Weight Management We think of binge eating and bulimia as parts of a curious and biologically perverse puzzle - the craving for and the compulsive eating of foods that cause illness and obvious suffering. Cravings may be interpreted as urges to find missing nutrients, but the foods found in compulsive searches are not biologically correct. Instead, food cravings are a symptom of an addictive loop. Among our patients, there is a consensus that eating-control is difficult to achieve, even when wrong food choices involve risk of serious illness. The consensus is also that some foods trigger compulsive eating, uncontrollable by ordinary acts of consciousness. We compare binge eating with binge drinking and find many similarities. The Solution
  • Treat compulsive eating as an addiction.

Chemical malfunction plays role in bulimia, researchers say
February 14, 1999
Web posted at: 9:49 p.m. EST (0249 GMT) CHICAGO (AP) A new study adds to evidence that the eating disorder bulimia springs at least in part from a chemical malfunction in the brain and not merely from excessive desire to remain thin, researchers say. In the study released Sunday, women who had suffered from bulimia and recovered were more affected psychologically than other women by being deprived of tryptophan, which plays an indirect role in appetite regulation, researchers found. Tryptophan is an amino acid that occurs naturally in many foods and is used by the body to make serotonin, a mood- and appetite-regulating chemical in the brain. Compared with normal women, the recovered bulimics reported bigger dips in mood, greater worries about body image and more fear of losing control of eating after being deprived of dietary tryptophan for about 17 hours, researchers said.

What Is Bulimia?
A psychological eating disorder, bulimia is characterised by episodes of binge-eating followed by purging the food out of your system by vomiting, fasting, enemas, laxatives and diuretics, or compulsive exercise. It is a disorder, which provides control, power and a sense of purpose to those who suffer from it. Binge eating is not a response to intense hunger. It is usually a response to depression, stress or self esteem issues. During the binge episode, the individual experiences a loss of control. However, the sense of a loss of control is also followed by a short-lived calmness through purging as it serves as an emotional, physical and mental release. The cycle of overeating and purging usually becomes an obsession and is repeated often. People with bulimia can look perfectly normal. Most of them are of normal weight, and some overweight. Women with bulimia tend to be high achievers. It is often difficult to determine whether a person is suffering from bulimia. This occurs because bingeing and purging is often done in secret.

cautionary statement
A. Recurrent episodes of binge eating. An episode of binge eating is characterized by both of the following: (1) eating, in a discrete period of time (e.g., within any 2-hour period), an amount of food that is definitely larger than most people would eat during a similar period of time and under similar circumstances
(2) a sense of lack of control over eating during the episode (e.g., a feeling that one cannot stop eating or control what or how much one is eating) B. Recurrent inappropriate compensatory behavior in order to prevent weight gain, such as self-induced vomiting; misuse of laxatives, diuretics, enemas, or other medications; fasting; or excessive exercise. C. The binge eating and inappropriate compensatory behaviors both occur, on average, at least twice a week for 3 months. D. Self-evaluation is unduly influenced by body shape and weight.

Eating Disorders: Anorexia and Bulimia March 2002 WHAT ARE EATING DISORDERS? Eating disorders are devastating behavioral maladies brought on by a complex interplay of factors, which may include emotional and personality disorders, family pressures, a possible genetic or biologic susceptibility, and a culture in which there is an overabundance of food and an obsession with thinness. There are four general categories of eating disorders:
  • Bulimia nervosa.
  • Anorexia nervosa.
  • Binge eating.
  • Eating disorders not otherwise specified.
These are not new disorders. Although anorexia nervosa was first defined as a medical problem in the late 1800s, descriptions of self-starvation have been found even in medieval writings. Bulimia Nervosa Bulimia nervosa is more common than anorexia and it usually begins early in adolescence. It is characterized by cycles of bingeing and purging, and typically takes the following pattern:
  • Bulimia is often triggered when young women attempt restrictive diets, fail, and react by binge eating. (Binge eating involves consuming amounts of food that are larger than average portions within a two-hour period.)
  • In response to the binges, patients compensate, usually by purging, by vomiting, by using enemas, or by taking laxatives, diet pills, or drugs to reduce fluids.

What is bulimia nervosa?
Bulimia (buh-LEE -me-ah) nervosa is a type of eating disorder. Someone with bulimia eats a lot of food in a short amount of time (bingeing) and then tries to get rid of the calories by purging. Purging might be done in these ways:
  • making oneself throw up taking laxatives (pills or liquids that increase how fast food moves through your body and leads to a bowel movement [BM])
A person with bulimia may also use these ways to prevent weight gain:
  • exercising a lot (more than normal) restricting her eating or not eating at all (like going without food for a day) taking diuretics (pills that make you urinate [pee])
What causes it?
Bulimia is more than just a problem with food. It's a way of using food to feel in control of other feelings that may seem overwhelming. Purging and other behaviors to prevent weight gain are ways for people with bulimia to feel more in control of their lives and to ease stress and anxiety. While there is no single known cause of bulimia, many things may have a role in its development:

Seeking the assistance of a therapist can be a difficult process ...
WHAT COMES FIRST? BINGING OR FEELINGS?
by Joanna Poppink, M.F.C.C.
Recently a colleague asked me, "Do you try to get your clients to diminish their bingeing behaviors from the beginning of therapy, before you together have explored the feelings that are fueling that behavior? I know there are differing views on this and I would be interested in hearing yours." When I heard this question, several points of equal value, in my opinion, arrived simultaneously in my mind concerning therapy work with eating disorder patients. Since writing is linear I can only communicate one point at a time. Please understand that these considerations are simultaneous. My first thought was that I don't try to 'get' my clients to do anything. I want them to heal and develop the capacity to live a fulfilling and satisfying life. But what that means to them and how they specifically accomplish that falls into the realm of their personal values, decision making and evolution. Therefore I do not address the bingeing or purging in terms of control in any way. Often it's the new client who is quite active in wanting immediate results in terms of stopping both bingeing and purging behaviors. In fact, I'm usually the one who is putting forth effort, gentle and consistent, to create an environment where we focus more on our newly forming relationship.

Bulimia Nervosa's an eating disorder that affects mostly females (around 1%) in their mid-teens/late 20s. Sufferers get overwhelming urges to binge on food and then, to avoid gaining weight, they purge what they've eaten by throwing up, taking laxatives, over-exercising, or all three. It's difficult to spot bulimia, as it doesn't necessarily lead to weight loss, and many sufferers confine their activities to their own home. Also, bulimia can come and go - it isn't necessarily permanent. Physically, bulimia can lead to:

"What is bulimia?"
Bulimia is an eating disorder characterized by binge eating and purging. Binge eating is the consumption of large amounts of food. During a binge, the person feels that she or he has no control over eating. Binges may last from a few minutes to several hours. Purging is getting rid of the food eaten by methods such as self-induced vomiting, laxatives, fasting, severe diets, vigorous exercise, and diuretics.
"Who develops bulimia?"
Bulimia is most likely to affect young women who are of normal weight and healthy in appearance. They often feel dissatisfied with their body weight, shape, or size. Food, weight, and appearance are extremely important to them. People with bulimia tend to be perfectionists but often have low self-esteem, feelings of inadequacy, and doubts about their own abilities. Sometimes they are emotionally insecure and do not know how to handle feelings of anger, sadness, anxiety, stress, and boredom. Bouts of depression are typical in women with bulimia. The binge/purge may be an attempt to cope with their feelings, with life's problems, and with low self-esteem. In recent years the incidence of bulimia is increasing among men and among people from under represented groups. Research shows that dieting is the main cause of bulimia!
