It's a parent's worst nightmare. What could be more terrifying than hearing or watching your baby stop breathing? With all the other complications of a preterm birth, is apnea of prematurity something else to worry about? Does it go away on its own or can it be cured? Worse yet, does this condition cause SIDS Apnea of prematurity is fairly common in preemies and usually resolves on its own around the time the premature infant has matured enough to equal the age of a full-term infant at birth. Once apnea of prematurity stops, it does not come back. But there's no doubt about it - it's frightening while it's happening. What's the Difference Between AOP and Periodic Breathing?
Apnea is a medical term that means someone has stopped breathing. Most experts define apnea of prematurity (AOP) as a condition in which premature infants stop breathing for 15 to 20 seconds during sleep. But Eric Gibson, MD, a neonatologist, says it's hard to measure and define the duration of cessation of breathing. He prefers to describe AOP as a condition in which babies (who are generally born at less than 35 weeks' gestation) have periods when they stop breathing or their heart rate drops (a slowed heart rate is known as bradycardia ). These breathing abnormalities may last for up to 2 to 3 months after the birth of these premature infants, depending on the infant's gestational age at birth.

    O bstructive sleep apnea is a disorder in which complete or partial obstruction of the airway during sleep causes loud snoring, oxyhemoglobin desaturations and frequent arousals. As a result, affected persons have unrestful sleep and excessive daytime sleepiness. The disorder is associated with hypertension, impotence and emotional problems. Because obstructive sleep apnea often occurs in obese persons with comorbid conditions, its individual contribution to health problems is difficult to discern. The disorder has, however, been linked to angina, nocturnal cardiac arrhythmias, myocardial infarction, stroke and motor vehicle crashes. Even though obstructive sleep apnea is fairly common, it often remains undiagnosed in primary care practice. Because the disorder is associated with significant morbidity and even some mortality, family physicians need to be familiar with its clinical presentation and treatment. Pathophysiology Obstructive sleep apnea is caused by repetitive upper airway obstruction during sleep as a result of narrowing of the respiratory passages. Patients with the disorder are most often overweight, with associated peripharyngeal infiltration of fat

    S leep apnea is a cessation of breathing during sleep that is caused by repetitive partial or complete obstruction of the airway by pharyngeal structures. Patients with obstructive sleep apnea often are overweight, snore loudly, and complain of daytime fatigue and sleepiness. A sleep laboratory may be used to confirm the diagnosis. Family physicians should be able to diagnose and manage this relatively common disorder. Deciding When to Treat The risk of long-term mortality increases when patients have 20 or more respiratory events per hour during sleep testing. The initial step in the management of sleep apnea is deciding which patients need treatment. The severity of the condition is measured by the number of apneas (cessations of airflow) or hypopneas (reductions in airflow) that cause sleep arousal. The number of these events per hour is commonly called the "apnea-hypopnea index" or the "respiratory disturbance index." An often quoted study recommended treatment if patients had 20 or more respiratory events per hour, because long-term mortality increases above that level.

    What is sleep apnea?
    Sleep apnea (sleep-disordered breathing) is a serious and common sleep disorder affecting about 12 million Americans, according to the National Institutes of Health (NIH). Its name comes from a Greek word, apnea , meaning "without breath." People with sleep apnea stop breathing briefly many times during the night. The breathing pauses last at least 10 seconds, and there may be 20 to 30 or more pauses per hour. The main symptoms of sleep apnea are persistent loud snoring at night and daytime sleepiness. Another symptom is frequent long pauses in breathing during sleep, followed by choking and gasping for breath. People with sleep apnea don't get enough restful sleep, and their daytime performance is often seriously affected. Sleep apnea may also lead to high blood pressure, heart disease, heart attack, and stroke. However, it can be diagnosed and treated.

Obstructive sleep apnea causes snoring and interruptions in breathing during sleep. Surgical correction involves removal of some of the tissue in the throat to widen the airway. The modified technique used in the study in the new issue of the Archives of Otolaryngology is called extended uvulopalatal flap surgery. It removes fatty tissues, soft glands and the tonsils to increase airway space but spares muscle tissue.
