Q: Can fasting or a vegetarian diet help control arthritis? I have quite bad arthritis in my hands and knees. I've been told that fasting or eating a vegetarian diet can help to control the arthritis. What should I do? Gwen Dr Trisha Macnair responds There has been a little bit of research to show that fasting can greatly reduce pain and symptoms during an acute worsening of arthritis but it's not clear how useful this is (arthritis is a chronic disease and we can't spend our life on a fast!) and it certainly isn't a cure - it won't stop the disease. Also it must be pointed out that this research was among people who had one particular type of arthritis - rheumatoid arthritis - and it's not certain that fasting helps at all in other types of arthritis. Fasting may reduce inflammation It seems that fasting, with a sudden drop of sugar or fats in the blood, may reduce the activity of some of the cells of the immune system. This seems to make sense - many people who have embarked on a major diet have discovered how they then seem to be vulnerable to infections and catch colds , etc.

    April 22, 2004 WASHINGTON (AP) - As baby boomers get older and fatter, they're also more likely to come down with one of the consequences of age and weight - arthritis. Although they can't prevent aging, boomers can reduce the risk from obesity, and diet and exercise can help them achieve it, doctors say. Osteoarthritis, the most common form, develops as cartilage that cushions the joints wears away, leaving the ends of the bones rubbing against each other. The Arthritis Foundation says X-rays can find signs of osteoarthritis in most people over 60. The condition commonly begins to show up between ages 40 and 60, so most boomers are "right smack in the target," said Bill Roberts, an associate professor of family medicine at the University of Minnesota and president-elect of the American College of Sports Medicine. Extra weight on the cartilage brings extra risk of the joint disease. "The simple explanation is, you're putting more biomechanical stress on the joints," said Kevin Fontaine, an assistant professor of rheumatology at the Johns Hopkins School of Medicine.

    by Carolyn J. Strange It may begin as a slight morning stiffness. For the lucky person with arthritis, that's as far as it goes. But for millions of others, arthritis can become a disabling, even crippling, disease. Roman Emperor Diocletian exempted citizens with severe arthritis from paying taxes, no doubt realizing that the disease itself can be taxing enough. One in seven Americansnearly 40 millionhave some form of arthritis. That number will climb as the baby boomers age. By 2020, about 60 million Americans will have arthritis, according to The National Arthritis Data Workgroup of the National Institute of Arthritis and Musculoskeletal and Skin Diseases. The disease is physical, but also exacts a mental, emotional and economic toll. "Chronic illness impacts a person's entire lifestylework, family and recreation," says Gail Wright, Ph.D., a rehabilitation psychologist at the University of Missouri, Columbia. To improve quality of life, doctors and health educators increasingly advise combining drug treatment with education, social support, and moderate forms of exercise.

    Clinical bottom line
    Few trials likely to be bias free (randomised, blind), and valid (longer duration) exist. The results are mixed, but a cautious interpretation may be that there is no evidence of lack of effect, while what evidence we have suggests some benefit. Clinical observation has been that diet may improve the symptoms of rheumatoid arthritis. The biology underlying any dietary influences is not exact. Some food additives are taken by patients with RA. It has also become traditional in some centres for patients to fast, followed by a vegan or vegetarian diet with add-back of foods not associated with symptom worsening. This may be accompanied by holistic approaches involving physiotherapy, exercise, and psychotherapy. A systematic review has examined the evidence-base for fasting and vegetarian diet.
    Reference
    Review
    Only MEDLINE was searched (up to 1997), and reference lists checked. Criteria for inclusion were trials with follow-up information for at least three months, and full journal publication.
    Results
    The review detected 31 reports. Only one was randomised. A further randomised report with just under three months duration was also included. The other reports were observational studies, or controlled studies. Most were very small, with few having more than 30 patients in total, or 30 patients per group. The main findings in each study are reported, including clinical and biochemical or immunological findings.

    ARTHRITIS AND CALCIUM The Arthritis Foundation has stated that the average adult eats only about 550 milligrams or so of calcium daily. The US Recommend Daily Allowance is between 800 and 1200 mg/day, and that itself may be to low. Everyone knows that arthritis, whether rheumatoid or osteoarthritis, is related to bones and joints... and so is what they are largely made out of: calcium. Nationwide calcium deficiency, and NO arthritis connection with diet? How curious.

In 1989, Arthritis Care ref 1 noted that greater than 50% of the Arthritis Care members who were surveyed, had invested in "unorthodox medicines, substances, or treatments (including diets), during the prior six months". Since the 1930's researchers have been exploring the link between diet and arthritis. We continue to hear claims that special diets, foods and supplements may help to cure or alleviate symptoms of arthritis, but most claims are unproven. Many experts speculate that claims of nutritional remedies and cures with food or dietary supplements are related to the "placebo affect". The placebo effect is the patient’s perception of improved symptoms and well being, resulting from new therapy, regardless of evidence for actual physical improvement. Because there is little scientific evidence confirming the benefits of modified diets for patients with arthritis, health care providers are apprehensive to recommend these types of dietary manipulations to their patients. Unfortunately, people with limited or no medical backgrounds have filled this void by advising patients about the benefits of diets and arthritis. The advice given is often questionable, expensive and can be dangerous.

    Described as an "anti-inflammatory diet" by proponents such as holistic health guru Dr. Andrew Weil, the food plan addresses inflammation, a burgeoning area of medical research.
    Although inflammation has long been implicated in arthritis, scientists increasingly are linking excessive inflammation to mental health problems as well as heart disease, Alzheimer's, cancer, diabetes and Parkinson's disease.
    Anti-inflammatory medications have shown to be effective in lowering the risk of Alzheimer's, but critics say they don't address the problem - just the symptoms - and can have unpleasant side effects, including headaches and nausea. As a supplemental approach, some doctors recommend following the ancient Hippocratic idea that suggests using food as medicine.

What can I do? ANSWER There is no miracle food cure for arthritis, rather a number of diet strategies that can improve symptoms for many patients. A recently published study on rheumatoid arthritis patients indicated the Mediterranean diet (based on the dietary habits of the residents of the island of Crete) emphasized fruits, vegetables, cereals and legumes with plenty of fish and little meat. Fats came mainly from olive oil, and the subjects drank moderate amounts of wine. The diet of the control group was higher in fats from meat and dairy products. After three months, the Mediterranean diet group had less pain, inflammation and disease activity and fewer affected joints than the control group. Researchers concluded that the Mediterranean diet provided moderate relief to patients with mild to moderate active arthritis.
