Ten of the studies reviewed in this section are school-based physical activity interventions (Tables 4 and 5); three are community-based interventions (Table 6). All of the studies quantitatively measured physical activity and included a control group. Readers are referred to Stone et al. (82) for a more complete review. The most common intervention site is the school because it provides easy access and the opportunity to reach many children, especially if resources are limited. Most of the school-based interventions reviewed here included both a physical activity and a nutrition component. The majority worked through physical education classes, which were altered to increase the amount of time spent being active and the intensity of the activity. Most of the interventions also modified school lunch programs to reduce energy, fat, and sodium content. The elementary school interventions generally succeeded in increasing activity in the modified physical education classes (25) (49) (71) (77) (Table 4). Some, but not all, also increased children's out-of-school physical activity. The Cardiovascular Health in Children (CHIC) study (36) found that self-reported total physical activity increased, but other studies (49) (71) found no change in out-of-school physical activity. Donnelly et al. (25) reported a decrease in out-of-school physical activity in fifth graders, although activity increased in physical education classes. Even when a family component was added to a school-based intervention, the Child and Adolescent Trial for Cardio-vascular Health (CATCH) program (49) found no increase in out-of-school physical activity, compared to the school-only intervention.

NUTRITION EDUCATION IN SCHOOLS

Background The goal of nutrition education is to motivate participants to eat a healthy diet. Children are a very important audience for nutrition education because a healthy diet is essential for their normal growth and development, and because children are establishing food patterns that carry into adulthood. Good nutrition promotes not only better physical health and reduced susceptibility to disease, but has also been demonstrated to contribute to cognitive development and academic success. Left to their own devices, children will not automatically select healthy foods. Their innate preference for sweet foods makes them particularly vulnerable to the highly sugared cereals, soda, and candy that are marketed to them virtually from birth. In order to develop lifelong healthy eating patterns, children need to be introduced to a variety of nutritious foods in a positive manner.

Background The benefits of a diet low in fat and high in fiber and fresh fruits and vegetables in promoting health and reducing chronic disease have been well established. However, food choices and dietary habits are influenced not just by a consumer's knowledge, but also by a number of factors in the social and physical environment. Many food choice decisions are made at the point of purchase in grocery stores, cafeterias, and restaurants. Nutrition research suggests that price, in addition to factors such as taste preferences, health concerns, and habits, significantly affects dietary and purchasing habits. In a recent survey in California, about one third of individuals surveyed cited expense as a barrier to eating low-fat food and fresh fruits and vegetables. Data from other fields, such as tobacco and alcohol control, also highlight the price sensitivity of consumers, especially adolescents, who reduce their purchases when the cost of a product increases. While some research shows that low-fat diets do not have to cost more, it is more difficult to eat healthily while keeping food expenditures low when people are cooking less and purchasing more prepared, convenience, and snack foods. In addition, high-fat, high-sodium fast foods are frequently priced extremely low compared to other restaurant options.

The Minnesota Obesity Center is an Obesity Nutrition Research Center (ONRC) funded by the National Institute of Diabetes, and Digestive and Kidney Diseases (NIDDK) of the National Institutes of Health. The mission of the Minnesota Obesity Center is to find ways to prevent weight gain and secondarily the onset of obesity and complications of obesity.
The ONRCs are part of a core center grant program established by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) to advance obesity and nutrition research. The CNRUs are part of the program established by NIDDK and the National Cancer Institute to bring together basic science and clinical investigators in a manner that enriches the effectiveness of nutrition research. Each of the ONRCs and CNRUs has made and continue to make significant advances in the areas of obesity and clinical nutrition.
