Shingles (otherwise known as Herpes Zoster) is a painful, blistering rash caused by the chickenpox (varicella) virus, which affects only a limited area of skin, and makes you feel surprisingly tired, run down, and even depressed.
Symptoms
You may feel slightly unwell, and develop a localised area of pain and tenderness a few days or sometimes up to two weeks before the rash appears. The rash starts off as red spots, which quickly turn into blisters. They always affect only one side of the body (left or right) and never cross the midline. This is because they come out on the area of skin which is supplied by one particular nerve. The rash may affect any part of the body, including head and limbs. It may thus appear as a band around one side of the chest or abdomen, or down an arm or leg. It may affect the head, and when it affects the upper cheek or the side of the forehead it may also affect the eye. You should certainly see your doctor if you have shingles affecting the side of the head, and especially if it seems to affect the tip of your nose or the eye itself. It is usually a very painful rash, and typically people can't bear clothes touching the affected area.

GENITAL HERPES What is genital herpes? Genital herpes is a contagious viral infection that affects an estimated 45 million Americans (more than one in five). Each year, as many as 1 million new cases are believed to occur. The infection is caused by the herpes simplex virus (HSV). There are two types of HSV and both can cause the symptoms of genital herpes. HSV type 1 most commonly causes sores on the lips (known as fever blisters or cold sores), but it can cause genital infections as well. HSV type 2 most often causes genital sores, but it also can infect the mouth. The virus remains in certain nerve cells of the body for life, causing periodic symptoms in some people. Many people who are infected with HSV never develop any symptoms. How is genital herpes transmitted? The infection is usually acquired by sexual contact with someone who has genital herpes. People with oral herpes can transmit the infection to the genital area of a partner during oral-genital sex. Herpes infections also can be transmitted by a person who is infected with HSV but has no noticeable symptoms. Such asymptomatic shedding of the virus may be fairly common, occurring from 5 percent to 20 percent of the time in infected individuals. What are the symptoms of genital herpes?

Der erste Kontakt mit dem Virus verläuft meist unbemerkt und oft ohne Krankheitszeichen. Die eindringenden Viren lösen die Bildung neutralisierender Antikörper aus. Vom Ort der primären Infektion gelangen sie über sensorische Nerven zum zugehörigen Ganglion (Nervenknoten) und lösen dort ebenfalls eine Infektion dieser Nerven aus. Während der ersten Woche werden sowohl die Viren in der Schleimhaut eliminiert als auch in einem Teil der infizierten Nervenzellen. In Form seiner DNA kann das Virus jedoch für das Immunsystem unerkennbar und daher unangreifbar in den überlebenden Nerven überdauern. Diese infizierten Zellen dienen als lebenslanges Erregerreservoir. Sobald das Immunsystem des Infizierten geschwächt ist, kommt es zu einer erneuten "Reinfektion", den so genannten endogenen Rezidiven.

Begleitende schmerzhafte Nahrungsaufnahme und Mundgeruch. Die Erosionen heilen ohne Behandlung innerhalb von 2-3 Wochen von selbst ab. Als Komplikation kommt die Ausbreitung der Erosionen auf die angrenzenden Schleimhäute des Rachens und der Speiseröhre vor. Ferner ist die Ausbreitung auf die Gesichtshaut, durch Daumenlutschen auf das Nagelbett (Paronchyia herpetica) und die Besiedlung von Ekzemen bei Allergikern möglich. Ein besonders ausgedehntes Befallsmuster, welches zusätzlich Finger und Genitalbereich erfasst, wird als "Aphtoid von Pospischill und Feyrter" beschrieben. Bei Kleinkindern kann die Mundfäule (Stomatitis aphtosa) mit schwerem Krankheitsgefühl, akutem Fieber und Schwellung regionaler Lymphknoten einhergehen.

    Screening for Genital Herpes Simplex RECOMMENDATION
    Routine screening for genital herpes simplex virus (HSV) infection by viral culture or other tests is not recommended for asymptomatic persons, including asymptomatic pregnant women. There is insufficient evidence to recommend for or against the examination of pregnant women in labor for signs of active genital HSV lesions, although recommendations to do so may be made on other grounds (see Clinical Intervention). See Chapter 62 for recommendations on counseling to prevent sexually transmitted diseases. Burden of Suffering
    Accuracy of Screening Tests

    History and physical examination are not adequate screening tests for either active (i.e., transmissible) or latent genital HSV infection, because most infected persons are asymptomatic;4,5,15 their clinical manifestations may resemble a number of other causes of genital ulcerations;16 and viral shedding in association with recurrent disease may be asymptomatic.3,17 Antibody testing can accurately distinguish HSV-seropositive from HSV- seronegative persons and therefore may be useful to detect asymptomatic carriers at potential risk for transmitting disease, as well as persons susceptible to primary infection. Commercial assays are insensitive to recent infections, however, and they are unreliable for distinguishing HSV-2 from HSV-1 antibodies.35,36 Antibody test results do not indicate whether the virus is currently capable of being transmitted.
