Backgrounder #1207
During the heated debate over health care reform several years ago, some states jumped ahead of the rest in aggressively regulating their health insurance markets to speed reform. The data are now in, and they show that these attempts have backfired by harming the very citizens they were designed to help. Between 1990 and 1994, 16 states were most aggressive in passing laws attempting to increase access to health insurance for their uninsured citizens. They imposed mandates and regulations on health insurance for small employers and individual citizens, implementing at the state level many of the provisions contained in the failed Clinton health care bill. To assess the results of these state efforts, the Galen Institute, in partnership with The Heritage Foundation, conducted a study of the 16 states. The data show that in 1996, after the legislation in all 16 states had been implemented, these states experienced an average annual growth in their uninsured population eight times that of the other 34 states. In 1996, the one-year average growth rate in the uninsured population in these 16 states was 8.14 percent; in the 34 other states, it actually had fallen to 1.02 percent. In 1990, before the blizzard of health care reform legislation, the two groups of states had been nearly equal at 4.6 percent and 3.9 percent, respectively.

Why Teach Sports Nutrition in Your Curriculum? Roger A. Shewmake, PhD., LN
Director, Section of Nutrition
Sports nutrition is often an engaging topic for medical students and residents. It can furnish an opportunity to encourage nutrition education through the application of sound nutrition practices to the world of sports. A sports nutrition curriculum component provides the opportunity to consider safe weight management, principles of hydration, energy production, nutrient needs, supplements, and eating disorders. It can strengthen the health promotion and disease prevention aspects of the curriculum through increased knowledge of appropriate physical activity and training. The curriculum can also provide additional knowledge and resources for the professional to be able to instruct patients, parents, and coaches with evidenced-based approaches to sound nutrition practices for athletic performance and for all aspects of life. Table 1: Recommended
Training Diet
Carbohydrate: 60-70% of calories - Complex: 48%
- Refined: 8-12% • Protein: 12%
• Fat: 20-30% Fundamental nutritional concepts of the curriculum would include protein, carbohydrate, and fat metabolism, hydration dynamics, body weight/composition, ergogenic aids and supplements. Discussions of the recommended training diet (see table 1) provide the opportunity to address typical diet problems and concepts of adequate nutrition. The protein component of the diet can be utilized to review the American intake of protein, the mystique of its importance, and the opportunity to suggest healthier means of addressing protein supplements.

Introduction
The UCSF Primary Care/General Internal Medicine Residency is a three-year program designed to train internists in both ambulatory and hospital care. As a part of the Department of Medicine Residency Program at the University of California, San Francisco (UCSF), it combines intensive in-hospital training with structured training in ambulatory medicine in a general internal medicine group practice, in internal medicine subspecialties, and in non-internal medicine specialties essential to the practicing general internist. The primary care residency is fully integrated with the Department of Medicine's other internal medicine residency programs. The primary care residents and the general internal medicine faculty participate in all Department of Medicine teaching and service activities. Back to Top
Program Objectives
The three-year residency program fulfills requirements for internal medicine board eligibility. The major goal of the UCSF Primary Care/General Internal Medicine Residency Program is to train highly competent, socially responsible general internists skilled in contemporary methods of primary care. Toward this end, the program has a number of specific objectives:
  • To provide training and a high degree of competence in the management of severely ill hospitalized patients. The resident should understand the role and contribution of various specialists and subspecialists in the acute care process, and learn to provide continuing care of patients hospitalized from their ambulatory practice.

    • Adolescents and young adults, both male and female, benefit from physical activity.
    • Physical activity need not be strenuous to be beneficial.
    • Moderate amounts of daily physical activity are recommended for people of all ages. This amount can be obtained in longer sessions of moderately intense activities, such as brisk walking for 30 minutes, or in shorter sessions of more intense activities, such as jogging or playing basketball for 15-20 minutes.
    • Greater amounts of physical activity are even more beneficial, up to a point. Excessive amounts of physical activity can lead to injuries, menstrual abnormalities, and bone weakening.
