(2) a pattern of unstable and intense interpersonal relationships characterized by alternating between extremes of idealization and devaluation (3) identity disturbance: markedly and persistently unstable self-image or sense of self (4) impulsivity in at least two areas that are potentially self-damaging (e.g., spending, sex, Substance Abuse , reckless driving, binge eating).

Borderline Personality Disorder?
Q. I began having serious problems controlling my emotions about a year ago. I got very depressed missed a lot of work, lost interest in everything. My diagnosis was borderline personality disorder. Paxil (40mg/day) seemed to be helping with my moods, but I was doing destructive things like taking OTC sleeping aids, occassionally cutting myself to see how it felt, binging and purging, and using alcohol to escape. The sleep aids became a consistent problem. My psychiatrist refused to continue working with me if I didn't control their use and reduce the Paxil to 20mg/day. This made me more depressed, so I took more pills. I'm finally feeling more stable with a new doctor who has me taking Serzone - currently 400mg/day. I started at a lower dosage in early May and worked up to 400 by 5/28. I'm still having mood swings. I'm still crying and feeling worthless.

AND OBSESSIVE-COMPULSIVENESS As opposed to psychopaths, sociopaths, and antisocials who are afflicted with a disease of detachment, borderlines and the obsessive-compulsive are afflicted with a disease of attachment . People with Borderline Personality Disorder (BPD) or Obsessive Compulsive Disorder (OCD) are pathologically attached to persons, places, and things so much that, like antisocials, they block out any other person's right to an opinion, and in some cases, even another person's right to exist. As with all personality disorders, there's a disinhibition of violence. Both BPD and OCD are more often diagnosed in females than in males, although not exclusively so. Female serial killers who act as "Angels of Mercy", for example, would suffer from BPD, as might anyone who attempts to right the world's wrongs, seek revenge or payback. Aileen Wuornos, for example, was diagnosed by a prison neuropsychologist as being a perfect case of BPD. Jeffrey Dahmer's sexual behaviors were characterized by another psychiatrist as displaying OCD. It's important not to confuse OCD (the personality disorder) with obsessive compulsive anxiety disorder (the anxiety disorder). It's the anxiety disorder most people are thinking of when they characterize obsessive-compulsiveness as irrational rituals (repetitive housecleaning, handwashing, book organizing, etc.)

    Among non-psychologists, it is common to confuse personality disorders with psychological disorders generally (i.e., psychopathology), when in fact personality disorders are only one variety of psychopathology. The Diagnostic and Statistical Manual of Mental Disorders DSM-IV , American Psychiatric Association, 1994) distinguishes disorders like depression and schizophrenia (Axis I disorders) from personality disorders (Axis II disorders). Although there is a great deal of overlap among these different kinds of disorders (e.g., borderline personality disorder often occurs with depression, and personality disorders often occur together), it is probably worthwhile at least to be aware of the conceptual distinction. Personality disorders, by definition, are disorders of personality. Consequently, they are typified by early onset and pervasive effects. Nevertheless, there are treatments that can help those with personality disorders learn to cope with their distinctive problems in living.

Borderline Personality Disorder
Borderline personality disorder (BPD) is a serious mental illness characterized by pervasive instability in moods, interpersonal relationships, self-image, and behavior. This instability often disrupts family and work life, long-term planning, and the individual's sense of self-identity. Originally thought to be at the "borderline" of psychosis, people with BPD suffer from a disorder of emotion regulation. While less well known than schizophrenia or bipolar disorder (manic-depressive illness), BPD is more common, affecting 2% of adults, mostly young women. There is a high rate of self-injury without suicide intent, as well as a significant rate of suicide attempts and completed suicide in severe cases. Patients often need extensive mental health services, and account for 20% of psychiatric hospitalizations. Yet, with help, many improve over time and are eventually able to lead productive lives.
Symptoms
While a person with depression or bipolar disorder typically endures the same mood for weeks, a person with BPD may experience intense bouts of anger, depression and anxiety that may last only hours, or at most a day. These may be associated with episodes of impulsive aggression, self-injury, and drug or alcohol abuse.

Personality disorders are chronic patterns of behavior that impair relationships and work. The cause of borderline personality disorder (BPD) is unknown. People with BPD are impulsive in areas that have a potential for self-harm, such as drug use, drinking, and other risk-taking behaviors. Risk factors for BPD include abandonment issues in childhood or adolescence, sexual abuse, disrupted family life, and poor communication within the family. This personality disorder tends to occur more often in women and among hospitalized psychiatric patients.

The Infinite Mind: Borderline Personality Disorder Week of July 4, 2001
Statistics on Borderline Personality Disorder can be hard to come by. But reliable estimates indicate as many as one in fifty Americans may suffer from the disorder. It was first recognized in the 1930s, but therapists still argue over how to define and treat it. In fact, many object to the name itself, since personality disorders are often seen as less "serious" than the major mental illnesses like schizophrenia, depression, and bipolar disorder. This week on The Infinite Mind, we learn about the symptoms of BPD, why it is so frustrating for therapists to treat it, and a new form of therapy that has been effective. First, we hear from Kathleen. Kathleen is currently studying for her master's degree in social work, and plans to be a psychotherapist. She describes her own struggle with borderline personality disorder, which resulted in her being hospitalized in Maine. Kathleen describes her overwhelming feelings of hopelessness, which led to her slashing and burning herself in order to feel relief from her anxiety and numbness. She repeatedly overdosed on pills and thrived on the drama of being taken to the emergency room under police escort. She was angry, and aware that other people including therapists and other caregivers were afraid of her due to her rage. She had trouble forming an alliance with caregivers and didn't really want to get better. What she did want was to get rid of herself.
