What is Lupus?
There are two types of lupus: Systemic lupus erythematosus (SLE) and discoid lupus erythematosus (DLE). DLE involves mainly the skin. This site will focus on SLE. Lupus is a chronic autoimmune disease that causes inflammation throughout the body. The body's immune system becomes confused and believes that parts of the body are foreign (like a virus or bacteria), so the immune system creates antibodies to attack those body parts. These antibodies form immune complexes that build up in the tissues and cause inflammation, injury, and pain. Criteria
If 4 out of the following 11 criteria are met, a person may have SLE:
  • Butterfly rash (red rash over cheeks and nose).
    Discoid rash (thick, disk-like rash over sun-exposed areas).
    Sun sensitivity (rash after being exposed to sunlight).
    Recurrent ulcers in mouth or nose.
    Arthritis involving 2 or more joints. Inflammation of the lining of the lung or heart (serositis, pleuritis). Kidney problems (protein in urine).

    This topic is huge and requires the explanation of pain in virtually every body part and side effects thereof including fatigue and brain fog. For this reason this article will be a little longer than most to follow. I apologize for the fact that I will be very technical in some of my explanations, so that pain physicians may appreciate some of the material as well. I will also recommend that the readers look through the website to gain a general idea of some of the new pain theories. Some reference may be given to these topics.
    What is Fibromyalgia?
    Fibromyalgia is not a new condition. There are many names for this condition given all around the world and over hundreds of years of medicine. There is a tremendous controversy over its existence, and even more over it’s criteria for diagnosis. I will be assuming its existence, and I will not be arguing the criteria for diagnosis in this forum. Fibromyalgia has been around as long as there have been animals and humans. It can affect any person on the planet and is associated with many "Injury Factors" .The complete condition is more common in women, but it is also common in men, and I have seen it even in children and young people. At a basic level, most people with fibromyalgia are suffering from diffuse muscle and tendon shortening, driven in part by multiple nerve compressions throughout the spine and limbs. Spinal origins from the neck, mid back and low back figure prominently in it’s source of pain. It seems deep spinal muscles cause deep nerve compressions and traction that cause further muscle problems in the spine, limbs and head. Pain can be directly from the muscles crushing a joint, or from pain referring into the joint from the spine. Typically both exist at the same time. Headaches can exist from referred pain from the neck but also from local muscle problems around the head.

    Fibromyalgia (pronounced Fy-bro-my-AL-ja) is a complex chronic pain illness that is recognized by the American Medical Association, the National Institutes of Health and the American College of Rheumatology. FM experts estimate that about 10 million Americans and approximately 5% of the population worldwide suffer with FM. While it is most common in women, the illness strikes men, women and children of all ages and ethnic backgrounds. For those with severe symptoms, FM can be extremely debilitating and interfere with even routine daily activities. Diagnosis:
    • The FM diagnostic criteria, established by the American College of Rheumatology (ACR) in 1990, include a history of widespread pain in all four quadrants of the body for a minimum duration of three months, and pain in at least 11 of 18 designated tender points when a specified amount of pressure is applied.
      Since people with FM tend to look healthy and conventional tests are typically normal, a physician knowledgeable about the illness is necessary to make a diagnosis.

    Fibromyalgia (fi-bro-my-AL-ja) syndrome (FMS) is a chronic disorder characterized by extreme pain and tenderness in multiple muscles and joints. The term Fibromyalgia means pain in fibrous and muscular tissues. The pain occurs in areas where the muscles attach to bone or ligaments and is similar to the pain of arthritis. The joints are not affected, so they are not deformed, nor do they deteriorate as they may in arthritis. The worst part about fibromyalgia is the pain is not localized to specific joints. The pain is felt "all over," spreading out from the spine over large areas of the body, especially in the lower back and arms. The pain can vary from muscle twitching to a burning sensation. People with fibromyalgia are susceptible to other chronic painful conditions such as osteoarthritis or migraine . The pain can vary, depending on the time of day, weather changes, physical activity, and the presence of stressful situations. Fibromyalgia was originally named fibrositis. Fibromyalgia pain continues throughout a person’s lifetime.

    "Have a positive attitude." How many times have we heard that one? While our emotions can not cause fibromyalgia or chronic fatigue syndrome, they no doubt affect our symptoms. But how can we maintain good thoughts when our bodies feel so lousy? This challenge, of course, does not pertain exclusively to chronic illness, but to any time when things do not go as we wish. But in the case of ongoing illness, seeing the positive presents a continuous struggle. Yet our moods are not perfectly correlated with our physical state. Most likely we can all recall times that despite much pain or fatigue, we were able to cope and even achieve high spirits. Perhaps the weather was perfect, good friends visited, we just accomplished something or helped somebody, making us feel good about ourselves. Other times, depression seems to take hold even when our physical discomfort is at a manageable level. Why is this? Answering this question is the key to finding optimism. To me, the vicissitudes of fibromyalgia feel like a swim in the turbulent sea sometimes it seems we have fallen and the waves continue to crash on our heads, as we fight to rise, only to be knocked down yet again. But that same ocean sometimes allows us to find a wave we can ride smoothly to the shore.

    Fibromyalgia is a chronic condition characterized primarily by widespread pain throughout the body. The name fibromyalgia, introduced by Muhammad Yunus MD and his colleagues in 1981, literally means pain in the muscles and tissue. "Fibro" refers to the fascia or connective tissue, "my" to muscle, and "algia" to pain. Other ailments associated with fibromyalgia include fatigue, poor quality sleep, difficulty concentrating, sensitivity to light, noise and cold, and irritable bowel. As many as 2% of the general population may suffer from this condition. No ethnic group seems any more likely to have fibromyalgia; however women develop it approximately 8 times more often than do men. While the medical community does not yet understand the pathology underlying fibromyalgia, more and more information about this condition is becoming known. Medical researchers are now actively searching for the cause, mechanisms and best treatments for fibromyalgia and related conditions.     ACCOMMODATING PEOPLE WITH FIBROMYALGIA

    Preface
    Fibromyalgia syndrome (FMS) is a form of generalized muscular pain and fatigue that affects seven to ten million Americans. The Americans with Disabilities Act (ADA) coupled with the high number of working individuals with FMS show why knowing about workplace accommodations for people with FMS is important. When considering accommodations for people with FMS, the accommodation process must be conducted on a case-by-case basis. Symptoms caused by FMS vary, so when determining effective accommodations, the person's individual abilities and limitations should be considered and problematic job tasks must be identified. Therefore, the person with FMS should be involved in the accommodation process. Not all people with FMS will need accommodations to perform their jobs and many others may need only a few accommodations. For those who need accommodation, the following pages provide basic information about common limitations, symptoms, useful questions to consider, and accommodation possibilities. The following is only a sample of possibilities to consider; numerous other solutions and considerations may exist.
