Support Groups Bipolar disorder (also known as manic depression) is a treatable illness marked by extreme changes in mood, thought, energy and behavior. It is not a character flaw or a sign of personal weakness. Bipolar disorder is also known as manic depression because a person’s mood can alternate between the "poles" mania (highs) and depression (lows). This change in mood or "mood swing" can last for hours, days weeks or months. Bipolar disorder affects more than two million adult Americans. It usually begins in late adolescence (often appearing as depression during teen years) although it can start in early childhood or later in life. An equal number of men and women develop this illness (men tend to begin with a manic episode, women with a depressive episode) and it is found among all ages, races, ethnic groups and social classes. The illness tends to run in families and appears to have a genetic link. Like depression and other serious illnesses, bipolar disorder can also negatively affect spouses and partners, family members, friends and coworkers.

  • What is a depressive disorder? What are the types of depression? What is a depressive disorder?
    Depressive disorders have been with man since the beginning of recorded history. In the Bible, King David, as well as Job, suffered from this affliction. Hippocrates referred to depression as melancholia, which literally means black bile. Black bile, along with blood, phlegm, and yellow bile were the four humors (fluids) that accounted for the basic medical physiology of that time. Depression has been portrayed in literature and the arts for hundreds of years, but what do we mean today when we refer to a depressive disorder? In the nineteenth century, depression was seen as an inherited weakness of temperament. In the first half of the twentieth century, Freud linked the development (pathogenesis) of depression to guilt and conflict. John Cheever, the author and a modern sufferer of depressive disorder, wrote of conflict and experiences with his parents as influencing his development of depression. In the 1950's and 60's, depression was divided into two types, endogenous and neurotic. Endogenous means that the depression comes from within the body, perhaps of genetic origin, or comes out of nowhere. Neurotic or reactive depression has a clear environmental precipitating factor, such as the death of a spouse, or other significant loss, such as the loss of a job. In the 1970's and 80's, the focus of attention shifted from the cause of depression to its effects on the afflicted people. That is to say, whatever the cause in a particular case, what are the symptoms and impaired functions that experts can agree make up a depressive disorder? Although there is some argument even today (as in all branches of medicines), most experts agree that:

Dysthymia
Dysthymia is a less severe type of depression. It involves long-term (chronic) symptoms that do not disable, but yet prevent the affected person from functioning at "full steam" or from feeling good. Sometimes, people with dysthymia also experience episodes of major depression. This combination of the two types of depression is referred to as double-depression. Bipolar Depression Disorder (Manic Depression)
Another type of depression is bipolar disorder, which was formerly called manic-depressive illness or manic depression. This condition shows a particular pattern of inheritance. Not nearly as common as the other types of depressive disorders, bipolar disorder involves cycles of depression and mania, or elation. Bipolar disorder is often a chronic, recurring condition. Sometimes, the mood switches are dramatic and rapid, but most often they are gradual. When in the depressed cycle, the person can experience any or all of the symptoms of a depressive disorder. When in the manic cycle, any or all of the symptoms listed under mania may be experienced. Mania often affects thinking, judgment, and social behavior in ways that cause serious problems and embarrassment. For example, unwise business or financial decisions may be made when an individual is in a manic phase.

Manic-depressive illness has been with human beings since the beginning of recorded time. Throughout the centuries effective treatment of manic-depressive illness has been a formidable and largely unmet challenge. It is only in recent decades that certain discoveries have influenced diagnostic and treatment practices in such a way as to diminish the severity and frequency of manic-depressive episodes and in some cases, prevent such episodes altogether! Among many important findings in the continued research on manic-depressive illness is the fact that the education of patients and families is a critical factor in determining treatment outcome. With this in mind, this booklet is offered as a vehicle by which to start the all-important educational journey.

Manic depression causes mood swings US scientists have identified a gene mutation that causes manic depression in as many as one in 10 patients. The gene, GRK3, plays an important role in regulating the way the brain responds to chemicals such as dopamine which are known to control mood. The mutation, discovered by a team from the University of California, San Diego, occurs in the gene's on-off switch. Our hope is that discovery of genetic defects that cause bipolar disorder will lead to new drugs that can be directed to those specific genes

One in 100 people has bipolar disorder
A drug currently used to treat schizophrenia could also help millions of people with manic depression. Results from a major clinical trial suggest quetiapine is an effective treatment for those with bipolar disorder. The drug, which is manufactured by AstraZeneca, and sold under the brand name Seroquel, works well if it is used in combination with mood stabilising medication.

Dr Tony Cleare, Institute of Psychiatry Experts said the trial paved the way for the drug to be used as an alternative treatment for people with manic depression. Professor Gary Sachs from Harvard Medical School in the United States who headed the study said the drug was more effective than some existing treatments. Existing drugs "Current treatment options for patients with bipolar disorder are limited. Commonly used therapies involving mood stabilisers are not effective for all patients and can be associated with troublesome side effects," he said.

Bipolar Disorder - episodes of mania.
Any discussion of the process of bipolar disorder and mania cannot be entirely divorced from a discussion of the depressive response. Within any episode of mania lurks the possibility, the likelihood of impending depression. There are certain techniques and exercises which I have identified while dealing with my own periods of bipolar disorder mania which help temper the mania and thereby lessen the impact and severity of the subsequent depressive response. It is essential that you have read my article on Emotional Thought Stopping and are familiar with the concept and my suggestions for its use, before you attempt this exercise. I assume that if you are reading this in order to help temper your periods of bipolar disorder mania that this is not your first episode. Look back at your previous situation just prior to those episodes and see if you can identify if there was some event which triggered the episode and begin to identify how you felt at the start of the episode. It is important to be able to recognize these periods as early as possible so that you can begin this tempering exercise before it reaches a crisis stage. This is especially true if you begin your episodes with a euphoric feeling.

What is Manic Depression?
Manic depression involves wide mood alterations, with periods of both depression and mania. A person experiencing depression or mania may have intense mood swings and consequent changes in thinking and behaviour. The term that is professionally used for this illness is Bipolar Affective Disorder. Bipolar means sharing two poles (high and low) and Affective Disorder means a disorder having to do with mood.
What Causes Manic Depression and Who Are The People At Risk?
The cause is not known. Any person can develop manic depression, however, studies indicate that highly creative, sensitive people, people tending to be perfectionists and high achievers, have a higher incidence of bipolar affective disorder. Although biological factors seem to play a major role in producing the illness, a person's personality make up and or stresses in the environment may also play a part in bringing on an acute episode.
How Many People Are Affected and What Are The Symptoms?
It is estimated that 1% of the population will have a manic depressive illness. Environmental factors such as death, separation and divorce may trigger the disorder. The illness manifests itself with the individual experiencing episodes of mania or elation followed by low mood or depression. The number of manic and depressive episodes varies greatly from person to person and most individuals experience "normal" periods between their manic and depressive episodes.
