August 8, 2002 It's clear that weight loss and exercise can help reduce blood pressure, but do they produce enough of a change to reverse physical changes to the cardiovascular system that increase disease risk? They may. A study published in the Archives of Internal Medicine shows that moderate changes to eating and exercise habits produce measurable improvements in left ventricle parameters in hypertensive individuals. Lifestyle interventions Researchers from the University of North Carolina recruited 82 overweight, but otherwise healthy, adults with an average blood pressure of 140/93. Each was randomly assigned to one of three groups. The exercise group performed an aerobic activity (walking, jogging, biking) for 35 minutes, 3 or 4 times per week. The weight control group attended weekly behavior modification classes designed to teach lasting weight management strategies. The control group was wait-listed for treatment. Electrocardiograms performed at baseline and at the end of the study measured left ventricular mass, relative wall thickness, end diastolic dimension, posterior wall thickness, and septal thickness. Effect of weight loss and exercise Clinical implications Other studies have shown that medication-induced reductions in blood pressure result in improvements in left ventricular hypertrophy. These results, however, show that similar results can be achieved with weight loss and increased physical activity - the type of lifestyle changes that physicians are likely to recommend to their mildly hypertensive patients as a reasonable first line of treatment.

August 24, 2000 (Reviewed: December 13, 2002) Dieting and exercise are often the first treatments prescribed for people with mild or moderate high blood pressure (hypertension). A new study published in Archives of Internal Medicine provides further proof that that exercise, especially in combination with weight loss, can help keep high blood pressure under control. This study divided 133 sedentary, overweight men and women with mild hypertension into two groups. One group cycled, walked or jogged for 30 minutes, three to four times per week while following their usual diet. The second group followed the same exercise routine and also participated in an organized weight loss program. At the end of the six-month study, researchers found exercise alone significantly lowered blood pressure. Those that combined exercise and weight loss, though, showed an even greater reduction in blood pressure and lost an average of 7.8 kg (17 lbs) of excess weight. Among the latter group, 67% of those classified as mildly hypertensive at the start of the study had normal blood pressures after six months.

The benefits of the DASH diet seem to be most noticeable in people sensitive to the blood pressure effects of salt. The study included 375 adults with normal to moderately elevated blood pressure. They were randomly assigned to consume a DASH diet or a control diet for three consecutive 30-day periods. During each of those 30-day periods, they had different levels of salt intake. People on the DASH diet excreted salt easier and in greater amounts and also had reduced blood pressure. The study also found the impact of the DASH diet decreased as the level of salt intake decreased.

Summary
Background: The Sixth Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure placed increased emphasis on lifestyle modification for the prevention and management of hypertension. The Dietary Approaches to Stop Hypertension (DASH) diet, rich in fruits, vegetables, nuts, and low-fat dairy foods, with reduced saturated and total fats, was found in clinical trials to lower blood pressure substantially and significantly. The DASH diet appears appropriate for use in the primary care setting, although it is unknown whether results will mirror those found in clinical trial. Methods: A review of the literature of successful physician-based dietary interventions and of the Stages of Change model as it applies to dietary behavior was completed. Some changes needed to adapt the DASH diet to the outpatient family practice setting were identified and implemented among a predominantly non-Caucasian (56%), female (61%) population. The most common concerns and diagnoses among this population are essential hypertension, diabetes, and general medical examination. Results: Conclusions: The DASH diet can be used successfully by patients in the primary care setting to lower blood pressure. The challenge of incorporating this intervention into primary care by more practitioners remains. The challenges for the patient and provider to sustain lifestyle modifications are formidable and also continuing.

One of salt's major functions is to regulate blood volume and pressure including the flexibility of the blood vessels. The human heart is a big pump. When it contracts, it forces blood through the arteries of the circulatory system; that pressure is "systolic," the "top" number. Between heartbeats, the heart relaxes.
