During a routine well-child exam, you may be surprised to find the doctor examining your child's teeth and asking you about your water supply . That's because fluoride, a substance that's found naturally in water, plays an important role in healthy tooth development and cavity prevention.
What Is Fluoride?
Fluoride exists naturally in water sources and is derived from fluorine, the thirteenth most common element in the Earth's crust. It is well known that fluoride helps prevent and even reverse the early stages of tooth decay. Tooth decay occurs when bacteria - found in the plaque the dentist tries so hard to get rid of - break down sugars in food. This process produces damaging acids that dissolve the hard enamel surfaces of teeth. If the damage is not stopped or treated, the bacteria can penetrate through the enamel to the underlying tissues of the teeth, causing cavities. Cavities weaken teeth and cause pain, tooth loss, or even widespread infection in the most severe cases. Fluoride combats tooth decay in two ways. It strengthens tooth enamel, a hard and shiny substance that protects the teeth, so that it can better resist the acid formed by plaque. Fluoride also allows teeth damaged by acid to repair, or remineralize, themselves. Fluoride cannot repair cavities, but it can reverse low levels of tooth decay and thus prevent new cavities from forming.

In July 1970 we were told by our doctor that we were going to have a baby. In my first visit to the Hospital for a check-up in October, I was told by the Matron to start taking fluoride tablets for the benefit of the baby. I did so until the 20th of February 1971 when our first child, Jason, was born. I took him to the doctor straight away and he gave our son a stomach pump treatment. The doctor told me he could only find four tablets in his stomach. I was told to take him home and he would sleep for a while, but he would be okay when he woke up. This was at 2:00 P.M. On May 15th, five days after admission, Jason passed away. The doctor verified that his brain, due to fluoride poisoning, was completely dead. We have now two children. They have never or will never receive fluoride tablets from us.

Fluoride Dental decay is a common disease in North America. One way to help prevent this disease from occurring is through the use of fluoride. Fluoride helps harden the tooth enamel and make it more resistant to tooth decay. A small cavity can be stopped and even reversed by the remineralization process enhanced by fluoride. Fluoride can be delivered to the teeth in 2 ways; topically (direct contact on the teeth) and systemically (enters the blood stream).
Systemic Fluoride Systemic fluoride is delivered to the tooth surface via the bloodstream. Systemic fluoride can be derived from a food source, water source, or dietary supplements ( pill, tablet, lozenge, drop ). Fluoridated water is an effective way to reduce the problem of dental decay. It has been shown that fluoridated water and dietary supplements can reduce up to 60 % of tooth decay. Some water supplies already naturally contain fluoride, for others, fluoride is intentionally added to the water supply. Some communities have no fluoride in their water. Other communities are unaware of this option. If you would like to know if your community's water supply contains fluoride, call your local water or health department.

How fluoride helps
Fluoride that your child ingests either in food or water enters the bloodstream and becomes incorporated in the tooth enamel even before the teeth erupt, making them strong and more resistant to decay. Fluoride applied topically through toothpaste or by your dentist also makes the teeth more resistant to decay and strengthens the enamel as it repairs itself from normal wear. Fluoride is a naturally-occurring trace mineral, and like calcium, iron and other minerals, it is often found naturally in water. Unlike other minerals, it has a narrow risk/benefit ratio. This means that just the right amount of fluoride helps the teeth and too much harms the teeth, causing a condition called fluorosis.

The Associate Committee on Scientific Criteria for Environmental Quality was established by the National Research Council of Canada in response to a mandate provided by the Federal Government to develop scientific guidelines for defining the quality of the environment. The concern of the NRC Associate Committee is strictly with scientific criteria. Pollution standards and objectives are the responsibility of the regulatory authorities and are set for the purpose of pollution control. These may be based on scientific criteria starting point but they also take into account the optimal socioeconomic impact of proposed measures as well as the state of existing technology.
The Associate Committee's program includes the evaluation of available information on the probability of effects of contaminants on receptors together with the related fundamental principles and scientific knowledge. In this work particular attention is directed to receptors and contaminants (and their interactions) important to Canada. This Canadian approach is necessary because evaluations made in other countries or regions will not always be applicable to the particular circumstances prevailing in Canada.
Members of the Associate Committee, its Subcommittees and Expert Panels, serve voluntarily and are selected for their individual competence and relevant experience with due consideration for a balance among all sectors in Canada. Responsibility for the quality of study documents rests with the Associate Committee. Each report is carefully reviewed according to a multi-stage procedure established and monitored by the National Research Council of Canada in order to preserve objectivity in presentation of the scientific knowledge. Publication and distribution of the report are undertaken only after completion of this review process.

Fluoride toothpaste is now the most widely used method of prevention in the world, but it cost remains a deterrent for many of the world's poorer populations. This was the conclusion reached by a WHO expert Committee meeting a few month before this year's World Health Day, on 7 April, which will focus on oral health. Among measures to promote greater access to these toothpastes, the Committee proposed exemption from duties and taxation, since fluorides are added for the sake of public health and not for cosmetic purposes.
According to data presented at a recent meeting in Geneva, more than 800 million people throughout the world now benefits from fluorides, the most common natural occurring fluoride chemical compounds, as a means of controlling caries and maintaining oral health. This is mostly achieved through the fluoridation of water for (210 million people), salt (50 million) or toothpaste (450 million). Ground water in its natural condition almost always contains fluoride, but in concentrations that vary widely from one place to another.
Fluoride has been known for several decades to be effective against dental caries, and it acts in several ways. When present in dental plaque and saliva, it hastens the remineralization of incipient enamel lesions before cavities can become established. It also interferes with glycolysis, the process by which bacteria metabolize sugars to produce acid. In higher concentrations, it has a bacterial effect and when ingested during the period of tooth development, fluoride is thought to make the enamel more resistant to later acid attacks.

ental fluorosis is a condition caused by an excessive intake of fluorides - while the teeth develop - characterized mainly by mottling of the enamel, although the skeletal system is also affected.
