General Concepts
American Trypanosomiasis (Chagas Disease)
Clinical Manifestations
Symptoms of acute disease may include fever, local or general edema, lymphadenopathy, tachycardia, heart enlargement, and myocarditis. Heart alterations and, occasionally, megaesophagus or megacolon may appear as late sequelae.
Structure
Typical, small trypomastigotes are found in peripheral blood and intracellular amastigotes in tissues.
Classification and Antigenic Types
Strains of Trypanosoma cruzi are differentiated by isoenzyme patterns and DNA sequencing. No antigenic variation is observed.
Multiplication and Life Cycle
Intracellular amastigotes divide to form pseudocysts, which release nondividing trypomastigotes into the blood. Trypomastigotes ingested by a vector bug transform in the insect intestine into epimastigotes, which reproduce to form infective metacyclic trypomastigotes, which are expelled in feces and enter a new host through skin abrasions.
Pathogenesis
Inflammatory reactions around pseudocysts lead to myocarditis and destruction of parasympathetic ganglia (mainly of the heart and myenteric plexus). An autoimmune reaction may develop.

A note about your final grades
Your final grades were calculated and verified by me and the two laboratory instructors. In the unlikely event that you discover a mistake in the computation of your final grade, a grade change will be considered. It is futile to contact me for explanations of "curves" and for discussions of personal problems that may have affected your performance. In a class having more than 80 students, there are invariably a number of misfortunes, including students who are victims of accidents or who learn that they (or members of their family) have cancers, require major surgery, etc. In cases involving serious disruptions, a request of an "incomplete" along with proof of the cause of the disruption, well before the end of the quarter, is advisable. A grade is earned, not granted. It certifies a level of understanding of the subject matter. It bears little relationship to the performance of the other students, and it has no relationship whatsoever to a student's personal problems or to his/her future plans. Anything else would devalue the reputation of the University.

T he early decades of the 20th century witnessed a major revolution in the understanding and treatment of mental diseases. Until then, people with psychoses were usually locked away in insane asylums, receiving only limited custodial care and sometimes social support, with practically no effective therapeutic options left to the alienist, as psychiatrists were called then. Although well-intentioned medical reformers such as Phillipe Pinel eased the nightmarish conditions of insane asylums in force well into the first half of the 19th century, no treatments were generally performed. The first revolution was put in motion by scientific psychotherapy, based on theories of the mind proposed by Austrian physician Sigmund Freud , the founder of psychoanalysis. Its value was mainly observable in mild mental disturbances, particularly neuroses. But beginning in the 1930s these methods began to be supplemented by physical approaches using drugs, electroconvulsive therapy, and surgery. The knowledge that head trauma, convulsions and high fever could be good for improving mental disturbances is not new in Medicine.
