WHAT YOU SHOULD KNOW
A hiatal (hi-A-tul) hernia occurs when a part of the stomach slides above the diaphragm (DIE-uh-fram), the thin muscle separating the stomach from the chest. This is a common problem and most people are not bothered by it. Your hernia may allow stomach acid to flow back into your esophagus (ee-sof-uh-gus), the tube that connects the mouth to the stomach. With time, the stomach acid may irritate your esophagus and cause problems. If this happens, you may need surgery to repair the hernia.
Causes
You can be born with a hiatal hernia or develop one when you are older. A trauma or surgery can cause the problem too.
Signs/Symptoms
The most common symptom is burning in your chest (heartburn), especially at night when you are lying down. Other possible signs include burping and trouble swallowing.
Care
Usually, treatment at home is all that's needed. Surgery is required only if your symptoms get worse.

Inleiding
Een hernia (Hernia Nuclei Pulposi, HNP) is een uitstulping van de tussenwervelschijf. Deze uitstulping drukt op een zenuw, waardoor pijnklachten in het been ontstaan, eventueel met verschijnselen van uitval van de zenuw (doof gevoel, krachtsvermindering).
Anatomie van de wervelkolom
De wervelkolom heeft een centrale plaats in het bewegingsapparaat. Hij bestaat uit 7 nekwervels, 12 borstwervels, 5 lendenwervels en het heiligbeen. Tussen twee wervellichamen ligt telkens een tussenwervelschijf, en deze 23 schijven verhogen de elasticiteit en de bewegingsmogelijkheden van de wervelkolom. Hoewel een hernia theoretisch van iedere tussenwervelschijf kan voorkomen, zijn voor de praktijk alleen de drie onderste tussenwervelschijven van belang. De hernia's van de halswervelkolom en die van de borstwervelkolom vormen een categorie apart waarop hier niet nader wordt ingegaan. Deze worden apart beschreven (volg de bovenstaande link). Karakteristieke pijnuitstraling in het been. De meeste hernia's komen voor op de onderste drie niveaus In deze figuur zijn de wervelbogen niet afgebeeld. A hernia is the protrusion of an organ or tissue out of the body cavity in which it normally lies. The most common hernias, by a generous margin, are inguinal hernias , both direct and indirect . Inguinal hernias are hernias involving a defect in the inguinal canal (in the lower abdomen). Inguinal hernias are more common in males than females. Indirect inguinal hernias are more common in the young as it is due to a congenital defect in the processes vaginalis whilst direct inguinal hernias are more common in the elderly as they are cause by mechanical breakdown of the fascia over many years. Terms used to describe a hernia:
reducible
- the hernial contents can be returned to their normal site
irreducible

incarcerated
- a.k.a. irreducible; the contents are fixed within the hernial sac
strangulated
- incarcerated hernia with ischaemia of contents; often associated with signs and symptoms of intestinal obstruction
complete hernia
- hernia sac and contents protrude all the way through the defect
incomplete hernia
- hernial contents do not protrude completely

Inguinal hernia Prince Charles is far from alone in needing an operation to repair a hernia. It is the commonest male operation performed in the UK, with the NHS performing over 100,000 hernia repairs every year. What is a hernia? A hernia is caused by a tear to the stomach muscles. In some cases part of the intestine or stomach will bulge through the hole and form a painful lump under the skin. The commonest type of hernia is found in the groin. It is known as an inguinal hernia because the weak spot usually occurs along the inguinal canal, an area where the tube that carries sperm enters the scrotum. Men are more likely to have an inherent weakness along the inguinal canal than women because of the way they develop in the womb. Are there other types of hernia? Yes. Protrusions elsewhere on the abdominal wall are called femoral and umbilical hernias. Highest up of all is the hiatal hernia, which occurs when part of the stomach pushes through the diaphragm separating the chest and the abdomen. Sometimes a hernia can form under the scar of an operation, in which case it is called an incisional hernia.

