Treating Type 2 Diabetes With Diet and Exercise
Source: Diabetes Self-Management
If you have Type 2 diabetes, you've no doubt been told to "watch your diet and get plenty of exercise." Diet and exercise are cornerstones of any diabetes control regimen. In fact, Type 2 diabetes can sometimes be managed with diet and exercise alone. Yet few people with Type 2 diabetes receive useful advice on how to change their diet and exercise habits. Your diabetes meal plan is not just about "going on a diet." While your meal plan may be designed to help you lose weight, it is also designed to keep your blood sugar level stable. Following a meal plan that distributes carbohydrates evenly throughout the day helps prevent periods of high blood glucose. And a diet that is low in fat and includes lots of fruits and vegetables can help bring down high blood pressure—a common problem in Type 2 diabetes. Exercise is important for managing diabetes because it lowers blood glucose, both by burning glucose for energy and by increasing the cells' ability to take glucose from the blood. Exercise can also help lower blood pressure. And it can diminish your appetite and boost your mood, making it easier to stick to your diabetes-care plan. Together, diet and exercise can help you lose weight and keep it off, and weight loss also makes insulin more effective. Even a modest weight loss of eight to ten pounds can help control blood glucose levels and blood pressure levels.

Eskimo food "pyramid" The Balanced Diet There is nothing so dear to a nutritionist's heart as the idea of a "balanced" diet. DiabetesUK say: "Foods can be divided into five main groups. In order for us to enjoy a balanced diet we need to eat foods from these groups." And the ADA say: "No single food will supply all the nutrients your body needs, so good nutrition means eating a variety of foods." Here is my definition of a balanced diet: A balanced diet is any diet that supplies all the nutrients the body needs in the correct proportions.

  • Diabetes is not caused by obesity; both conditions are caused by the same thing
  • Dietary carbohydrates cause obesity
  • Dietary carbohydrates cause diabetes
  • Obesity is merely evident before diabetes
  • To reduce disease, reduce carbohydrates.

    Lifestyle Changes Can Prevent Diabetes
People at high risk for developing Type 2 diabetes can reduce their chances of getting the disease by 58 percent if they lose as few as 10 pounds, exercise and follow a healthy diet, according to a report published in the New England Journal of Medicine. (May 3, 2001). This report underscores the significant benefits of lifestyle changes in fighting the disease. The number of people having diabetes, especially Type 2 diabetes, also known as adult onset diabetes, is increasing nationwide. Recent reports suggest that even children are getting diabetes and it is not confined to adults anymore as it used to be. Sixteen million people in the United States have diabetes, according to the U .S. Centers for Disease Control and Prevention. The researchers on the study and other experts said lifestyle changes have long been known to be beneficial but they deemed the magnitude of the benefits found in the study -even in the case of relatively small changes - remarkable. "The changes that were required to prevent diabetes were not drastic -they were just modest," said Jaakko Tuomi- lehto, author of the study and professor at the National Public Health Institute in Finland.

DIET AND EXERCISE DELAY DIABETES AND NORMALIZE BLOOD GLUCOSE
Millions of overweight Americans at high risk for type 2 diabetes can delay and possibly prevent the disease with moderate diet and exercise, a major clinical trial has found. The same study found that the oral diabetes drug metformin (Glucophage) also reduces type 2 diabetes risk, although not as effectively as lifestyle changes. Researchers announced results of the trial, called the Diabetes Prevention Program (DPP), last August after ending the study a year early. The study results are reported in the Feb. 7 issue of the New England Journal of Medicine. "This research conveys a powerful message of hope to individuals at risk for type 2 diabetes, a painful, life-threatening disease that has been increasing in this country along with obesity," said HHS Secretary Tommy G. Thompson. "By adopting a moderate, consistent diet and exercise program, many people with one or more of the risk factors for type 2 diabetes can stop the disease before it becomes irreversible." The DPP compared three approacheslifestyle modification, treatment with metformin, and standard medical advicein 3,234 overweight people with impaired glucose tolerance (IGT), a condition in which blood glucose levels are higher than normal but not yet diabetic. About 20 million people in the United States have IGT, which raises the risk of developing type 2 diabetes and cardiovascular disease. Once a person has type 2 diabetes, the risk of heart and blood vessel disease is even greater 2 to 4 times that of people without diabetes.

