What is an inguinal hernia?
An inguinal hernia occurs when the membrane that separates the abdominal cavity that holds the intensities breaks open and allows protrusions into the groin region.
An inguinal hernia occurs when the membrane that separates the abdominal cavity that holds the intenstines breaks open and allows protrusions into the groin region. The intestines and other organs can drop into the groin cavity and cause pain and pressure. A hernia can occur in several different parts of the body. The definition of hernia is the protruding of an organ or a part of an organ through the wall that normally contains it. This can happen in the groin area, the abdominal wall, the bowels, the diaphram, the scrotal sac or even a disk in the vertebral bones. The most common hernia is an inguinal hernia in the groin area of men, but can also occur to women but less frequently. There are two types of inguinal hernias, congenital inguinal hernia and an inguinal hernia. bodyOffer(27175) An inguinal hernia can occur in both men and women. It happens later in life; anywhere in childhood to adulthood. Its occurrence is usually related to a weakness of the peritoneum which separates the abdominal cavity from the groin area. The peritoneum will stretch and rupture, usually from lifting something heavy or straining. Sometimes if there is great pressure in abdomen, as in pregnancy, the breakage will occur from within. When a hole occurs in the peritoneum the intestines or bowels can be pushed through the opening down into the groin. You will feel a soft lump; which may be painful or may not. Usually there is more pain if you stretch, cough, laugh, lift something heavy or strain yourself in any way. Go to the doctor if you suspect a hernia.

What is a hernia?
A hernia occurs when a section of intestine protrudes through a weakness in the abdominal muscles. A soft bulge is seen underneath the skin where the hernia has occurred. In children, a hernia usually occurs in one of two places:
  • around the belly button in the groin area
A hernia that occurs in the belly-button area is called an umbilical hernia . A hernia that occurs in the groin area is called an inguinal hernia What causes a hernia? A hernia can develop in the first few months after the baby is born because of a weakness in the muscles of the abdomen. Inguinal and umbilical hernias happen for slightly different reasons. What is an inguinal hernia? As a male fetus grows and matures during pregnancy, the testicles develop in the abdomen and then move down into the scrotum through an area called the inguinal canal. Shortly after the baby is born, the inguinal canal closes, preventing the testicles from moving back into the abdomen. If this area does not close off completely, a loop of intestine can move into the inguinal canal through the weakened area of the lower abdominal wall, causing a hernia. Although girls do not have testicles, they do have an inguinal canal, so they can develop hernias in this area as well.

Findings: There is a left-sided diaphragmatic hernia with an intrathoracic stomach and multiple air-filled bowel loops in the left hemi-thorax. Mass effect with mediastinal shift from left to right is present. There is minimal aerated right lung, and no aerated left lung. Additionally, an ET tube is identified with tip at the Caring tip of feeding tube projecting over the stomach, and an umbilical venous catheter overlying the T- 12 vertebral body. Diagnosis: Congenital Diaphragmatic Hernia: Bochdalek Hernia Discussion: A congenital diaphragmatic hernia (CDH) is a displacement of abdominal contents into the thoracic cavity through a defect in the diaphragm. The most common type of CDH is the posterolateral or Bochdalek hernia which occurs in approximately 1 per 3000 live births (M:F=2:1). Hernias through the foramen of Morgagni are rare and represent 2 to 4% of all CDH. The Bochdalek hernia results from failure of the pleuroperitoneal space to close prior to the return of the bowel to the abdominal cavity during week 8 to 10 of early fetal life. The pleuroperitoneal space is closed by the development of the diaphragm. If the bowel returns to the abdomen prematurely or if there is delayed or incomplete formation of the diaphragm, a CDH develops. Bochdalek hernias occur on the left side 75% of the time. This is most likely secondary to the fact that the pleuroperitoneal canal closes earlier on the right. About 3% of affected children have bilateral hernias. Portions of the Gl tract are usually found in left-sided hernias; whereas, the liver may be present in the thorax with right-sided hernias which account for much of the morbidity and mortality associated with CDH.

    History of the Clinic and Hernia Repair
    Until the advent of laparoscopic surgery and the interest in laparoscopic repair of groin hernia, there were only two well known clinics in the world specialising in the treatment of hernia. These clinics were the Shouldice Centre in Canada and the Lichtenstein Centre in the United States. Prior to the advent of the key hole approach which led to a renewed world wide interest in hernia repair, most surgeons were performing a standard Bassini hernia repair. This involved stitching of tissues under tension. When the results of this procedure were critically examined it was found that there was an unacceptably high recurrence rate. The Bassini hernia repair was effective and efficient in 80% of patients but there were many patients troubled by recurrence of hernia and pain after surgery. There was a tendency to treat hernia repair as a relatively minor operation. Because the procedure was so well established in the minds of surgeons and because of the absence of controversy about the established method of repair, there was little pressure for change. read more...

    On December 22, 1998, I went to the University of Washington medical center in Seattle for a hernia operation. This allowed me to avoid the stress of the pre-Christmas shopping rush by giving me a perfect excuse to do nothing more than lolligag around on the couch, read some P.G. Wodehouse, watch some teevee, and generally veg out. Having taken a camera along for some dental work at Greenlake Family Dentistry with excellent results, I decided to try it again and see what I could get away with. It takes a confident sawbones to let a doped-up geek with a web server and a digital camera into the operating room in this day and age, and with a little cajoling from Dr. Andrew Smith, Dr. Mika Sinanan went for it! Woohoo! It was all pretty interesting to me, and I walked out of there (with a little help from Shirley Nelson, a post-op nurse) about 2 hours after Dr. Visha Kapoor had sewn me up. The operation was performed under local anesthetic, but the anesthesiologist had me nodding off through most of it, probably because they were getting tired of all my inane questions. Dr. Smith also took most of the pictures seen here (thanks Andrew - you ROCK!) and Karen and Tracy, the scrub and circulation nurses, took a few as well. I had such a rollicking good time that I can't wait to get another hernia, and now I jump with squeals of anticipation whenever an opportunity to do some really heavy lifting presents itself. These pics are in chronological order. I'm medically illiterate and have no idea what's going on in them, but I won't let that stop me from captioning every one with a detailed explanation. I put this page together 10 days after the surgery, and after 6 weeks was back to deadlifting old water heaters.
