A M ANIC D EPRESSION P RIMER
by Dimitri Mihalas During graduate school in the late eighties, I had the privilege of meeting Dimitri Mihalas, then a distinguished professor of astronomy at the University of Illinois at Urbana-Champaign. Dimitri is a leading astrophysicist of our times and a member of the prestigious U.S. National Academy of Sciences. He is now at the Los Alamos National Lab in New Mexico. Though he suffers from the illness, he feels that he has actually gained from it instead of losing to it. He has also been a pioneer in attempts to increase public awareness of (and therefore decreasing the stigma associated with) the bipolar illness by the act of being completely open about it. Soon after a major, life-threatening episode of depression (which was treated successfully with medication), he set upon himself the task of composing a primer on manic-depression. Because of his openness, the primer is quite personal and many have thus found it to be useful in gauging their own experience with the illness. It also contains a great deal of useful information, particularly about the spiritual aspects of recovery, and contains a bibliography for those who want to learn more. Someone who read it described it as a "life saver" for her.

FACTS ABOUT MANIC DEPRESSION
from the flyer of the same name sponsored by the Missouri Council for Comprehensive Psychiatric Services Roller coaster rides of emotion from frantic highs to devastating lows are the classic signs of manic-depressive illness. Also known as bipolar disorder, manic depression is one of the most treatable mental illnesses, but left untreated it can cause mental suffering, disruption of family life, poor job performance, and reckless or dangerous behavior.
Symptoms
A person with manic-depressive illness experiences mood swings from mania to depression, with a "normal" period between these cycles of up and down. The length of the cycles varies from a few days to several months and they can occur without warning. During the manic phase, a person may feel "on top of the world" and have an abundance of energy. He seems to talk and think faster and espouse a number of ideas. He also may think he is invincible, leading to reckless behavior and acts that may endanger his life or well-being. A person in the manic phase also may have delusions of fame or believe she has a special relationship with a famous person. During this phase she also sleeps less, is easily distracted, and often is irritable. During the depressive phase, a person may feel hopeless and lose all interest in other people or usual activities. Her weight may fluctuate and she feels tired all the time. She may sleep more than usual or have insomnia. She often complains of unexplained aches or pains and has trouble concentrating. A person in the depressive phase is a suicide risk.

What is it?
What is manic depression?
Manic depression is a serious mental health problem involving extreme swings of mood (highs and lows). It is also known as Bi-polar Affective Disorder. Both men and women, of any age from adolescence onwards and from any social or ethnic background, can develop manic depression. It often first occurs when work, study, family or emotional pressures are at their greatest. In women it can also be triggered by childbirth or during the menopause. The illness is episodic (occurs in phases). It is possible to remain well for long periods. Typically the key to coping with manic depression is an early diagnosis and acceptance of the condition. From this point, self-management, health care, therapy and medication can be taken up as appropriate. Severe and/or untreated episodes of manic depression can be very damaging for all affected.
Mania (high)
Someone experiencing mania may not recognise it is happening. Incoherent, rapid or disjointed thinking or being easily distracted are common features of an episode of mania. Other symptoms may include paranoia and hallucinations affecting vision, hearing or perception. Grandiose delusions or ideas can occur where a sense of identity and self has been distorted by the illness. Sometimes the term psychosis (losing touch with reality) is used to describe these symptoms when they are severe.
Depression (low)
Most people who have a diagnosis of manic depression will experience a severe depression at some time. Usually this will follow an episode of mania where the trauma of the manic episode can sometimes compound the depression. For some people depression will be more likely to occur in the winter months. Symptoms, which are commonly experienced, include a feeling of emptiness or worthlessness (as opposed to sadness), loss of energy and motivation for many (or all) everyday activities, pessimism and negativity about most things (or everything). Thoughts of death and suicide can be common but may be hard to discuss.

About bipolar disorder / manic depression
Manic-depression is a chronic mental illness, a physical disease of the brain. There is no known cause, but there is a proven hereditary influence. Many bipolar people have parents or extended family who suffered from depression, hyperactivity, rages, suicide or any number of behaviors. It is not a condition that can be cured but it can be controlled by medication. If you think you may have bipolar disorder, go and see a doctor immediately. (See also "Bad Times and Blessed Times" , a testimony of one person with bipolar disorder.)
Symptoms
There are two aspects of the bipolar disorder. Manic "highs" and depression "lows." Often these are called mood swings. The manic state is the high part of the cycle. It is characterized by hyper or excentric behaviors:
  • euphoria; feelings of joy or wellbeing grandiose thinking; able to accomplish anything
