Crohn's disease is a chronic inflammatory disease of the intestines. It primarily causes ulcerations (breaks in the lining) of the small and large intestines, but can affect the digestive system anywhere from the mouth to the anus. It is named after the physician who described the disease in 1932. It also is called granulomatous enteritis or colitis, regional enteritis, ileitis, or terminal ileitis. Crohn's disease is related closely to another chronic inflammatory condition that involves only the colon called ulcerative colitis . Together, Crohn's disease and ulcerative colitis are frequently referred to as inflammatory bowel disease (IBD). Ulcerative colitis and Crohn's disease have no medical cure. Once the diseases begin, they tend to fluctuate between periods of inactivity (remission) and activity (relapse). They affect approximately 500,000 to 2 million people in the United States. Men and women are equally affected. IBD most commonly begins during adolescence and early adulthood, but it also can begin during childhood and later in life. Crohn's disease tends to be more common in relatives of patients with Crohn's disease. It also is more common among relatives of patients with ulcerative colitis.

Inflammatory Bowel Disease (IBD) is a term that describes chronic inflammation disorder of the small and/or large intestine. The mucous membranes lining the colon become inflamed and develop ulcers, causing bloody diarrhea, pain, gas, bloating, and sometimes hard stools. Included within the area of inflammatory bowel is ulcerative colitis and Crohn's disease. Both involve most of the same symptoms. However, ulcerative colitis is limited to the mucosa and submucosa, which are the first two layers of the lining of the bowel. Crohn's disease involves those two layers and also the next two layers below, the muscular layer and the connective tissue layer. The factors causing the two problems appear to be parallel. Since so many similarities exist the details of differences are not covered here.

Complications in general can be defined as events that make a simple matter more complex. In uncomplicated Crohn's Disease, appropriate treatment of inflammation results in clinical improvement. When a complication takes place however, improvement is delayed. How are complications categorized?
Complications are usually categorized as local or systemic. The term local refers to a complication involving the intestinal tract itself. The term systemic refers to complications involving other organs or complications that affect the patient as a whole rather than the bowel locally. What are some of the more important local complications of Crohn's Disease and Ulcerative Colitis?
Local complications include profuse bleeding from deep ulcerations, perforation (rupture) of the bowel, marked dilatation (termed toxic megacolon if the patient is seriously ill), or simply failure of the patient to respond appropriatly to the usual medical treatments. Take from a supplement of a national newspaper dated 28 July 96 :- That Gut Feeling : Scientists at St Georges hospital in London are claiming there is a link between Crohns disease - a debilitating digestive problem that affects more than 40,000 people in the UK - and drinking milk. Professor John Hermon-Taylor, a surgeon, and his team have reported finding minute traces of an organism known as myco-bacterium paratubercolosis in two thirds of the intestinal tissue removed from Crohns patients after surgery and although the National Dairy Council has disputed such claims on the basis of its own studies, the hospital researchers say they have also found the organism in supplies of whole, pasteurised milk. Hermon-Taylor and his colleagues are suggesting mycobacterium, which causes Johnes disease in sheep and cattle (a condition similar to Crohns disease in humans), is being transferred through food and water systems and can sometimes survive the process of pasteurisation. The full results of the study will be published in September and these may be convincing enough for sufferers to be told, for the first time since the condition was originally diagnosed in 1932, that scientists have discovered a concrete cause. Although public awareness of the condition is still limited, 3000 new cases are diagnosed every year. Crohns affects both men and women equally. The incidence has doubled in the past 20 years.

By the 1920's, the belief was fading that intestinal tuberculosis occurred without acid-fast bacilli or caseous necrosis, and a disease known as "non-specific granulomata" emerged (197, 298). In these cases, as well as those described by Crohn et al (59) in later years, the authors each discussed "the remarkable resemblance" of these cases to intestinal tuberculosis. The landmark article by Crohn, Ginzberg, and Oppenheimer (59) recognized regional ileitis as a separate and unique disease entity and displaced the long-held belief of a mycobacterial etiology. We now know that hypertrophic intestinal tuberculosis (3) and tuberculosis without caseation or demonstrable acid-fast bacilli (296) do exist, as well as a distinct disease known as Crohn's disease. Nevertheless, over the years, the notion recurs that Crohn's disease might in fact be mycobacterial in origin.
