This destruction means that you become unable to breathe in enough oxygen. You also have trouble breathing out carbon dioxide. How does it occur? Emphysema is almost always caused by cigarette smoking. However, rare genetic diseases can cause similar changes in the lungs and, when combined with cigarette smoking, more rapid destruction of the lungs. Cigarette smoke can damage the cells in your lungs. This damage changes the structure of the tiny air sacs in your lungs called alveoli. The surface area of the air sacs for the exchange of oxygen and carbon dioxide becomes smaller, and the air sacs become less elastic. These changes affect your ability to breathe out carbon dioxide after breathing in. As the carbon dioxide accumulates in your lungs, there is less and less room available for oxygen to be breathed in. Once this damage has occurred, it is not reversible. Researchers are looking for ways to predict who is more likely to develop emphysema if they smoke. At this time, however, they have found no tests for this risk.

Lung Reduction Surgery is a procedure designed to improve respiratory function in a select group of patients with severe bullous emphysema. In patient with emphysema normal lung tissue is often replaced by large "bullae". Bullae are thin walled balloon-like structures that can expand to compress relatively normal lung causing further reduction in lung capacity. Bullae more often occur in the upper portions of the lung and when they are localized or are very large their surgical removal (bullectomy) may allow better function in the remaining lung. For other emphysema patients bullae may be distributed more homogeneously throughout the lung. In these patients the lungs may also become over inflated for the size of the chest cavity leaving inadequate space for expansion. By surgically reducing the size of the lungs some patients have noted significant improvement in symptoms due to this over expansion and loss of elasticity.

I have had my back cut open, and had my lung partially removed. I have also spent months at a time in a hospital away from my family. The suffering is still not over; I now have to have my other lung partially removed. This procedure will prolong my life, for a later lung transplant.

The goal of treatment is to slow the development of disabling symptoms. The most important step to take is to stop smoking. Treatments for emphysema caused by smoking include medication, breathing retraining, and surgery. People with inherited emphysema due to alpha-1 antitrypsin deficiency can receive alpha 1-proteinase inhibitor (A1PI), which slows lung tissue destruction. This medication is administered weekly through an intravenous (IV) infusion. In an IV, the medication will drip from a plastic bag into a narrow tube and through a needle inserted in a vein in your arm so that it can reach your bloodstream. It usually takes about 30 minutes for the medication to be administered. An IV may be uncomfortable when the needle is inserted, but the administration of the medication will not be painful. Side effects of this treatment are rare and, if they do occur, are mild. They include fever, light-headedness, dizziness, and flu-like symptoms.