Unfortunately, lack of awareness can put people suffering from a hernia at risk: An abdominal hernia can potentially have fatal complications if not treated properly. With modern surgical techniques, hernias can be fixed more easily than they could in the past. A shorter operation using a less traumatic technique, along with advances in anesthesiology, have made this a different surgery than the one your grandfather or father may have endured.

Hiatus Hernia The hiatus hernia is one of the most misunderstood and maligned conditions in medicine. People blame this hernia for much more than it ever does. Patients with a hiatus hernia need to understand what it is and what might occur with it. Most importantly, they need to know it is unusual for serious problems to develop from this type of hernia. Anatomy
The diaphragm is a sheet of muscle that separates the lungs from the abdomen. When a person takes a deep breath, the dome-shaped diaphragm contracts and flattens. In doing this, the diaphragm pulls air into the lungs. The left diaphragm contains a small hole through which passes the tube-shaped esophagus that carries food and liquid to the stomach. Normally this hole, called a hiatus, is small and fits snugly around the esophagus. The J-shaped stomach sits below the diaphragm. What Causes a Hiatus Hernia?
In some people, the hiatus or hole in the diaphragm weakens and enlarges. It is not known why this occurs. It may be due to heredity, while in others it may be caused by obesity, exercises such as weightlifting, or straining at stool. Whatever the cause, a portion of the stomach herniates, or moves up, into the chest cavity through this enlarged hole. A hiatus hernia is now present. Hiatus hernias are very common, occuring in up to 60 percent of people by age 60. What Are the Different Types of Hiatus Hernia?

The bulge usually contains tissue lining the inside of the abdomen and may include fatty tissue from inside the abdomen or a loop of intestine. There are 2 types of inguinal hernias:
  • Direct inguinal hernias occur when a weak spot develops in the lower abdominal muscles. Often the cause of the hernia is not known, but lifting, straining, coughing, excessive weight gain, pregnancy, or constipation are often thought to cause a hernia. Indirect inguinal hernias occur when the inguinal canal fails to close before birth. The hernia may appear in a male's scrotum or in the fold of skin at the opening of a female's vagina. This is the most common type of inguinal hernia, and it may occur at birth or later in life. Indirect hernias are more common in males.

What is a Hernia?
HERNIA
A hernia is the protrusion of an organ (small bowel, large bowel, ovary, etc..) through a weakness in the abdominal wall. This weakness can be congenital (umbilical hernias, epigastric, inguinal hernias) or acquired (some inguinal hernias), and following abdominal operations. SURGICAL TECHNIQUES
The surgical techniques available for the treatment of hernia are classed in three groups:
    1. The pure tissue repairs (Shouldice operation);
    2. The Tension-free repairs (Lichtenstein, Gilbert, Rutkow);
    3. The laproscopic techniques.
THE SHOULDICE OPERATION
This pure tissue technique was introduced in 1945 by Dr. E.E. Shouldice. Since then, the operation has proved to be the most reliable, safe and cost-effective technique. The operation is carried out under local anaesthesia, contributing to the exceptional safety of the operation. Just as important as the technique is the fact that a team of 12 surgeons have devoted all their time and resources exclusively to the practice of hernia surgery. Shouldice Hospital surgeons will each perform from 400 to 700 operations a year, providing a sixth valuable sense in the recognition and handling of all the problems that surround hernia pathology. The Shouldice Team of Surgeons have performed more than 290,000 operations.
TENSION-FREE REPAIRS
Tension-free repairs demand the use of foreign prosthetic materials (mesh) for all necessary repairs. The actual hernia is left in place and the defect occluded or covered by a prosthesis. Tissue then grows into the mesh. These prosthesis are not without problems. Should infection supervene, all prostheses must be removed and the hernia repaired by a pure tissue repair. In the few cases where prostheses are a must (less than 2%), the Shouldice surgeons have the knowledge and the techniques to use them. In any case, those surgeons who use mesh for the 'tension-free' repairs have not demonstrated better results. Their chief claim is an earlier return to work, and even that claim is being questioned, as our patients have, on average, returned to work in 8.2 days.
