India's malnutrition 'crisis'
Women and children are the worst sufferers

The World Bank has said malnutrition affects huge numbers of people in India, especially women and children, despite decades of often effective government action.
Its report "Wasting Away - The Crisis of Malnutrition in India" blames poverty and low status of women for some of its most shocking statistics - half of all children under four are malnourished, it says, and 60% of women are anaemic.
Mothers and sisters often forego food in poor families to give husbands and sons more than their share, the report says.
One of the authors of the report, Mira Chatterjee, says malnutrition on such scale means money invested in education is not used effectively as hungry children cannot study. Economic cost The report estimates that malnourished workers mean an annual loss of $10 bn in lost productivity. The green revolution helped Indian food production It praises India's effort to feed its people through agricultural innovation and poverty alleviation schemes, but says population growth, high prices and unemployment often offset well intentioned government programmes.

The number of overweight people in the world now equals those made thin by hunger, but both groups are malnourished according to a study of obesity and inadequate diet.

Gary Gardner The study, Underfed and Overfed: The Global Epidemic of Malnutrition, is published by the Worldwatch Institute, based in Washington, DC.
The authors say that while the world's underfed population has shrunk slightly to 1.2 billion, the number of overweight people has risen to the same figure. They say obesity cost the US 12% of the national health care budget in the late 1990s - $118bn - more than double the $47bn attributable to smoking. They define both the overweight and the underweight as suffering from malnutrition. . . . and so does obesity, another form of malnutrition

This document presents the official recommendations of the American Gastroenterological Association (AGA) on the management of malnutrition and cachexia, chronic diarrhea, and hepatobiliary disease in patients with human immunodeficiency virus infection. It was approved by the Patient Care Committee on July 15, 1996, and by the AGA Governing Board on July 27, 1996. H uman immunodeficiency virus (HIV) infection is a devastating illness with major public health implications throughout the world. Gastrointestinal and hepatobiliary complications are common in patients with acquired immunodeficiency syndrome (AIDS) and affect most patients at some point during the course of the disease. Gastrointestinal complications are important not only because of their frequency but because they result in significant morbidity, mortality, and use of health care resources. Management of these patients provides a challenge to the practicing physician for several reasons, including the atypical presentations, the broad spectrum of potential causes, and the multisystem nature of these complications. In addition, because these complications often occur in patients with advanced disease, ethical considerations must be taken into account. The following guidelines provide recommendations for the management of HIV-infected patients with malnutrition and cachexia, diarrhea, and hepatobiliary diseases. These guidelines are based on a systematic, comprehensive review of the medical literature, which is provided in an accompanying report.

General Description
Malnutrition and starvation are very different. Malnutrition is the inadequate intake of any of the required nutrients. This can even occur if one is receiving large amounts of food, but is not able to ingest, digest, absorb, or utilize this food. Malnutrition also occur if the food is inadequate in one or more of the required nutrients. Malnutrition can also mean over nutrition, an intake in excess of needs, resulting in overweight and obesity. Very high intakes of minerals, fats and vitamins can be toxic.

Possible Causes
Malnutrition can be caused by poor teeth, parasites, disease, foreign bodies in the digestive tract, tumors, or an injury of the digestive tract which reduces absorption of some nutrients from food. Insufficient nutrition can also be caused by disorders like anorexia, bulimia, or other physiological problems. Some medications may affect appetite or the absorption of nutrients from foods. Vegetarian and fad diets contribute to the list of causes of malnutrition.

Possible Symptoms and Complications
Some general symptoms include fatigue, confusion, dizziness, diarrhea, dehydration, weight loss, obesity and decreased immune response. If not corrected malnutrition can lead to changes in biochemical functions and structure. Blood and bleeding disorders develop. In advanced stages the skin becomes dry and scaly. The teeth become lose and gums swell and bleed. Hair becomes dry, brittle and falls out easily. Nails are caved in or spooned. Muscles become weak and tender, eyesight fails. Bones become distorted and joints painful.

