The Diabetes Programme
The Diabetes Programme of the Noncommunicable Diseases and Mental Health cluster provides advice on appropriate policies and strategies for monitoring, prevention and control of diabetes. At least 171 million people worldwide suffer from diabetes; this figure is likely to more than double, to reach 366 million, by 2030. Most of this increase will occur as a result of a 150% rise in developing countries. "Diabetes is a growing and massive silent epidemic that has the potential to cripple health services in all parts of the world." What is Insulin Resistance?
Insulin is a hormone produced by the pancreas that helps unlock the body's cells so that sugar (glucose) from the food we eat can be used by the cells for energy. In people with type 2 diabetes, a combination of problems occurs, and scientists aren't really sure which is the chicken and which is the egg. The person's body may not be producing enough insulin to meet their needs, so some glucose can't get into the cells. Glucose remains in the bloodstream, causing high blood glucose levels. In many cases, the person may actually be producing more insulin than one might reasonably expect that person to need to convert the amount of food they've eaten at a meal into energy. Their pancreas is actually working overtime to produce more insulin because the body's cells are resistant to the effects of insulin. Basically the cells, despite the presence of insulin in the bloodstream, don't become unlocked and don't let enough of the glucose in the blood into the cells.

Hypoglycaemia Emergency in Hypoglycaemia Hypoglycemia is commonly called "low blood sugar". Even people who do not have diabetes have episodic low blood sugar when they have not eaten enough or exercised vigorously. The difference in common episodic low blood sugar and the severe hypoglycemia of diabetes is the potential for natural processes of the body to respond and recover from it. In diabetes, hypoglycemia occurs in combination with medication that is intended to keep blood glucose down. If low blood sugar occurs in diabetes, it is usually the result of an oversupply of medication relative to the amount of carbohydrate eaten and/or exercise done. Because of this medication, a normal recovery from low blood sugar (increase glucose release from the liver) is not possible. When blood glucose levels drop below 60 mg/dl, most people will experience symptoms of shakiness, irritability, fatique, etc. If a person with diabetes senses these symptoms at this blood sugar level, carbohydrate treatment should be initiated and recovery quick. In some people, however, especially those who have had diabetes for many years, hypoglycemia may not be easily detected. Or it may come on and advance very rapidly. In these situations, blood glucose levels can fall to dangerous levels.

The islets of Langerhans are destroyed in type I diabetes mellitus. This occurs probably as a consequence of a genetic susceptibility, followed by the onset of autoimmune destruction triggered by some environmental factor such as a viral infection. Heavy lymphocytic infiltrates appear in and around islets. The number and size of islets are eventually reduced, leading to decreased insulin production and glucose intolerance. The islets of Langerhans are normal in number or somewhat reduced with type II diabetes mellitus. Fibrosis and deposition of amylin polypeptide within islets are most characteristic of the chronic states of type II diabetes.
