and that can be monitored by PedNSS include the following 2010 objectives:*

Pediatric Nutrition Guidelines for Discharge Home
When you return home after your transplant, your immune system is still functioning below normal. You may also have increased nutrient needs due to medications or loss of muscle mass. Calories, protein, fluid, vitamins, and minerals are necessary for you to regain your immunity, maintain or achieve your weight goal, strengthen your muscles, and feel better!
Food Safety
  • Allogeneic patients should continue the "Immunosuppressed Patient Diet" until off all immunosuppressive therapy (such as prednisone, FK506, cyclosporine). Autologous patients should continue the diet during the first three months after transplant. When you discontinue the Immunosuppressed Patient Diet continue to use care when purchasing foods, preparing meals and dining out. Situations where you cannot control the cleanliness of the food put you at greater risk for acquiring a food-borne illness. Examples include salad bars, smorgasbords, delicatessens, bulk food bins, free food samples offered in stores. Review the Food Safety Guidelines in the Patient/Caregiver Manual. Continue to avoid herbal and other preparations, since they may contain harmful ingredients or infectious organisms. Herbs are not regulated for purity or for effect on health.

William J. Klish, M.D., is Professor of Pediatrics, Head of Pediatric Gastroenterology at Baylor College of Medicine and Chief of the Nutrition and Gastroenterology Department at Texas Children's Hospital in Houston. Dr. Klish has served as chair of the Committee on Nutrition of the American Academy of Pediatrics. klish@yourbabytoday.com . While Dr. Klish cannot respond to each individual inquiry, we will post answers to the most common questions here. Q: My little girl is 15 days old, and she is already suffering from colic. We have tried everything to console her, from car rides to rocking. How long will this last? What do you suggest?
A: Colic is a self-limited problem that usually begins to disappear after two months of age. Occasionally, a child will continue to have symptoms until four months of age. Let's hope that your child is not one of the more prolonged cases since, as you have pointed out, it is very difficult to comfort a colicky baby.

Assuring Pediatric Nutrition in the Community
FREQUENTLY ASKED QUESTIONS:
Specific Diagnoses - Ketogenic Diet
Is the ketogenic diet being used with young children? Has TPN been used for children on the ketogenic diet, and if so what kind of start up is recommended? Has anyone used crackers or cereal to increase variety of textures? A pediatric neurologist has prescribed the ketogenic diet for a 14 month old and a 7 month old. The 14 month old has recently been sick and vomiting, and when the mother called the hospital she was told to add more Polycose and give it some time. The child is already below the 5th percentile for both weight and length. Is the ketogenic diet being used with kids this young? Yes, the ketogenic diet is being used with infants under age 2 years, more now than in the past. There is some literature to support the use of the ketogenic diet for infants (1,2). The ketogenic diet is very complicated to manage successfully and definitely requires a multi-disciplinary approach. There are centers in many parts of the country that have developed programs with guidelines to treat seizures with the ketogenic diet. Infants and children who start the diet are frequently below the 5th percentile for weight and length, often due to the negative impact of the intractable seizures on being able to eat.

Proper diet and nutrition is critical for young patients undergoing medical treatment. Some conditions such as epilepsy heart disease and diabetes can be greatly improved through dietary regimens. Other conditions, including cancer , require special care to encourage children to eat properly to help their bodies get the greatest benefit from treatment. Still other conditions, such as failure to thrive , malnutrition, nutritional anemias, obesity and food allergies are related directly to a child's diet.
Sutter Children's Center team of pediatric specialists, under the direction of a pediatric endocrinologist and pediatric gastroenterologist, provide nutrition screening, assessment, intervention, education and counseling to ensure patients' diets match the needs of their body. Team members include pediatric nutritionists and pediatric rehabilitation feeding specialists, as needed.
