Down Syndrome (Trisomy 21) Adam K. Wilson PCB 3016 Trisomy 21 (Down Syndrome) was first reported by Down in 1866 (Down, 1866). It is one of the most common chromosomal disorders, occuring once in every 650 to 1,000 live births (Hook, 1982). It appears to occur with approximately equal frequency across ethnicities (Christianson, 1996). Trisomy 21 infants occur less frequently among live births to mothers at age 20 (1/1600) than at age 35 (1/370), but most infants are born to younger mothers. Therefore, most (75-80%) Down syndrome children are born to young women. Trisomy 21 is characterized by distinct phenotypic features, such as a broad face, narrow, upslanting eyes, and a large protruding tongue. Other visible characteristics include short stature, small ears with a folded helix, and broad hands with a Simian crease. Down syndrome leads to retardation and often disorders of the heart (particularly atrioventricular) and gastrointestinal tract (such as misformed intestines or anus) as well. Down syndrome also correlates positively with the development of Alzheimer disease (or at least Alzheimer symptoms) much earlier than for the population on average (Wisniewski et al., 1985).

Angelman syndrome Angelman syndrome is a genetic disorder in which a gene on chromosome 15 is missing or unexpressed. It was discovered in 1965 by Harry Angelman. Children with Angelman syndrome are generally late to develop and speak very little but have good linguistic comprehension. They have a protruding lower jaw or tongue. They laugh often, hold their arms as if they were a marionette, and flap them around, which gave the syndrome its initial name, "happy puppet syndrome".

Incidence of Down syndrome is estimated at 1 per 660 births and is the most common chromosomal abnormality. It is a commonly known observation that maternal age influences the risk of conceiving a baby with the syndrome. At age 20-24, it is only 1/1490 and at age 40 1/11. (Hook EB. It was originally called mongolism, after a perceived resemblance observed by Down between the faces of some of his patients with Down syndrome and the mongoloid race. This usage, like other medical terms used at the time, has become a term of abuse, and is now generally viewed as both offensive and medically meaningless. Trisomy 21 is the existence of the third copy of the chromosome 21 in cells throughout the body of the affected person. Whatever the actual molecular reason, over-expression of chromosome 21 genes puts children with Down syndrome at immediate disadvantage as compared with non-Down children. The IQ of a child with Down syndrome is rarely measured above 60. The brain of children with Down syndrome is usually small and underweight. The cerebellum and brain stem are unusually small. So is the superior temporal gyrus. Intellectual potential is further limited by a number of ailments such as recurring infectious diseases, heart problems, poor eyesight, etc.

6 August 2001 The Minister of Health, Dr Manto Tshabalala-Msimang, will open the first international conference on Birth Defects and Disabilities in the Developing World in Sandton tomorrow. At the conference, the Minister will also launch South Africa's first policy guidelines for the Management and Prevention of Genetic Disorders, Birth Defects and Disabilities. It is estimated that in South Africa, 1 in every 40 babies has one or another birth defect, and over 150 000 children born annually are affected by a significant genetic defect by the age of five, requiring medical and supporting interventions. The health burden of congenital disorders can be greatly reduced by using approaches for primary prevention, cure when possible, and supportive management when not possible. "The aim of the guidelines is to reduce the burden of genetic disorders and birth defects on individuals, families and society in general. They will also help to empower individuals with genetic disorders and their families to live and reproduce as normally and responsibly as possible. Having this policy will also help stimulate awareness of the psycho-social and fiscal impact of genetic disorders and defects," said Dr Eddie Mhlanga, head of maternal, child and women's health at the Department of Health. Managing such conditions includes diagnosis and providing the individual or family with genetic counselling, psychosocial support, and the best possible medical treatment. Primary health care management includes prevention of disease complications such as eye and skin care for people with albinism, mental stimulation for children with Down syndrome, and providing psychosocial support to affected individuals and their families.

Children with Down Syndrome (DS) account for one of every 800 births. The risk of chromosome disorders like DS, trisomy 13 and trisomy 18 increases with maternal age. The incidence of DS at birth is lower at age 20 (1/1600) than at age 35 ( 1/370), but many more younger women have children than older women. So most (75-80%) DS children are born to younger women. If a couple has a child with DS, there is usually an increased risk for a second affected child.
Genetic Forms of DS
All individuals with DS have extra chromosome 21 material. There are 3 genetic mechanisms for trisomy 21. The first and most common, is called non-disjunction, where there is an entire extra chromosome 21 in all cells. A chromosome study (karyotype) of trisomy 21 is shown in Fig. 1. Figure 1 The second is mosaic DS, where trisomy 21 cells are mixed with a second cell line, usually "normal" (46,XX or 46,XY). Individuals with this form of DS are frequently a bit milder in their presentation, depending on the proportion of normal cells.
