(CBS/AP) With two-thirds of Americans now overweight and at greater risk for heart disease, diabetes and certain cancers, many in the medical community want to redefine the role doctors have in helping people slim down.
Doctors say the pressures of general practice — which can limit time with patients to mere minutes — and a lack of specialized training make it difficult to treat obesity in a primary care environment.
They say treatment involves not only creating individualized diet and fitness plans, but also assessing serious health concerns and addressing the underlying emotional and psychological issues that contributed to the weight gain. "In eight minutes you can't undo all that stuff," said Peter Vash, a Los Angeles doctor who has treated obesity for 25 years.

Drug treatment can play a major part in any management method, and combination therapy, with two anorectic drugs, may be the best approach. Joining two anorectics together permits decreasing the dose of both agents, which reduces adverse effects. Either that, or plan for long-term drug therapy for obesity-related disorders such as hypertension, hyperlipidemia, and diabetes. David S. Weigle (University of Washington School of Medicine, Seattle) describes four common misconceptions about obesity: First, "that body weight is not a physiologically regulated variable but is set by acquired food habits and conscious or unconscious desires." Second, "that an individual can set his or her body weight to any desired level without adverse effects on health." Third, "everyone who weighs more than the numbers set forth in certain life insurance tables is at increased risk of mortality." And fourth, "that body fat distribution is not an important health consideration." A look at treatment Risks and popular attitudes At any given time, up to 40 percent of women, 44 percent of high school girls and 24 percent of men are trying to lose weight. Many of these people do not need to lose weight, and of those that do, the majority will fail. But this failure should not lead medical professionals to abandon their obese patients. According to physician Arthur Frank of George Washington University Obesity Management Program, obesity is an orphan disease of monumental size: "We are much better at managing patients even though we are only a little bit closer to understanding the mystery of how we weigh what we weigh."

Sanofi, the French pharmaceuticals firm bidding to swallow its rival Aventis, paraded new evidence for its anti-smoking, anti-obesity drug yesterday in an effort to raise investors' enthusiasm for a takeover. Aventis and Sanofi, which have been locked in a fierce takeover battle since January, took their fight to New Orleans, with each exhibiting its latest scientific advance to the annual gathering of the American College of Cardiology.

Medications That Promote Weight Loss

Most available weight-loss medications are "appetite-suppressant" medications. Appetite-suppressant medications promote weight loss by decreasing appetite or increasing the feeling of being full. These medications decrease appetite by increasing serotonin or catecholaminetwo brain chemicals that affect mood and appetite. In 1999, the drug orlistat was approved by the Food and Drug Administration (FDA) as an obesity treatment. Orlistat works by reducing the body's ability to absorb dietary fat by about one third. The association between obesity and blood clots is well known; but the cause has remained a mystery. Now, new research with mice conducted by scientists at the University of Michigan Medical School and published in the April 3 issue of the Journal of the American Medical Association indicates that leptin may be responsible.
Knowing how to block this leptin-receptor interaction could help prevent heart attacks and strokes in people who are either obese or overweight, which is half the adult population of the United States.
