Getting Help with Depression
by Dan Kavanaugh, M.S., social worker, National Institute for Neurological Disorders and Stroke (NINDS), Bethesda, Maryland, one of the U.S. National Institutes of Health Last summer my youngest daughter broke her leg in a playground accident when another child ran into her. We took her to the physician and the X-rays confirmed a break. The healing process began when the cast was placed on her leg. What if I had taken my daughter to the physician and instead of putting a cast on her leg he had just said "That’s understandable that she broke her leg." Suppose the physician had left it at that — no further medical intervention, no cast, just the words "That’s understandable." Wouldn’t that be a fairly shocking response? Yet when a person who is diagnosed with a chronic illness begins to express feelings of depression, they are often met with the words "That’s understandable" and are not offered the opportunity to have the depression treated.

The risk of depression also increases with the possible complications of a chronic illness. A person may be successfully managing a chronic illness, but the illness may have so many possible complications (frequent hospitalizations, depletion of financial resources, decreased ability to maintain gainful employment) that even one who is in control of the disease may have a high rate of depression.

ISSUE BRIEF School Health Nursing Services Role in Health Care PEER BULLYING INTRODUCTION Peer Bullying refers to dynamic and repetitive persistent patterns of verbal and/or non-verbal behaviors directed by one or more children on another child that are intended to deliberately inflict physical, verbal, or emotional abuse in the presence of a real or perceived power differential. All three components (e.g., patterns of behavior, intentional harm, and a power differential) must exist to constitute bullying (Olweus, 1997). Bullies’ behaviors exploit the personal weaknesses or differences of the recipients through ridicule, derision, and sometimes physical confrontation. Instigators bully for provocation, the enjoyment of others, and self-aggrandizement or to enhance their own prestige. BACKGROUND
  • Passive victims are most common. These children tend to be more submissive, insecure, and initiate fewer prosocial behaviors. If threatened, they live in fear, restrict their activities, and do not readily defend themselves. Provocative victims , also called victim-bullies, are less common. They tend to be highly aggressive, have deficient or deviant interpretations of social situations, and frequently exhibit conduct problems. Many come from adverse family backgrounds or have neuro-developmental disorders (e.g., ADHD, learning disabilities) associated with learning/information-processing deficits.

Dr. A. Bela Sood, associate professor of psychiatry, chair of the Division of Child and Adolescent Psychiatry and medical director of the Virginia Treatment Center for Children at VCU, says recent studies on the controversial drugs – part of a class of pharmaceuticals known as selective serotonin reuptake inhibitors (SSRIs) – indicate that SSRIs benefit many youths with major depressive disorder. But potential side effects, including impulsive behavior, suggest that doctors should individualize treatment plans and carefully monitor SSRI usage in aggressive and impulsive children who might be prone to risky or suicidal behavior. Tips: kids in divorce By Robert Lovinger Standard-Times staff writer
If you want to know what kids go through when their parents break up, just look inside Kevin Lee's "wish bucket."
The bucket hangs in the room which hosts meetings of the Banana Splits, a Dartmouth support group for children of divorce.
Kids scribble wishes, put them in the bucket, then look at them a few weeks later. A typical one came from a youngster who wrote, "I wish my father could put me to bed."
Mr. Lee, who supervises the group meetings, says of the kids, "They're like little ancient people. The wisdom that comes from them, speaking to one another, giving each other comfort, is immense. It's very moving. It's profound how they come so willing to share what hurts."
In some homes, the tension is so great that separation comes as a relief for the child living there. But for most kids, the break-up of a family even when it's for the best is a traumatic event that brings sorrow and in a small number of cases, clinical depression and self-destructive behavior.
Parents can do little to prevent sadness, but a lot to cushion the shock and avoid depression. With divorce now an epidemic, consciousness of its effect on children is growing. Judges nationwide, for example, are increasingly ordering divorcing parents to take classes on how their split will affect the children.

    Helping Rural Women Overcome Depression
    When associate professor Emily Hauenstein talks about depression, she is not talking about the blues. She is referring to major depressive disorder (MDD), a recurrent, incapacitating illness characterized by debilitating changes in sleep patterns, appetite, mood, and memory lasting at least two weeks. It is a disease that particularly singles out women. Hauenstein was struck by the prevalence of MDD among rural women of childbearing age while working with chronically ill children at U.Va. during the 1980s. "Sick kids are a serious drain on maternal resources, and a great many of the moms were depressed," she recalls. Poverty and rural life in all its manifestationslack of health insurance, transportation, and child careconspired to prevent these women from taking advantage of the mental health services they needed. Hauenstein noted that when these women sought health care at all, they tended to use primary care settings. Accordingly, when she applied for a research demonstration grant from the National Institute of Mental Health, she made the rural community health centerand not the large regional hospitalthe point of entry for women needing treatment for MDD.

    What causes people to want to take their own life?
    There has been an increasing amount of research into the factors that place people at risk of taking their own lives. This research is beginning to present a clear picture of the mix of conditions that contribute to the endpoint of suicide at an individual level. These factors include:
    • socioeconomic and educational disadvantages exposure to childhood adversity and trauma mental disorders including depression, bipolar disorder, substance-use disorders (alcohol, cannabis and other drug abuse and dependence), antisocial and offending behaviours, schizophrenia, and anxiety disorders exposure to recent stress or life difficulty tendencies to react impulsively and aggressively under stress.

We have provided these mental health services for more than 75 years and can help with a range of problems, such as difficulties making friends, trouble at school or depression. We believe that positive change is always possible. We know that children and their families have the strengths and abilities to grow and develop new skills and behaviors.     After a brief telephone intake process, we will assign your child to a counselor and schedule an appointment. At the first appointment, a custodial parent or legal guardian must be present at the Clinic. The counselor may also want to see other family members and/or spend some time alone with the child. We will let you know before the first appointment if other family members should attend. Other appointments will be scheduled as needed. Counseling sessions last 50 minutes and it is recommended that a family member be at the Clinic whenever a child is being seen. If you cannot keep an appointment, please call us 24 hours in advance so that we can offer your appointment time to another client who needs service. To reschedule an appointment, call 651-642-4001. Although we welcome initial referrals from doctors, schools, social workers and other community caregivers, we ask parents to schedule appointments with the Clinic.
