Ernest Bramah, Kailung Unrolled His Mat In June 1957 the BBC radio news reported on a white paper, prepared by Doll and Hill, which had been presented to parliament. The paper claimed that cigarette smoking appreciably increased a person's risk of developing lung cancer, with the incidence increasing in proportion to the number of cigarettes smoked. This news was published in a statement by the Medical Research Council

for Detection of Lung Cancer
Why is there a need for lung cancer screening?
In 1998 there were:
  • 160,000 deaths due to lung cancer in the United States
  • 172,000 new cases of lung cancer
  • 12% cure rate for lung cancer
By contrast,
  • the 5 year survival rate for lung cancer detected early (Stage IA) was 85-100%
Routine chest x-ray screening often are unable to detect lung cancers until they are of advanced stage. * Screening CT Chest shows lung carcinoma in the left lower lobe which was obscured by the heart on the routine chest x-ray performed two weeks earlier.
What studies have been done to evaluate this test?
A study by Henschke, et al.* found that n on-calcified pulmonary nodules were detected in 23% participants by low dose CT at baseline, compared with 7% by chest radiography. Malignant disease was detected in 2-7% by CT and of the 27 CT-detected cancers, 26 were resectable. It was concluded that low-dose CT can greatly improve the likelihood of detection of small (early) lung cancers, and thus potentially more curable stage.

The physician or surgeon who becomes the patient's first treating specialist may not be involved years later when metastases appear. The radiation oncologist is not going to be in the patient's home when the mist morphine needs to be replaced by a subcutaneous infusion. The medical oncologist will not be treating the family after the patients death. The GP's role begins with patient education about lung cancer prevention, early detection and prompt and appropriate referral. The GP also provides support through the phases of specialist treatment and has an important part to play in the management of patients with advanced disease, including the in-home palliative care of the dying patient. Lung cancer is the most common cause of cancer death in Australia. While it used to be identified as a male-dominated disease, this is changing with changes in smoking patterns. Both tobacco use and lung cancer incidence are increasing in Australian women aged 25-44 and, in one state at least, now exceed levels seen in men of the same age. The incidence of mesothelioma (other than cases related to asbestos exposure) is also increasing.

It is estimated that 48 million mammograms are performed each year in the United States.
