Standard Endodontic Techniques Kenneth Lyon United States INTRODUCTION Standard root canal therapy is more clearly described as conventional endodontics. Most modern endodontic treatment involves removal of the irreversibly damaged pulp followed by cleaning and shaping of the root canal space and subsequent filling, or obturation, with a semisolid material and a sealer. Shaping of the canal is done by either hand or engine-driven instruments. Cleaning is done by irrigating the canal system with one of a number of solutions that may be antibacterial and have tissue-dissolving ability. Obturation is achieved with gutta-percha and a root canal sealer. The cleaning and shaping phase of endodontic treatment is regarded as the most important. When the canal is clean, it is important that the system is not recontaminated by microorganisms. Because of the complex anatomy of the root canal system, complete disinfection is almost impossible to achieve. It is important, therefore, that any remaining microorganisms in the dentinal tubules are prevented from multiplying by the use of an antimicrobial dressing followed by three-dimensional filling. Recontamination from the oral cavity must be avoided, and the importance of a good coronal seal cannot be overestimated. DIAGNOSIS OF ENDODONTIC DISEASE PULPECTOMY TECHNIQUE Pulpectomy involves removal of the contents of the pulp chamber and canal. There are three major goals of veterinary endodontic therapy that should be sequentially attained for reasonable assurance of success.

DADE Endodontic Program
Duration of program: 2 years
This program is designed to provide postdoctoral students with advanced training in endodontics, and upon its successful completion, students are issued a certificate by DADE and are eligible for the specialty Board of Endodontics. Approved by the American Dental Association, the program has a strong clinical component - approximately 50 percent of the time is spent in the clinic. Students are exposed to the many facets of modern clinical endodontics with an emphasis on the utilization of the microscope in all types of procedures, including emergency treatment, complex conservative treatment, microsurgery, pediatric endodontics, and the treatment of traumatic injuries to teeth. Part of the core basic science component of the program is provided by DADE. The endodontic department arranges further basic science courses related to the specialty. Clinical seminars, planned reading seminars, and an extensive guest lecture series are also part of the program. In addition, students will gain some teaching and research experience. An effort is made to involve students in ongoing clinical and basic research projects and to make the research experience as meaningful as possible in the limited time available.

Advancements in Endodontic Surgery Treatment Objective of Endodontic Surgery Many dentistsl believe that the objective of endodontic surgery is to eliminate infected root apicies and/or periapical tissue. Often endodontic surgery is referred to incorrectly as an apicoectomy. Actually, apicoectomy by itself is seldom enough to resolve root canal failures. The purpose of an apicoecomy is only to allow us to read the root and examine the canals. To seal the canals, some form of retrofilling is usually necessary. Apicoectomy may be considered definite treatment, however, in cases of mechanical failure such as apical blockage or perforation. Such complications may result in failure of an otherwise perfectly obturated root canal system. Surgical removal of the untreated apical portion of the root will correct the problem. Apicoectomy is merely one step toward the final objective - the retroseal. Retroseal is the process that finally resolves most endodontic failures. Since the 1950s most clinicians have realized that virtually all failures result from leaking root canal systems. The often quoted Washington Study attributed root canal failures to apical percolation (63.46%), operator error (14.42%) root perforation (9.61%), calcified canals (3.85%), broken instrument (.96%), or case poor selection. Apicoectomy and retroseal can reverse all of these errors except improper case selection and some types of operator error. It should be emphasized that endodontic surgery is not to be used instead of conventional endodontics. Surgery is indicated when conventional techniques cannot be used.

FORWARD Endodontics is the branch of dentistry which is concerned with the morphology, physiology and pathology of the human dental pulp and periradicular tissues. Its study and practice encompass the basic clinical sciences, including biology of the normal pulp, the etiology, diagnosis, prevention and treatment of diseases and injuries of the pulp and associated periradicular conditions. It is our belief that the development of scientific attitudes, of inquiring minds, and of professional curiosity, is every bit as important as the development of technical skill and craftsmanship. Put differently, we are dedicated to the task of producing not only a competent endodontic specialist, but a professional personprofessional in every sense of the word.
GENERAL INFORMATION The University of Detroit began its educational work in 1877 and was incorporated as the Detroit College in 1881. In 1911, it was reorganized and became the University of Detroit. In 1990, it consolidated with Mercy College and was given its present name, the University of Detroit Mercy. The School of Dentistry was established in 1932. The Postgraduate Endodontic Program began in 1959 as a part-time program, with the first graduate in 1961. The first full-time student graduated in 1966, receiving a Certificate in Endodontics. The first Master of Science degree was awarded in 1984. Our graduates are among the leading clinicians, researchers and educators in the United States and Canada.

Department Chair, Endodontics Description Students completing the program earn a specialty Certificate in Endodontics and a Master of Science degree. Students must complete the requirements for the master's degree prior to being awarded the specialty certificate. The program conforms with the Standards for Advanced Specialty Education in Endodontics and carries a full approval status from the Commission on Dental Accreditation of the American Dental Association. Facilities and Resources The facilities of the School of Dentistry are housed in the Wood Memorial and Lyons Buildings and contain clinical facilities, research facilities, classrooms, student laboratories, departmental offices, and a computer learning laboratory. The laboratories and classrooms contain closed circuit television receivers. The Tompkins-McCall Library for the Health Sciences is located across the street from the School of Dentistry. The Graduate Endodontic Clinic is an eight-chair clinic housed on the second floor of the Lyons Building. Each resident is assigned to and will maintain their own individual operatory for the two years of the program. The Graduate Endodontic Conference Room and sterilization area are adjacent to the clinic area. The Graduate Endodontic Clinic area was renovated in 2001. The Endodontic Academic Offices and Graduate Endodontic Resident Study are conveniently located on the third floor of the Lyons Building.