The primary goal when you are diabetic is to keep your blood glucose level as close to normal as possible. This may help to alleviate many of the long-term risks associated with diabetes, such as heart disease, nerve, kidney and eye damage. Diet plays a significant role in managing your blood glucose level but many newly diagnosed diabetics are unsure of where to begin. According to the American Diabetes Association , a good nutrition guide to use is the Diabetes Food Pyramid, which divides the food groups into six sections. Whole grains, beans, legumes and other high fiber foods are the largest group. This group comprises the bottom of the pyramid. These are the foods you should strive to eat more every day. The smallest group, consisting of fats, sweets, and alcohol, is at the top. These are the foods that should be consumed less often, and in smaller portion sizes.

Healthy Eating Healthy eating is the basis of managing diabetes. Choosing an appropriate healthy eating plan will assist with:
    • control of blood glucose levels control of body weight preventing short term complications (such as high or low blood glucose) and long term complications (of the heart, blood vessels, kidneys and nerves) improvement of overall health by optimum nutrition
Over the past 50 years the eating plan recommended for people with diabetes has changed considerably. It is strongly recommended that every person with diabetes be referred promptly to an accredited practising dietitian for an individualised dietary assessment and up to date accurate information and practical advice.

High Protein, Low Carbohydrate Diet High protein, low carbohydrate diets are recommended in publications such as The Zone Diet, Sugar Busters, Dr. Atkins New Diet Revolution and the Carbohydrate Addicts Healthy Heart Program. According to the claims, this type of diet can provide benefits ranging from weight loss and disease prevention, to improvements in blood glucose levels. This type of approach is not new - the low carbohydrate theme was first proposed as early as 1864, and has resurfaced approximately once a decade since the 1950's. The recommended dietary protein intake for Australians is 0.75g/kg for adults. The maximum protein requirements is around 2.0g/kg per day. A high protein diet advocates up to 4g/kg, which is far in excess of what most people need. The dietitians at DAV do not recommend that individuals follow this diet. Potential problems include: 1. Too much protein can lead to dehydration through the removal of nitrogen which requires fluid.

Many people believe that if you have diabetes, you have to follow a "special" diet and that there are some foods you are never allowed to eat. This simply isn't true. People with diabetes can still enjoy their favorite foods and can eat some of just about anything they want. But to keep weight and blood-sugar levels in their target range, the American Diabetes Association strongly encourages people with diabetes to work with their health-care team-which should at least include a physician, a certified diabetes educator and/or a registered dietitian-to create customized meal plans based on their individual tastes, lifestyles and ethnic/cultural eating habits. This approach to meal planning has been shown to help lower glucose, lipids and blood -pressure levels, which in turn helps ward off the serious complications associated with diabetes. New Guidelines About What to Eat
Earlier this year, a panel of experts put together by the association completed its review of a large body of scientific research focused on how the food we eat affects people with diabetes and their health. The association's revised nutritional guidelines, published in January 2002, provides people with diabetes and their health-care providers with the information they need for making choices about what to eat and planning meals.

Diet, Exercise Can Stop Diabetes Before It Starts
New government estimates show 17 Million Americans already have diabetes, and 16 million more of us are likely to get it, unless we start taking preventive measures now. Diabetes. Most of us know something about it because, as Americans, we almost certainly know someone who has the disease – or we’re at risk for it ourselves. Diabetes is a metabolic disorder in which people have problems converting food to energy. It is the leading cause of adult blindness, kidney failure, and non-traumatic amputations, and a major cause of heart disease and stroke. Last month the US Department of Health and Human Services (HHS) released an updated estimate – based on population changes in the most recent U.S. census – indicating that the number of Americans with diabetes has ballooned to 17 million. This represents an increase of 8% from the most commonly-used previous estimate, and another step in the wrong direction for a population increasingly plagued by diseases that reflect our less-than-healthy lifestyles. In addition, HHS Secretary Tommy G. Thompson warned Americans of the risks of "pre-diabetes," a condition affecting nearly 16 million Americans that sharply raises the risk for developing Type-2 diabetes and increases the risk of heart disease by 50%. (Type-2 diabetes used to be called “Adult Diabetes” because of its preponderance in the older population, but even that parameter is becoming blurred by the increasing number of young people who acquire Type-2 diabetes.)

Most people are unaware that their eating habits are harmful. They eat what they like and consider any advice to change their eating pattern as sheer nonsense - until struck by disease. Recently, the shocking statistics on diabetes revealed in a magazine has sent a barrage of enquiries into my office. The statistics revealed that 20 per cent of the world's diabetics are Indians. Thirty per cent of diabetics in urban India are below the age of 40 and that by 2025, WHO has estimated that India would have the highest number of diabetics followed by China, so I thought it best to address the problem in my column. A lot of my diabetic friends take methi seeds. How can methi help me to control blood glucose?
