A hernia is an opening or weakness in the muscular structure of the wall of the abdomen. This defect causes a bulging of the abdominal wall. This bulging is usually more noticeable when the abdominal muscles are tightened, thereby increasing the pressure in the abdomen. Examples of activities that can worsen a hernia are lifting, coughing, or even straining to have a bowel movement. Imagine a barrel with a hole in its side and a balloon that is blown up inside the barrel. Part of the inflated balloon would bulge out through the hole. The balloon going through the hole is like the tissues of the abdomen bulging through a hernia. And, like many parents, you may think the condition has little to do with children. But hernias are often found in children, even in babies (especially premature babies ), who can be born with them. In fact, hernia repair is the most common surgery performed on children. What Is a Hernia? When part of a bodily organ (such as a loop of intestine ) or some abdominal membrane or other tissue pushes through an opening or weak spot in a muscle wall, it can protrude into a space where it does not belong. This protrusion is called a hernia. Hernias look like a bulge or lump. How Does a Child Get a Hernia? Some babies are born with various small openings in the body that are supposed to close but have not closed yet. Nearby tissues can squeeze into such an opening and become a hernia. Sometimes tissues can squeeze through muscle wall openings that are only meant for arteries or other special channels. In other cases, a child suffers a strain or injury that creates a weak spot in a muscle wall. Part of a nearby organ or other tissues can be pushed into the weak spot so that it bulges and becomes a hernia. Technique Introduction Local anaesthesia may be employed in hernia operations, either on its own or combined with general anaesthesia. The choice of technique will be influenced not only by local resources and skills, but also by patient preference. Advantages of Local Anaesthesia for Hernia Repair With a careful technique, local anaesthesia causes minimal physiological disturbance. This may be particularly useful for patients with cardiovascular or respiratory disease for whom there may be advantages in avoiding a general anaesthetic. The absence of postoperative sedation or drowsiness allows early ambulation and diminishes the requirement for recovery facilities. Local anaesthesia provides postoperative analgesia for up to four hours and may be administered by the surgeon. When adrenaline is mixed with the local anaesthetic (normally in a dilution of 1:200,000) useful vasoconstriction is produced resulting in a relatively bloodless field.
