DESCRIPCION
El dengue es una infección proveniente del mosquito que en años recientes se ha convertido en una preocupación principal de salud pública internacional. Una complicación aún más letal, la fiebre hemorrágica del dengue (FHD), fue por primera vez reconocida durante los años '50 y actualmente es la causa principal de muerte en niños en muchos países.
TRANSMISION
Los virus del dengue son transmitidos a los humanos a través de la mordida de los mosquitos Aëdes infectados. Una vez infectados, el mosquito permanece infectado de por vida, transmitiendo el virus a individuos susceptibles durante la picadura y la alimentación de la sangre, Los mosquitos hembra infectados pueden también transmitir el virus a la siguiente generación de mosquitos por transmisión transovarial. Los humanos son el anfitrión amplificado del virus, aunque estudios han demostrado que en algunas partes del mundo los monos pueden infectarse y probablemente servir como fuente del virus para mosquitos no infectados.
La propagación del dengue es atribuido a la expansión geográfica de los cuatro virus del dengue y de sus mosquitos vectores, de los cuales el más importante es el Aëdes aegypti. El rápido crecimiento en las poblaciones urbanas trae a un mayor número de personas en contacto con el mosquito vector, mientras que los malos servicios de saneamiento en muchos centros urbanos, la falta de recursos adecuados para el almacenamiento de agua y el desecho de basura proveen más oportunidades para la propagación del mosquito.

Introduction:
Dengue fever is an acute febrile infectious disease, caused by all four serotypes (1, 2, 3 or 4) of a virus from genus Flavivirus, called dengue virus. It’s the most prevalent flavivirus infection of humans, with a worldwide distribution in the tropics and warm areas of the temperate zone corresponding to that of the principal vector, Aedes aegypti. When simultaneous or sequential introduction of two or more serotypes occurs in the same area, there may be an increased number of cases with worse clinical presentation (dengue hemorrhagic fever). The term ‘hemorrhagic’ is imprecise, because what characterizes this form of the disease is not the presence of hemorrhagic manifestations, but the abrupt increase of capillary permeability, with diffuse capillary leakage of plasma, hemoconcentration and, in some cases, with non-hemorrhagic hypovolemic shock (dengue shock syndrome).
Epidemiology:
The highest incidence of dengue is in southeast Asia, India and the American tropics, where A. aegypti can be found. In the 1980s, dengue emerged in explosive epidemics in Rio de Janeiro (1986 - serotype 1 and 1990 - serotype 2 was isolated in Niterói city), São Paulo and in many other towns and cities in Brazil. In areas such as southeast of Asia, where all four dengue virus types are hyperendemic, children are almost exclusively affected, and seroprevalence approaches 100% by young adulthood. Transmission occurs by the bite of Aedes aegypti female mosquitoes - the same vector of urban yellow fever - a day-active species with low fly-autonomy that is abundant in and around human habitations. In Brazil and other countries Aedes albopictus may also be responsible for transmission. Viremic humans (till the fifth day of disease) serve as the source of virus for mosquito infection; there is not person-to-person transmission. Movement of viremic humans provides the principal means of spread, and rapid air travel is a factor in most recent epidemic emergences.

March 1994 ABSTRACT A prospective epidemiological study of dengue haemorrhagic fever (DHF) was undertaken in two townships in Yangon between 1984 and 1988. The aim of the study was to determine - (a) whether the incidence of anamnestic dengue virus infection was higher among dengue shock syndrome (DSS) patients than in the general community, (b) whether DSS was commonly associated with infection with any particular dengue serotype and whether DSS was associated with infection with any particular sequence of dengue serotype. The epidemiological investigations were accompanied by assessment of changes in levels of selected blood proteins (ant-dengue IgG 1-4; complement proteins C3, C3a, C4 Factor B, factor H, factor I; C-reactive protein and tumor necrosis factor -à) in dengue patients with disease of varying severity during the course of their disease. Seroepidemiological surveys in the study areas enabled primary and anamnestic dengue transmission rates, as well as transmission patterns of individual dengue serotypes, to be calculated. Patients from the study areas that attended Yangon Children's Hospital were studied clinically, serologically and virologically to grade severity of disease, to identify primary and anamnestic dengue infections and to identify the virus serotypes associated with DSS.

This year, as in past years, a significant number of cases of dengue fever (DF) and dengue haemorrhagic fever (DHF) are expected in the Caribbean, Central America, South America, Mexico and South East Asia. Health Canada is aware of the following DF and DHF activity as reported and verified by international sources. This list is not necessarily comprehensive worldwide. Travellers who visit countries where dengue is common (i.e., endemic) should be aware that outbreaks can occur at any time. Travellers to these areas are advised to use personal precautions to reduce the risk of day-time biting mosquitoes that spread the disease. Recommendations include wearing protective clothing and using an insect repellent containing DEET on exposed skin. La Dengue è una malattia virale urbana e rurale trasmessa da zanzare del genere Aedes aegypti , e Aedes albopictus , zanzare largamente prevalenti in Asia e in Medio Oriente, negli ultimi anni diventate un problema importante soprattutto in America Centrale e Meridionale. La Dengue è diffusa anche nell'Africa sub-sahariana e in Oceania. Tali zanzare si sono ben adattate negli ambienti tropicali urbani, pungono principalmente durante le prime ore dopo l'alba e durante il pomeriggio; la puntura non è dolorosa ma provoca prurito.
La Dengue è dovuta a 4 tipi di virus; dopo un breve periodo di incubazione, inferiore ad una settimana, esordisce con febbre elevata, cefalea, mialgie e talora con un esantema. Dopo 4-5 giorni la febbre scompare per ricomparire dopo alcuni giorni.
Viaggiatori in paesi tropicali, specialmente nel Sud-est asiatico e nell'America Latina, possono essere a rischio di Dengue.
