The annual cost of Arthritis health care in Canada is $14.3 billion. Overview of Standard Medical Treatment Arthritis is usually treated with drugs and physiotherapy. Drug treatment of arthritis is often unsatisfactory. If you critically examine treatment regimens, there has been little progress beyond the basic drugs, aspirin and prednisone that have been in uses for several decades. The use of immunosuppressant agents does offer control of the chronic inflammatory process but with penalties that can be severe. The older immunosuppressant drugs, such as methotrexate and cyclophosphamide, are poisons that can do much harm. Treatment with these drugs involves continuous monitoring and these drugs can promote or cause secondary diseases that are sometimes life-threatening.

The authors concluded: "Increasing numbers of scientific studies suggest that dietary manipulation may help at least some rheumatoid patients and perhaps the greatest need now is for more careful and well-designed research so that preconceptions may be put aside and role of diet, as a specific or even a non-specific adjunctive therapy, may be determined."

Diet and Arthritis There are certain arthritic conditions which are definitely partially treatable with diet, these include gout and coeliac disease, which are distinct entities. In these -diet is a factor. In the case of gout , the cell nucleus is metabolically broken down, on cell death, from purine content to uric acid. Ultimately this is excreted by the kidney. The dietary source of purines is only approximately 4-5%, and the bodies own metabolism results in the other 95%. Diet however becomes a factor in acute and subacute phases of gout. A properly treated patient with gout can liberalize the diet once controlled. Therefore with gout -diet should be controlled rigidly, especially in those times where there is an attack or risk of attacks or where there are tophi. I only start to liberalise the diet, when patients are stable on the drugs that control hyperuricaemia.
I am strict regarding alcohol, red meats, gravies and meat extracts, rich seafoods such as shellfish and lobster.
I do allow ordinary fish, some chicken and unrestricted fruit and vegetables.

Researchers compared the outcome in 26 people with well controlled but active rheumatoid arthritis who were placed on a “Mediterranean diet” for three months with a similar control group of 25 patients who remained on a traditional Western diet. The Mediterranean diet group ate lots of fruit, vegetables and legumes, more fish and less meat. A moderate wine intake was coupled with the use of olive oil in meals. This test group showed less pain, inflammation and arthritis activity and a lower number of affected joints than those in the control group who ate a typical Western diet. EFFECTIVE ARTHRITIS TREATMENT, SAY WFU RESEARCHERS “Considering that side effects often limit the use of drug therapy and surgical intervention is often ineffective for mild or moderate knee osteoarthritis, our results give strong support to the combination of exercise and weight loss as a cornerstone for the treatment of overweight osteoarthritis patients,” said Stephen Messier, professor of health and exercise science and principal investigator of the study. Researchers from Wake Forest’s health and exercise science department and Wake Forest Baptist Medical Center teamed for the study, called ADAPT (Arthritis, Diet and Activity Promotion Trial). They found that participants in an 18-month program of exercise and calorie-restricted diet had a 24 percent improvement in physical function. Participants in this group also reported the most significant improvements in knee pain – a decrease of more than 30 percent.

Rheumatoid Arthritis by Kristine Duncan, MS, RD
Rheumatoid arthritis (RA) is estimated to affect 1% of the adult population worldwide (1), but appears most frequently in the elderly. It is defined as a chronic systemic disease characterized by inflammatory changes in joints and related structures that result in crippling deformities. The exact etiology is unknown, but it is assumed to be an auto-immune disease (2). An Italian survey of 46,693 people noted a strong correlation between vegetable consumption (odds ratio of 0.84) and decreased incidence of RA Food Elimination Vegetarian Diets Probably the most intensive study to look at the effects of diet on RA was conducted in Norway (8). Forty-five of the original 53 subjects were re-examined and comparisons were made between diet responders, non-responders and omnivorous controls at baseline and 2 years following introduction of the vegetarian diet (10). At follow-up, all diet responders and 50% of the non-responders were on some type of modified diet, described as either the original lacto-vegetarian study diet, or an omnivorous diet excluding foods identified by subjects as exacerbating their symptoms. Refined sugar, coffee, white flour and milk were most commonly excluded. There was significant improvement in disease symptoms in the diet responders compared to both non-responders and omnivore controls. There was also some improvement in non-responders on the vegetarian diet compared to the controls. Researchers concluded that patients with RA benefit from dietary manipulation with improvement lasting at least 2 years.

Until recently it's been unclear whether changing your diet may influence the symptoms of a chronic illness like arthritis. Nevertheless, new research continues to pour in touting the healing benefits of certain foods. Perhaps these foods aren't the miracle cure many hoped for, but through scientific studies we do know that certain nutrients can boost immune function and decrease inflammation in those with arthritis. Be sure to include the following suggestions in your pain-free diet to further reduce inflammation and pain. Sip Tea
You can now add tea to your list of healing foods. In fact, some experts claim that we should add tea to the list of disease-fighting fruits and vegetables that we should eat daily. Some intriguing information was presented at the Society of Critical Care Medicine in January 2002 on how green tea may help decrease inflammation. Green tea contains a type of polyphenol known as epigallocatechin-3 gallate, or EGCG, that inhibits the expression of the interieukin-8 gene. This is a key gene involved in the arthritis-inflammatory response. In these findings, researchers theorized that "more may be better" when it comes to green tea reducing the inflammatory response as EGCG shortcircuits the process that leads to inflammation. (If you like black tea, drink up! Black tea is made from the same leaves as green and contains theaflavins, strong phytochemicals that help to protect the body. Though processed differently, black tea may be equally effective and is tolerable for many people.)

Another factor that contributes to the controversy regarding diet is that most people want to help themselves. The majority of medications used to treat arthritis are available only by prescription, but foods and vitamins can be found on every store shelf. By manipulating diet and vitamin intake a person can be his or her own physician...for most people an appealing thought! There are several problems with studies evaluating diet and arthritis. The most obvious problem is that there have been very few studies of any kind. Those studies that have been done generally have involved a small number of patients. Many reports that have been published have been anecdotal and not subjected to investigation by the scientific method. Also, much of the research has involved animals. The results of animal studies are not always relevant to humans.
