Physician-authored summaries and The New England Journal of Medicine Please sign in for full text Summary and Comment Heart-Rate Recovery After Exercise Testing Is a Strong Predictor of Mortality Results from previous studies have shown that exercise duration is a strong independent predictor of mortality in patients who are referred for exercise electrocardiography. More recently, investigators have demonstrated that post-exercise recovery of heart rate also helps predict mortality. Now, researchers at Cleveland Clinic have prospectively analyzed all-cause mortality rates in 9454 consecutive patients who underwent symptom-limited exercise ECG testing. Abnormal heart-rate recovery was defined as a decrease of 12 bpm or less during the first minute after peak exercise. During a median 5.2-year follow-up, 312 subjects died. All-cause mortality rates were 8 percent among patients with abnormal heart-rate recovery and 2 percent among patients with normal recovery a significant difference. After adjustment for age, sex, typical cardiovascular risk factors, and medication use, abnormal heart-rate recovery remained an independent predictor of death (adjusted odds ratio, 2.13). No interaction was found between heart-rate recovery and overall treadmill risk score, which took into account exercise duration, ST-segment deviation, and anginal status. Comment: The results of this very large study add to a growing body of evidence suggesting that what we observe after exercise testing may be nearly as important as what we observe during testing. An inadequate drop in heart rate soon after a stress test is a strong independent predictor of mortality and should be taken into account by physicians as they make risk-assessment and therapeutic decisions.

It may be a consequence of heart muscle weakness such as after a series of heart attacks, valvular disease like rheumatic aortic stenosis, or toxic injury such as alcohol poisoning. Symptoms frequently include shortness of breath, particularly while lying down (orthopnea), leg swelling (edema), and awakening from sleep gasping for air (paroxysmal nocturnal dyspnea). CHF is a cause for serious concern. The outlook for patients with CHF continues to improve. There are several groups of medications (Beta Blockers, ACE inhibitors, and Angiotensin II receptor blockers) that have been shown to prolong life and reduce hospitalizations in patients with CHF. The New England Heart Institute participates in nation-wide clinical research trials to help assess whether other treatments like antiarrhythmic agents or implantable defibrillators will further protect the patient with CHF. The optimal treatment for patients with CHF involves considerably more than prescribing the right medicine. It requires the full support of the patient to work with the health care team in managing his or her heart failure. It has been well established that the main reason for re-hospitalization is the discontinuation of prescribed medications, dietary indiscretions, and the failure to identify early signs of worsening heart failure. It is therefore the goal of the Heart Failure Management Program to not only provide the patient with the best medical treatment possible, but also to educate the patient regarding diet, medications, exercise, and most importantly, symptoms to report before a trip to the hospital becomes necessary.
