  • Before distributing the Release, fill in the blanks for program name and dates as appropriate. Be sure to include the examples of significant, non-obvious risks. Be specific about possible injuries or even death.
    Distribute the release sufficiently in advance of the activity or event to give each prospective participant and or parent/guardian a change to read it and understand the content.
    In any oral or written communications accompanying the Release, make clear that the Release is voluntary (in the sense that the participant is not required to participate in the underlying activity) not the content of the Release.
    Give participant and or parent/guardian an opportunity to ask questions about the Release.
    Be sure you get back and keep in your files the original, signed release from each participant. The participant or parent/guardian should have a copy of the release.

    Maryland General Hospital is a 300-bed community teaching institution that serves as the cornerstone of Maryland General Health Systems, Inc., a comprehensive network covering the continuum of care needs for more than 110,000 patients annually. Our History Maryland General was founded by a group of local physicians as a university affiliated teaching hospital in 1881. In 1911, the Baltimore Medical College, as MGH was then known, merged with the University of Maryland School of Medicine and began its long tradition of medical training and patient care that continues today.     Infants and Toddlers
    The Individuals with Disabilities Education Act (IDEA), authorized in 1975 and reauthorized most recently in 1997, supports efforts to provide coordinated service delivery systems for children with disabilities from birth through age 5. Two programs that provide support to Infants and Toddlers are the Early Intervention Program for Infants and Toddlers with Disabilities (Part C) (from birth through age 2), and the Preschool Grants Program (Section 619 of Part B) (ages 3 through 5). Children are best served within the context of their families. Services for infants and toddlers are designed to identify and treat a problem or delay as early as possible. You may receive services in different settings, such as your home, a clinic, hospital, daycare center or the local health department.

    Infants and Toddlers is a system of early intervention and supports, offered by public and private agencies through out the state to young children with developmental delays and their families.     About the Program Quality Child Care for Healthy Families
    The University of Maryland School of Nursing, a nationally recognized top ten School of Nursing has been awarded a grant by the Department of Human Resources Child Care Administration to develop and implement a new state-wide system of health care consultation and training for child care providers as part of the national campaign by Healthy Child Care America to promote health and safety in the child care environment. Healthy Child Care Maryland was formally launched 2 years ago with three main objectives:
    • Increase access to health screenings Increase the availability of training for child care providers that enhances their ability to identify the need for child/family health and social support services, articulate concerns to parents, help families access services and care for children with special health needs

    The Healthy Child Care Maryland pilot program was developed as a first step in implementing the above objectives. The goals of the Healthy Child Care Maryland Program are consistent with Action steps 5 and 9 of the HCCA Blueprint for Action:

    The Breast and Cervical Cancer Screening Program (BCCP)
    In 1992 the Maryland Department of Health and Mental Hygiene received a grant from the federal Centers for Disease Control and Prevention to provide breast and cervical cancer screening services to low income, uninsured or underinsured women. In 1997, the grant was renewed for an additional five years. The Maryland legislature, in 1998, made state funds available to supplement the CDC grant. These funds will pay for an annual clinical breast examination, annual mammogram and pap smears, as needed, for women 40 -64 years of age and for women over 65, if they do not have Medicare Part B. Participants in the program must be Maryland residents with family incomes less than 250% of the federal poverty level and not have insurance with screening benefits.

    The MDC-LARP is a collaboration between the Maryland Higher Education Commission (MHEC) and the Department of Health and Mental Hygiene, Office of Oral Health (OOH). Under MHEC, the State Scholarship Administration collects all applications, convenes the review panel, and distributes the loan repayment checks. OOH monitors award recipient to confirm they are meeting their obligations promotes the program, gathers feedback from award recipients, and conducts initial orientation with award recipients.

In a short period of time, the Baltimore program was serving over 60 kids. Maryland's programs have expanded to include sites in Lanham, Frostburg and the District of Columbia and server over 150 children and families daily. In 1998, PRYDE Baltimore became one of the first treatment foster care programs in Maryland. PRYDE's mission is to serve seriously emotionally disturbed children by providing them with a highly structured treatment foster home. The PRYDE program is guided by three core values:

Family Leadership and Resource Management University of Maryland Program Title: Volunteer Financial Counseling Service Description: Maryland Families continue to experience financial stress due to unemployment , underemployment, sporadic income, illness, death, divorce, and misuse of credit. Since 1967 volunteers have been trained as counselors to assist clientele in assessing their financial situation, exploring available options and choosing appropriate options to establish financial stability. Debt reduction is the most frequent reason for seeking assistance, however clientele do also request help in establishing saving programs, retirement planning, and cutting insurance costs. Audience: As the FCS program has evolved , the counselors trained have become diverse in experience and motivation. As they are trained in an 18 to 24 hour course, they learn skills from basic budgeting to debt resolution. Evaluating insurance, credit use, saving, and banking, are taught as well as counseling skills. At the request of Military Family Services, social agencies, local housing authorities, financial institutions and churches , our training opportunities have expanded to include less traditional volunteers. Clientele served by these expanded volunteers include those in crisis: facing eviction , utility cut-off, judgements, repossession, etc. as well as those who wish to "do a little better job of managing their money". Clients are refered by churches, lending institutions, employers, lawyers, and parents.
