For the Patos, the genetics of psychiatric maladies-most recently bipolar disorder-and the country of Portugal-specifically its island group called the Azores, a thousand miles into the Atlantic off the Portuguese coast-are intimately tied together. Current treatments involve mood-stabilizing drugs like lithium carbonate and depakoate for the manic (high) phase, and antidepressants for the bottom of the roller coaster. But the treatments aren't 100 percent effective, come with serious side effects, and certainly are not a cure.

Tuesday, April 27, 1999; Page Z07 Scientists believe they have found a surprising new ally in their efforts to understand and treat the sharp mood swings of manic depressionthe fatty acids of fish oil. A Harvard University clinical trial of 44 patients suffering from manic, or bipolar, depression had such positive results with fish oil that the experiment was stopped after four months and all patients were put on a treatment of 14 capsules per day. "The group taking the fish oil was performing strikingly better than the placebo group, including significantly longer periods of remission," said Andrew L. Stoll, director of the Psychopharmacology Research Laboratory at Harvard Medical School/McLean Hospital. "A decision was made to stop the trial on ethical grounds." Based on those promising findings, Stoll said, the National Institutes of Health (NIH) has given preliminary approval for a larger fish oil trial starting this summer. That trial, at McLean and Baylor College of Medicine in Houston, would include 120 people suffering from manic depression and would last for three years. "If this works, it would be one of the most exciting findings in psychiatry in the past 20 years," said Jerry Cott, chief of the psychopharmacology research program at the National Institute of Mental Health. "This is the first time we would be testing a nutritional supplement that appears to be having efficacy about to the degree of a synthetic medication."

Ups, Downs and In-Betweens: Manic Depression and Bipolar Disorder, One and the Same
Because the terms “bipolar disorder” and “manic depression” are often used independently in the popular media, it is a fairly common presumption that they describe two separate conditions of ill health. But bipolar disorder and manic depression are the same thing, nomenclature notwithstanding. This insight and others about the illness are offered by Robert C. Risinger, MD , Medical College of Wisconsin Assistant Professor of Psychiatry and Behavioral Medicine. “Bipolar disorder is the propensity to go through periods of depression and periods of mania, which is why it’s called bipolar,” said Dr. Risinger. “There are two extremes, depression and mania. “Manic depression is what it’s traditionally been called. In the DSM (Diagnostic and Statistical Manual of Mental Disorders), it’s Bipolar Disorder. Manic depression is simply more descriptive. There are frantic ‘manic’ episodes and after there are also low ‘depressions,’ which characterizes the cycling nature of the illness. It’s the same thing, absolutely the same thing, as bipolar disorder.”

    Women with manic depression more vulnerable to alcohol problems Reported by Susan Aldridge, PhD, medical journalist A new study reveals that a woman with bipolar disorder is seven times more likely than average to have alcohol problems.
    Alcohol abuse is known to be more common among people with mental disorder. They may drink to treat their symptoms and there may be a family history of such problems. Researchers at the University of California, Los Angeles, have learned more about the incidence of alcohol problems in men and women with bipolar disorder - also known as manic depression. Bipolar disorder is marked by severe mood swings - from wild euphoria to deepest depression. In this study, 49 per cent of men with bipolar disorder were affected also by alcoholism compared to 29 per cent of women. For men, this is a threefold increase of the risk of alcohol problems compared to men without bipolar disorder. For women, it's a seven fold increase in risk. The researchers also found that men with bipolar disorder and alcohol problems were likely to have a family history of the conditions. Women with bipolar disorder were more likely to drink to forget their problems. The study underlines the importance of monitoring alcohol consumption among women with bipolar disorder.

    As with depression and schizophrenia , there is also a genetic element to manic depression. In fact, there is a very strong genetic influence, for example, if you are an identical twin and have manic depression then there is a 75% chance that your twin will also suffer from the illness. If your mother or father or brother or sister has manic depression then there is a 10% chance of you becoming ill. If a more distant relative has the illness then there is little, if any, increased risk that you will suffer from it.
    Manic depression and diet
    Food allergies may cause severe mood swings in some people. This could provide some support into the theory that manic depression may be helped by changes in diet. If you suffer from manic depression, a well-balanced, healthy diet could help your body to fight off mood swings more effectively. Some people claim that deficiencies in Vitamin B complex, B1 (Thiamin), B6, B12, folic acid, magnesium, calcium, manganese and zinc have all been linked to mood disorders, and that taking food supplements and vitamin pills can help.

    In the normal course of life, everyone has times when they feel somewhat less than happy and optimistic. Although many people attribute their feelings of sadness or lethargy to the onset of depression, it's important to understand that medical professionals have specific indicators that they use to make a diagnosis of clinical depression. When you know more about what's wrong you can make it right.
    Let's start with the word "clinical." By definition, a condition is clinical if it is directly observed. That is, the professional does not count on a patient's self-report but is more likely to check for symptoms of depression that can be observed and documented. Clinical depression is a phrase used to describe a condition serious enough to require clinical (professional) and possibly pharmacological intervention. Clinical depression, according to medical sources, lasts more than two weeks and is usually not precipitated by anything external.
    For example, if you've just lost your job and have no money in your bank account, you may feel stressed, anxious, panicked, and unable to function. However, most of your friends and family would reasonably conclude that it's quite normal to feel that way. Although your reaction might match the symptoms of depression, a traumatic event precipitated them and your state of anxiety would not be diagnosed as clinical depression. Clearly, a new and better job would probably end the negative feelings in an instant.
