Bone Marrow Aspiration

Step 1. Prepare and drape the site as for iliac crest graft harvest. With a surgical marker, outline the medial and lateral borders of the anterior crest and the anterior superior iliac spine. Note that the iliac crest curves dorsally toward the sacroiliac joint. Also note that the lateral anterior crest has a significant overhanging lip. Step 2. Introduce the aspiration needle into the crest. The entry point must be in the central-medial portion of the crest to avoid penetrating the lateral overhanging lip. Note that the inner and outer tables of the pelvis converge below the anterior crest. Gentle manipulation is necessary to direct the needle between the tables. Take care to stay within the tables of the pelvis. Too much angulation may perforate the medial wall of the pelvis and place the abdominal viscera at risk. Or, it may violate the lateral pelvic wall and enter the gluteal musculature, potentially damaging the lateral femoral cutaneous nerve. The sciatic nerve can also be entered inferiorly, endangering the sciatic nerve and the superior gluteal artery. Step 3.

Dear Pat I just re-read your message and felt I had to respond even though you realize the advice I am giving you, is just my opinion since I only know about hips.. Some people have problems with bones when the weather is bad. My hips tend to "lock up" on and off when it's cold outside, but when I was younger (in my20's) the weather never bothered me. I wonder if it's because the metal is inserted in the bones. Just a thought. I would have him ask his doctor or physical therapist that question. Ph.D. In the Department of Orthopedics, measurements at the tissue level had its beginnings in 1950s with the use of microradiography to study circulation in skeletal tissue. This same microradiograph is still in use today. In the 1960s, B. Lawrence Riggs, M.D. and Jennifer Jowsey, Ph.D., conducted groundbreaking work on the use of tissue measurements to diagnose metabolic bone disease. Tetracycline labeling was used in this early work to confirm measurements of bone formation; currently, the use of fluorochrome bone labeling is mandatory to assess any impact on bone growth or turnover (see image of tetracycline labeling in normal biopsy). Today, the preparation and quantification of iliac crest biopsies are routinely conducted in Orthopedics in the Bone Histo Lab. Over its 20+ year history, the lab has prepared and measured over 5000 human biopsies. The lab is associated with the Mayo Bone Disease Core Group, within which is a team of endocrinologists who are specialists in bone histomorphometry. These members (see staff affiliations) analyze and interpret the histological slides of human tissue. One such member of the Bone Core Group, Dr. Stephen J. Hodgson, M.D. (emeritus) published in 1986 a report on the

The Effect of Bone Graft Extenders to Enhance the Performance of Iliac Crest Bone Grafts in Instrumented Lumbar Spine Fusion

Autogenous cancellous bone is commonly reported as the most successful material for bone grafting. For successful fusion to occur, graft stabilization with rapid and reliable bone repair is necessary. However, surgical procedures requiring iliac crest bone graft (ICBG) harvesting introduce a variety of potential risks to patients including increased surgical time, increased blood loss, and increased postoperative trauma pain. There is only a finite amount of ICBG available for harvest from any one site, and optimization of the harvested graft is of paramount importance. Therefore, the challenge to surgeons is to achieve successful bone fusion while using a limited supply of autogenous bone graft, thereby minimizing risk to patients. The use of a bone graft extender may enhance the success of the bone fusion by expanding the available ICBG. This radiographic report details the short-term results of the use of ICBG with a commercially prepared allograft demineralized bone matrix (DBM) in patients undergoing lumbar spinal fusion.
