The homeless: death and dying
The dying homeless person is not uncommon. Unlike most of us, who die at home or in hospital, surrounded by family and friends, those whom we love and who love us, the dying homeless often appear to be dying alone, and are in reality dying alone. Surviving in impoverished surroundings, having long left all semblance of family behind, having little contact with society, these people often rely for their final support on those who, through their professional work, or as caring human beings in their own right, have developed and maintained a relationship with them, a commitment to them. Death, and the process of dying, is the final phase of life. It calls for a special kind of sensitivity, respect, affirmation of human dignity, and a willingness to accept the dying person irrespective of their state of mind. In contemplating this, it's worth examining your own feelings and emotions as well, given that your way of handling this special situation will often be guided by these feelings and emotions.

Another undignified year
for death with dignity
Last Friday I had the rather unpleasant task of contacting my clients to let them know that Hawaii's Death with Dignity bill did not get a hearing this year. My clients, all of whom are dying, had been hoping that Hawaii, like Oregon, would finally pass a law that allowed a competent and terminally ill adult the right to ask for a lethal dose of medication to hasten their death, and thus end what is often prolonged suffering. When asked why, I couldn't answer other than to proffer that perhaps some legislators were swayed by pressure from right-to-life groups; or perhaps they simply felt there were more pressing issues than easing the suffering of a small number of people; or perhaps, as one rather curmudgeonly client I have grown fond of replied, "Dead people can't vote, so they don't give a damn about us." Whatever the reasons, Hawaii once again has failed to enact legislation that would help our terminally ill citizens and the failure isn't limited to assisted dying. The Legislature didn't pass a single bill aimed at correcting the other end-of-life problems identified in a national study in which Hawaii received a negative rating for our failure to have adequate end-of-life pain-management policies. Please note: the document you are reading is one which was voted on by a church body which preceded the founding of the Evangelical Lutheran Church in America (ELCA). Any such statement has the status of a "historical document." These documents may guide the ELCA and inform the work of the Department for Studies, and act as policy when sufficient agreement exists and the ELCA has not adopted another statement on the same subject. However, social statements of predecessor church bodies do not constitute ELCA policy, as they have not been voted on by a Churchwide Assembly. When quoting or referring to these documents, please take care to make the non-binding nature of this relationship clear. Adopted by the Eleventh Biennial Convention, Louisville, Kentucky, September 3-10, 1982. Today it is commonplace to speak of the triumphs of modern medicine- achievements such as open heart surgery and organ transplants, dialysis machines that substitute for the kidneys, pacemakers that regulate the beating of the heart, and vaccines that have made once-dreaded diseases almost forgotten words. Each of these discoveries has saved countless lives and relieved much suffering. None of this, however, has changed the fact that death still occurs. The new technologies do not always cure but sometimes only prolong the dying process, at times with great suffering. The irony of modern medicine is that with the new technologies that vastly expand the range of what it is possible to do has also come the anguish of deciding when it is appropriate to use these capabilities.

Overview of Sandra Martin Productions: Sandra Martin Productions is a video production company in Omaha, Nebraska. Since our founding in 1996, we have produced a number of videotapes designed to raise viewers' awareness on a variety of sensitive topics. A small and highly focused company, we are unique in many ways. First unlike many other production houseswe do not accept outside commissions; our production choices are based solely on our own personal and heartfelt criteria. Secondly, we are personally involved in all aspects of a creative project, from developing the concept, researching, writing the script, and handling the on-camera interviewing, to directing, producing and marketing the finished production. And lastly, given the fact that we are creative people and not necessarily experts on the many complex issues we deal with, we work closely with those who are, affiliating early-on in a project with that project's appropriate professional organization. Barry Greenwald, Ph.D. While dying is the ultimate loss experience, the stages that the terminal patient passes through provide broad guidelines for understanding all loss and the grieving process as well. In her ground breaking work, On Death and Dying , Elisabeth Kubler-Ross outlined the stages of the dying patient which have profoundly influenced the treatment of those suffering terminal illnesses.
  • DENIAL. The person when first confronted with the fact that s/he has an illness that will bring about death is likely to be unable to take-in this information. The most immediate response is "This can't be true" or "This can't be happening to me." The fact of one's impending death is experienced as unreal and impossible.
Patients in denial are likely to make the rounds of many doctors looking for the one who will tell them that this is not a fatal illness. They want someone to "undo" the terrible and overwhelming news they have received. Under the best of circumstances, facing one's own mortality is very difficult. When faced with a "death sentence" the task of incorporating this news feels next to impossible. Denial simply eliminates a painful, hard-to-bear truth. It simply isn't so. In denial, a patient may refuse to talk about his/her illness, not allow others to mention it, or maintain unrealistic beliefs about miracle cures or their ability to escape the fatal outcome.
