Overview
The digestive system consists of one continuous tube from the mouth to the rectum (see Figure 35). Its basic role is to digest and absorb food so that it can be converted into energy. On the whole, the digestive system shows fewer age-related changes in function than most of the other organ systems. The lining cells of the gut maintain an extraordinary capacity to reproduce themselves and to absorb nutrients efficiently. Older people experience the same bowel and digestive symptoms and conditions that younger people do. The effects of stress often lead to gastrointestinal symptoms. Constipation and diverticular disease are associated with aging, but generally digestive problems do not begin in old age. However, gastrointestinal malignancies occur more commonly in elderly people, and any change in bowel or digestive function warrants further evaluation.
SYMPTOMS Abdominal Pain
Abdominal pain is a challenging condition to evaluate because it can be a sign of a serious illness or a relatively minor problem. Abdominal pain with bleeding or that becomes progressively worse can be a medical or surgical emergency.

Colorectal Cancer Anatomy of the Colon The colon is the first 6 feet of the large intestine. It has four sections:
  • The first section is called the ascending colon . It extends upward on the right side of the abdomen. The second section is called the transverse colon since it goes across the body to the left side. There it joins the third section, the descending colon , which continues downward on the left side. The fourth section is known as the sigmoid colon because of its S-shape.
The sigmoid colon joins the rectum, which, in turn, joins the anus, or the opening where waste matter passes out of the body.

Colorectal cancer is malignant cells found in the colon or rectum. The colon and the rectum are part of the large intestine, which is part of the digestive system. Because colon cancer and rectal cancers have many features in common, they are sometimes referred to together as colorectal cancer. Cancerous tumors found in the colon or rectum also may spread to other parts of the body. Colorectal cancer is the third leading cause of cancer deaths in the United States. However, the number of new cases of colorectal cancer, and the number of deaths due to colorectal cancer, have decreased, which is attributed to increased sigmoidoscopic screening and polyp removal.

In the Weight Management Program, the CDD offers several innovative, medically supervised approaches that can be tailored to individual needs and goals. All diets are medically safe and closely monitored by a physician. Some diets involve supplemented fasts for rapid weight loss; others are balanced-calorie, low-fat, high-fiber approaches for slower rates of weight loss. Pharmacotherapy can be coupled with a balanced diet for enhanced weight loss. Patients who weigh 100 pounds or more above their ideal weight are considered extremely obese. Their risk of other diseases such as diabetes, hypertension and coronary artery disease is increased. We consult with Boston Medical Center colleagues and refer patients for specialized care, if needed. If these patients have been unable to lose weight through diet and exercise or pharmacological therapy, they may be considered for a standard open or laparoscopic gastric bypass operation.

A female friend of mine is about 48 years old and has some severe
digestive disorder - she does not seem to be able to eat most foods.
She gets a bloated stomach, has to endure severe pain and develops
a fever (with pain all over her body, including burning/irritating
eyes) whenever she eats most foods - nuts, fruits, juices, dairy products, potatoes, foods with even little protein content and all foods that are even slightly sour in taste. All that she seems to be able to eat is plain rice with some boiled vegetables without any spices and apple juice. She also seems to suffer from severe constipation, at times (not always) but if she takes laxatives, she gets severe stomach pain.

Digestive Disorders, Minor and Major If we are what we eat, then what we are is only as good as our digestion. When it goes awry, it generally affects our total well-being. Whether it's mild nausea, a nasty virus, or a chronic disorder such as irritable bowel syndrome, a disorder of the digestive tract is usually impossible to ignore. If the condition persists, it can undermine our total health, eventually threatening our very lives.
How the Digestive System Works
The gastrointestinal (GI) tract is a long hollow tube stretching from the head to the end of the body, including the mouth, pharynx, esophagus, stomach, small intestine, and large intestine. The salivary glands, liver, gallbladder, and pancreas are also important parts of this far-reaching system. The main purpose of the GI tract is to break down carbohydrates, fats, and proteins into molecules small enough to be absorbed through cell membranes. In that way, it provides the cells with the necessary energy for life and health. Digestion begins in the mouth when food is chewed and starch is broken down by ptyalin, an enzyme secreted in saliva. Food then enters the stomach, where it is reduced to tiny particles and further transformed by gastric juices. The solid portion remains in the stomach for one to six hours until it liquefies completely; liquid passes into the duodenum (small intestine), where numerous enzymes produced by the pancreas, along with bile from the liver, break it down further for absorption. When it finally arrives in the large intestine, all nutritional value has been spent, and the only remaining process is the removal of water before final elimination.

Irritable Bowel Syndrome (IBS)
What is irritable bowel syndrome (IBS)?
Irritable bowel syndrome (IBS) is an intestinal disorder that causes the following:
  • crampy pain
  • gassiness
  • bloating
  • changes in bowel habits
IBS has inaccurately been called by many names, including the following:
  • colitis
  • mucous colitis
  • spastic colon
  • spastic bowel
  • functional bowel disease
IBS is called a functional disorder because there is no sign of disease when the colon is examined. Because physicians have been unable to find an organic cause, IBS often has been thought to be caused by emotional conflict or stress. While stress may worsen IBS symptoms, research suggests that other factors also are important. IBS often causes a great deal of discomfort and distress, but it is not believed to:
  • cause permanent harm to the intestines.
  • lead to intestinal bleeding of the bowel.
  • lead to a serious disease such as cancer.
It has not been shown to lead to serious, organic diseases, nor has a link been established between IBS and inflammatory bowel diseases such as Crohn's disease or ulcerative colitis.
