Apnea (literally, "without breath") is the term used when someone stops breathing for very short periods of time, usually 10 to 20 seconds. It's termed "obstructive" when respiratory efforts continue, such as movements of the chest. It's termed "central" when all respiratory effort stops. There is also a mixed version. In children, sleep apnea is almost always obstructive. During the apneic episode, the child will have decreased oxygenation of the blood. Children with Down syndrome (DS) are certainly at risk for OSA. In 1991, one study showed 45% had OSA. This can be caused by several different factors present in DS: the flattened midface, narrowed nasopharyngeal area, low tone of the muscles of the upper airway and enlarged adenoids and/or tonsils.
Why is this important? Well, first, there's the obvious problem of the child not getting enough quality sleep and the behavioral effects that brings. Second, I've mentioned above that during sleep apnea, the oxygenation of the blood decreases. It has been shown that in children with DS and heart disease this low oxygenation causes an increase in the blood pressure in the lungs as the body tries to get more oxygen. This "pulmonary hypertension" can cause the right side of the heart to become enlarged and other cardiac complications can follow. The incidence of death due to OSA is unknown.

There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.

Sleep Apnea is a disorder of breathing during sleep. Typically it is accompanied by loud snoring. Apnea during sleep consist of brief periods throughout the night in which breathing stops. People with sleep apnea do not get enough oxygen during sleep. There are 2 major types.
Obstructive Sleep Apnea is the most common type and is due to an obstruction in the throat during sleep. Bed partners notice pauses approx. 10 to 60 seconds between loud snores. The narrowing of the upper airway can be a result of several factors including inherent physical characteristics, excess weight, and alcohol consumption before sleep. Central Sleep Apnea - caused by a delay in the signal form the brain to breath . With both obstructive and central apnea you must wake up briefly to breathe, sometimes hundreds of times during the night. Usually there is no memory of these brief awakenings.

Sleep Apnea June 2001 WHAT IS SLEEP APNEA? General Description of Sleep Apnea and Snoring Sleep apnea is a disorder in which a person stops breathing during the night, perhaps hundreds of times, usually for periods of 10 seconds or longer. In most cases the person is unaware of it, although sometimes they awaken and gasp for breath. It is usually accompanied by snoring. People who have sleep apnea may not even be aware of the condition, but it inevitably causes daytime sleepiness.
Sleep apnea is generally categorized as obstructive, central, or mixed. A less severe form of obstructed breathing called upper airway resistance syndrome (UARS) is also worth mentioning. Obstructive Sleep Apnea Obstructive sleep apnea (OSA), the most common form of apnea, occurs when tissues in the upper throat (or airway) collapse at intervals during sleep, thereby blocking the passage of air. In general, OSA occurs as follows:
  • On its way to the lungs, air passes through the nose, mouth, and throat (known as the upper airway).
  • Under normal conditions, the back of the throat is soft and pliant and tends to collapse inward as a person breathes.

Treating sleep apnea could cut road deaths Last Updated: 2004-05-07 16:06:29 -0400 (Reuters Health) By Amy Norton NEW YORK (Reuters Health) - Nearly 1,000 deaths from vehicle collisions could be avoided each year in the U.S. if all drivers with the obstructive sleep apnea received a standard treatment, according to researchers. Obstructive sleep apnea is the most common form of sleep apnea, a disorder marked by repeated stops and starts in a person's breathing during sleep, sometimes hundreds of times during the night. These breathing disruptions make for poor-quality sleep and daytime symptoms such as drowsiness, concentration problems and dulled reaction times potential hazards on the road. The new study suggests that of an estimated 1,400 traffic deaths related to obstructive sleep apnea in 2000, 980 could have been prevented if all the drivers had been treated with continuous positive airway pressure (CPAP). The estimates, based on data from the National Safety Council and published medical studies, also indicate such treatment could save Americans $11 billion a year in collision costs. Dr. Alex Sassani of the University of California San Diego and his colleagues report the findings in the journal Sleep.