Alternate Names : Nutrition - Inadequate Definition Malnutrition means a person's body is not getting enough nutrients. The condition may result from an inadequate or unbalanced diet, digestive difficulties, absorption problems, or other medical conditions.
Food Guide Pyramid Malnutrition can occur because of the lack of a single vitamin in the diet, or it can be because the person isn't getting enough food. Starvation is a form of malnutrition. Malnutrition can also occur when nutrients ARE adequately consumed in the diet, but one or more nutrients are not digested or absorbed properly. Malnutrition may be mild enough to show no symptoms or so severe that the damage it has done is irreversible even though the individual may be kept alive.

GENEVA (Reuters Health) - A United Nations official has charged that a major factor in the HIV/AIDS pandemic in Africa is malnutrition. Speaking here at the annual gathering of the UN's Economic and Social Council, A. N. Ngongi, Deputy Executive Director of the World Food Programme, emphasized that the pandemic has been seriously aggravated by a lack of access to nourishing food that plagues a large percentage of Africa's people. According to the UN Food and Agricultural Organization, Ngongi told Reuters Health, food production in the nine countries most affected by HIV/AIDS has dropped by 15% in the past 10 years. Support for populations affected by the pandemic has tended to be focused on urban areas, he continued, with the result that rural areas have been especially hard hit, to the point where many rural communities have been thoroughly destabilized, with a concomitant drop in food output. Ngongi said that in hospitals he visited recently in Uganda and Kenya, some 60% of the beds were occupied by people infected with HIV who were suffering not from opportunistic infections but from severe malnutrition. Experts estimate, he noted, that 35% to 40% of Africa's people suffer from chronic malnutrition, and the problem is increasing rapidly.

According to the figures, over the last two years chronic malnutrition has declined by 23%, and acute malnutrition has declined by almost 50%. The improvement is visible. At the hospitals I’ve visited, particularly in Central and Northern Iraq, wasting diseases such as kwashiorkor and marasmus are no longer pandemic. And while doctors throughout Iraq continue to report shortages in essential medicines and equipment, pediatric cancers have replaced malnutrition as their chief complaint. Despite these improvements - UNICEF figures show that over 1 in 5 Iraqi children remain malnourished. Our work isn’t over yet. There are several reasons why malnutrition has declined - almost all due to busting sanctions. One reason is, fairly obviously, because more food is available. In December 1999, the UN lifted the limit it had placed on Iraqi oil sales through the Oil-for-Food program, and in early 2000 exempted food from the security review process. This allowed Iraq to import more food, more quickly, and distribute it to families in need. Of the $24.2 billion in supplies Iraq has been allowed to import under the Oil-for-Food program to date, almost $10 billion has arrived in just the last year - allowing the Iraqi government to increase the food ration they provide to everyone in Iraq.

  • Vitamin A deficiency (VAD) leads to increased childhood death from infections and is the most common cause of preventable blindness in children. Approximately 2.8 million preschoolers are at risk of xerophthalmia (the clinical eye lesions in VAD leading to rupture of the cornea and blindness) each year, and it is estimated that between 250,000 and 500,000 preschool children go blind from vitamin A deficiency annually. Approximately 250 million people are at risk of deficiency of vitamin A worldwide, the prevention of which would reduce childhood mortality rates by up to 30 percent.
  • Iron deficiency anemia (IDA) increases the risk of prematurity and low birth weight, and is a major contributor to maternal mortality. Severe anemia may account for one in five maternal deaths. IDA affects an estimated 42 percent of all women, 58 percent of pregnant women, and a third of all young children globally. IDA is a primary cause of cognitive deficit in infants and young children. In adults, physical work capacity is reduced with even moderate anemia.
  • Iodine deficiency disorders (IDD) impair growth, development, and intellectual capacity, and contribute to reproductive problems in women. IDD affects an estimated 655 million people worldwide, and 915 million more people live in areas that put them at risk for IDD. Iodine deficiency is the greatest single preventable cause of intellectual impairment in the world today. It is a significant health problem in 118 countries.
