INGUINAL HERNIA What is a hernia? A hernia is the bulging of a portion of an organ through an abnormal opening. The danger from herniation arises when the organ protruding through the opening is constricted to the extent that circulation is stopped or when the protruding organ impairs the function of other structures. What is an inguinal hernia? Inguinal hernias account for about 80% of all hernias and are the most common surgical procedures done in infancy. These hernias appear more frequently in boys than in girls. An inguinal hernia is derived from persistence of all or part of the processus vaginalis, the tube of peritoneum that precedes the testicle into the scrotum during the eighth month of pregnancy. Following the dropping of the testicles into the scrotum, the processus vaginalis withers and closes forming the tunica vaginalis that lies below the testicles in the scrotum. When this fails to happen, fluid from the abdomen or an abdominal organ (usually the intestines) can be forced into it causing a bulging or mass that can be felt. The process vaginalis can extend only partly from the inguinal canal or extend completely into the scrotum. Normal scrotum:
the processus vaginalis and tunica vaginalis are obliterated and contain no fluid or abdominal contents Inguinal hernia:
the processus vaginalis has remained open allowing abdominal contents (fluid and loops of bowel) to enter into the scrotum How is an inguinal hernia treated?

BILATERAL LAPAROSCOPIC HERNIA SURGERY The way this works is for post surgery hernia patients to tell of their experiences and add them to the guestbook so presurgical patients can learn from those experiences. In YOUR Guestbooks, and on the Hernia Surgery Discussion Board, within this site as of July 2003, there are THOUSANDS of personal experiences and comments by people like yourself who have accessed this site. Post surgery hernia patients gain from this as many will have to undergo a second surgery and they in turn will learn of new methods and recovery rates from those who they have helped in telling of their experience. It's a win....win situation. Basically, this site is devoted to hernia surgical patients for hernia surgical patients. Doctor comments are always welcome and encouraged. ( You can identify yourself as a physician, but no names, please. This constitutes advertising for business and this is a non-profit site). All patients should consult a physician as this site is an adjunct to a good physician's care. This site devoted to hernia surgical patients for hernia surgical patients will in the long run help both the pre and post surgical patients.

Local Anesthesia for Inguinal Hernia Repair (Originally posted 22 September 1997 on About Anesthesiology) INTRODUCTION
Many practitioners and institutions routinely and successfully employ local anesthesia for inguinal hernia repairs. Although there are certainly other options (including general anesthesia and regionl anesthesia such as spinal or epidural), local offers some unique advantages to the patient. As such, here is a short summary about the use of local for these operations including a discusssion of advantages and disadvantages. Obviously, the choice of anesthesia will be influenced by patient preferences and needs. In addition, to complete any operation successfully under local anesthesia requires a surgeon who is comfortable with the technique and willing to stop surgery and place additional local anesthesia should this become necessary. Interestingly with inguinal hernia repair it is rarely, if ever, necessary to supplement with additional injections once the initial infiltration is complete. Reported experiences with it have been uniformly positive. Advantages of Local Anesthesia
Minimal physiological disturbance which may be an advantage in the patient in whom you wish to avoid a general anesthetic and in whom a regional technique may be contraindicated. This could include patient's with serious cardiac or respiratory diseases which could tolerate other types of anesthesia but would be at reduced risk if given only local anesthesia.

Heartburn and Hiatus Hernia
What is Heartburn? Potentially dangerous confusion can arise when someone neglects their heart pain believing it to be heartburn. What causes Heartburn? Acid is present in the stomach to digest food. Heartburn occurs when small amounts of this acid rise up into the gullet (oesophagus) - the tube which carries food from the mouth to the stomach. This is called reflux. The gullet, unlike the stomach, does not have a protective lining. So when it is exposed to the acid, it can become inflamed and painful.
Why doesn't everyone get Heartburn? Some known factors that can lead to heartburn include:
  • pregnancy smoking eating large meals, especially near bedtime being overweight bending a lot wearing tight clothing around the waist
In most of these cases, it is the increased pressure which overcomes the normal muscle contraction and causes heartburn. Smoking, on the other hand, relaxes the muscle around the gullet, with the same result. What is Hiatus Hernia?

LAPAROSCOPIC HERNIA REPAIR
ABOUT YOUR HERNIA AND LAPAROSCOPIC REPAIR:
Approximately 600,000 hernia repair operations are performed annually in the United States. Many are performed by the conventional "open" method. Some are performed laparoscopically. If your surgeon has recommended a laparoscopic repair, this brochure can help you understand what a hernia is and about the treatment. LAPAROSCOPIC HERNIA REPAIR is a recent technique to fix tears in the abdominal wall (muscle) using small incisions, telescopes and a patch (mesh). It may offer quicker return to work and normal activities with decreased pain for some patients. WHAT IS A HERNIA? When a hernia occurs, it means the inside layers of the abdominal muscle have weakened, resulting in a bulge or tear. In the same way that an inner tube pushes through a damaged tire, the inner lining of the abdomen pushes through the weakened area of the abdominal wall to form a small balloon-like sac. This can allow a loop of intestine or abdominal tissue to push into the sac. The hernia can cause severe pain and other potentially serious problems that could require emergency surgery. Both men and women can get a hernia.

PATIENT INFORMATION
LAPAROSCOPIC VENTRAL HERNIA REPAIR Approximately 90,000 ventral hernia repairs will be performed this year in the United States. Many are performed by the conventional "open" method. Some are performed laparoscopically. If your surgeon has recommended a laparoscopic repair, this brochure can help you understand what a hernia is and more about the treatment. Laparoscopic hernia repair is a technique to fix tears or openings in the abdominal wall using small incisions, laparoscopes (small telescopes inserted into the abdomen) and a patch (screen or mesh) to reinforce the abdominal wall. It may offer a quicker return to work and normal activities with decreased pain for some patients. WHAT IS A VENTRAL HERNIA?
  • When a ventral hernia occurs, it usually arises in the abdominal wall where a previous surgical incision was made. In this area the abdominal muscles have weakened; this results in a bulge or a tear. In the same way that an inner tube pushes through a damaged tire, the inner lining of the abdomen pushes through the weakened area of the abdominal wall to form a balloon-like sac. This can allow a loop of intestines or other abdominal contents to push into the sac. If the abdominal contents get stuck within the sac, they can become trapped or "incarcerated." This could lead to potentially serious problems that might require emergency surgery.
