Hiatal Hernia or...A Stomach with High Aspirations
The name hiatal hernia literally means a protrusion through an opening, since the definition of a hiatus is an opening and that of a hernia is the protrusion of a structure through the tissues that normally contain it. The cause is usually unknown; it may be congenital or caused by trauma. Part or all of the stomach protrudes through the diaphragm, in the area where the esophagus enters the stomach. There are eight different types of hiatal hernia that are recognized. They vary from very small to those where the entire stomach is displaced into the chest cavity. The most common variety, accounting for 90% of all hiatal hernias, is the sliding hiatal hernia . This is one where both the junction of the esophagus and the stomach and the upper part of the stomach slide upward into the chest and back down again. More than 50% of people age 50 and above can be shown to have a sliding hiatal hernia but most do not experience any adverse effects. However, these hernias may cause discomfort and a feeling of fullness and "heartburn," especially when they move significantly up into the chest. This of course feels like chest or upper abdominal pain and may sometimes be difficult to distinguish from heart related pain. Another relatively common variety is the para-esophageal hiatal hernia.

Respiratory distress. The abdomen is scaphoid, due to the herniation of its contents into the chest. Etiology/Pathophysiology:
Due to a congenital failure of the diaphragm to close correctly. The most common diaphragmatic hernia in the newborn is Bochdalek which is posterolateral in location and five times more common on the left than on the right due to the presence of the liver on the right. The degree of ipsilateral pulmonary hypoplasia is based on the size of the hernia and what developmental stage it occurred at in utero and interrupted pulmonary growth. Cases with large hernias can also have contralateral pulmonary hypoplasia. Persistent fetal circulation and pulmonary hypertension can be complications of this arrested pulmonary development. Pathology:
Distinct abnormal openings in the diaphragm are seen at surgery. Imaging Findings:
Herniated bowel (stomach, small bowel, colon) and solid viscera (liver, omentum, spleen) are seen in the ipsilateral hemithorax causing mediastinal shift. If seen immediately after birth, the hemithorax may be opaque, until the bowel fills with gas

DEFINICIÓN Es la reparación quirúrgica de la pared muscular del abdomen, cuando, por debilidad de la misma, se ha producido una salida o desplazamiento de órganos fuera de la cavidad abdominal (hernia). Durante la intervención se ponen, además, en su sitio los órganos internos desplazados (normalmente asas intestinales). DESCRIPCIÓN La hernia inguinal es una frecuente enfermedad en los varones (2%) en los que se desplazan asas intestinales hacia el escroto (hernia indirecta) o hacia la ingle (hernia directa). El tejido (intestinal) puede quedar atrapado en la hernia (hernia incarcerada) y si el flujo sanguíneo cesa (hernia estrangulada) o el intestino se bloquea (obstrucción), se produce una urgencia quirúrgica, que precisa de intervención urgente.

STOMA ('Parastomal') HERNIAS A Hernia is a weakness or split in the muscle wall of the abdomen which allows the abdominal contents (usually some part of the intestine) to bulge out. The bulge is particularly noticeable upon tensing the abdominal wall muscles - such as occurs when coughing, sneezing, straining or simply standing. Stomas pose an additional problem. When a stoma is brought out to the surface of the abdomen it must pass through the muscles of the abdominal wall, thus a potential site of weakness is immediately created. In the ideal situation the abdominal wall muscles form a snug fit around the stoma opening. However, sometimes the muscles come away from the edges of the stoma thus creating a hernia - in this case, an area of the abdominal wall adjacent to the stoma where there is no muscle. Factors that can contribute to causing a stoma hernia to occur include coughing, being overweight or having developed an infection in the wound at the time the stoma was made. The development of a stoma hernia is often a gradual phenomenon, with the area next to the stoma stretching and becoming weaker with the passage of time. This weakness, or gap, means that every time one strains, coughs, sneezes or stands up, the area of the abdomen next to the stoma bulges, or the whole stoma itself protrudes as it is pushed forwards by the rest of the abdominal contents behind it. As with all hernias the size will increase as time goes by. Stoma hernias are rarely painful, but are usually uncomfortable and can become extremely inconvenient.

Hiatus Hernia
Hiatus hernia (also called hiatal hernia) is a disorder affecting the gastroesophageal junction, the site where the esophagus (food tube) joins the stomach.
Normal Structure
See the picture on the left. The esophagus is a tube, measuring about 10 inches long, that starts at the lower end of the pharynx (throat) and extends down through the chest and into the abdomen through an opening in the diaphragm . The diaphragm is a sheet-like muscle that separates the chest cavity from the abdominal cavity and is one of the main muscles that allows us to breathe. The opening in the diaphragm through which the esophagus passes is called the hiatus . At its lower end the esophagus joins the stomach . The stomach is a bag-like structure where some digestion of food occurs. The stomach contains very strong acid (hydrochloric acid) that performs two functions - the acid kills most bacteria (but not all) and activates a digestive enzyme. The stomach is usually resistant to this acid and is not harmed.

While the majority of diaphragmatic hernias are isolated, further (careful) fetal evaluation is indicated to rule out other birth defects as well as chromosomal anomalies, since they may adversely affect the outcome. Important prognostic predictors (for survival) include the degree of fetal liver herniation into the chest, the presence or absence of other anomalies as well as the Lung-to-Head Ratio (LHR), and ultrasound-based measurement of the lung area to the head circumference.

What is a hernia? A hernia is a weakness or defect in the abdominal wall. It may be present from birth, or develop over a period of time. If the defect is large enough, abdominal contents such as the bowels, may protrude through the defect causing a lump or bulge felt by the patient. Hernias develop at certain sites which have a natural tendency to be weak; the groin, umbilicus (belly button), and previous surgical incisions. Why should hernias be repaired? Once a hernia has developed, it will tend to enlarge and cause discomfort. If a loop of bowel gets caught in the hernia, it may become obstructed or its blood supply may be cut off. This could then become a life-threatening situation. Since hernias can be repaired effectively and with minimal risk, most surgeons therefore recommend that hernias be repaired when diagnosed, unless there is serious medical problem which makes it too risky. How are hernias repaired? The standard method of hernia repair involves making an incision in the abdominal wall. Normal healthy tissues are cut until the area of weakness is found. This area, the hernia, is then repaired with sutures. Often a prosthetic material such as Gore-Tex, or another plastic material, is sutured in place to strengthen the area of weakness. Finally, the skin and other healthy tissues that were cut at the beginning are sutured back together to complete the repair. How does the laparoscopic method differ?
