  • A formal minimum of three years (90 semester hours or 135 quarter hours), but a baccalaureate degree is preferred. All applicants are required to take and submit the Dental Admissions Test (DAT) scores. Preference is given to applicants who provide DAT scores are preferred by February 1 of the application year. No scores older than three years will be accepted. Applicants must have a minimum cumulative and science grade point average of 2.50 on a four-point scale. The overall and science GPA, the school(s) attended, and the rigor of the academic course load are all assessed on an individual basis. Applicants must be U.S. citizen or permanent U.S. resident.

Accreditation Our Physician Assistant Training in Primary Care grant, entitled "A Native American Physician Assistant (PA) Distance Education Program" was funded and approved by the Bureau of Health Professions. We are using a classroom, office space and dedicated conference/student study center within the Arizona State University-East complex (approximately 15,000 square feet) for the Arizona School of Health Sciences (ASHS) Native American Physician Assistant (PA) extension program. Each fall, we will enroll a minimum of six (6) Native American students who will attend didactic courses through interactive videoconferencing. In addition, the NA site will be staffed with full and part-time PA instructors and one administrative assistant. Clinical rotations will be completed at Tribal, Indian Health Service, rural and/or underserved facilities throughout the Southwest. The goal of this extension program is to increase retention of American Indian PAs in tribal communities. We look forward to our partnership in the recruitment and training of Native Americans. Our shared goal of improving the availability and accessibility of primary health care services within medically under-served Indian regions is one step closer to reality.

    School Facilities Board
    Students FIRST is a school capital finance program funded by revenues dedicated from the state transaction privilege (sales) tax. Monies are transferred annually by the State Treasurer to the Arizona School Facilities Board, which administers the program; there is no need for legislative appropriation. Arizona public school districts receive funding for the correction of deficiencies in existing school facilities, building renewal, and new school construction, while maintaining the ability to raise local funds through limited general obligation bonding and capital overrides.
    Background
    Deficiencies Correction
    The Students FIRST law established a deficiencies correction fund for the purpose of correcting deficiencies in existing school facilities. Deficiencies might take the form of a square footage deficiency or a quality deficiency, and must be corrected by June 30, 2003. The School Facilities Board is charged with adopting rules establishing minimum school facility guidelines, assessing school buildings against these guidelines, and providing monies to bring the buildings up to the guidelines. On November 18, 1999, the Board adopted Building Adequacy Guidelines that now serve as the minimum standards for existing and new school facilities in Arizona.

    The Students FIRST law established a new school facilities fund for the purpose of constructing new schools to meet the minimum adequacy guidelines. The criteria to determine district eligibility for monies from the new school facilities fund are based on annual evaluation and approval of district enrollment projections and the additional square footage that will be needed to maintain adequacy standards in a district. The School Facilities Board distributes new school facilities monies to school districts based on the following formula: (number of students) x (square footage) x (cost per square foot) = allocation. Land costs are funded in addition to formula funding for new construction.

    R ehabilitation Service Administration (RSA)
    The Rehabilitation Services Administration (RSA) manages a number of programs for persons with disabilities. One of these programs is the Vocational Rehabilitation (VR) program.
    VR Program
    If you have a disability, regardless of whether or not you are in Special Education, the VR program may be important as you begin planning your transition from school. Services may begin while you are still in school (if there are special agreements between the school and VR) or after you exit school. After you exit school, the VR program provides opportunities such as: college education; additional formal skill training; work related assistive technology goods/services; and/or job development and placement services; to help you become economically independent through work.

Arizona law (A.R.S. 43-1089.01) allows Arizona taxpayers to contribute up to $200 ($250 for married couples filing a joint tax return) to a public school and receive a dollar-for-dollar credit on their state taxes. The tax credit is available for contributions which are earmarked to support a qualifying extracurricular activity. Any taxpayer is eligible for the credit - you don't have to be a parent. Okay, so how does the "tax credit" work? Just send a donation by check or money order to the Amphitheater School of your choice. Your donation may be designated for the general support of extracurricular programs at the school of your choice, or you may designate a specific extracurricular program. You may also specify that your donation is a fee to cover the cost of a specific child's participation in a specific activity. You must enclose your social security number with your donation, so the school can send you the receipt which is required to take the tax credit.
