    Anxiety Disorders
    The anxiety disorders are the most common, or frequently occurring, mental disorders. They encompass a group of conditions that share extreme or pathological anxiety as the principal disturbance of mood or emotional tone. Anxiety, which may be understood as the pathological counterpart of normal fear, is manifest by disturbances of mood, as well as of thinking, behavior, and physiological activity.
    Types of Anxiety Disorders
    The anxiety disorders include panic disorder (with and without a history of agoraphobia), agoraphobia (with and without a history of panic disorder), generalized anxiety disorder, specific phobia, social phobia, obsessive-compulsive disorder, acute stress disorder, and post-traumatic stress disorder (DSM-IV). In addition, there are adjustment disorders with anxious features, anxiety disorders due to general medical conditions, substance-induced anxiety disorders, and the residual category of anxiety disorder not otherwise specified (DSM-IV). Childhood and Adolescent Anxiety Disorders

Goldman, MD Anxiety symptoms and disorders are the number one health problem in America, ranging from a simple Adjustment Disorder to more difficult and debilitating disorders such as Panic Disorder and Posttraumatic Stress Disorder. According to the most recent data, the lifetime prevalence for anxiety disorders as a whole in adults is about 25%; the frequency in children is unknown, but felt to be significantly underreported and under-diagnosed. More specifically Social Anxiety Disorder has a lifetime risk of 17%, while Panic Disorder occurs in approximately 1-3% of the adult population. Symptoms of Anxiety Disorders
Anxiety is a subjective sense of worry, apprehension, fear and distress. Often it is normal to have these sensations on occasion, and so it is important to distinguish between normal levels of anxiety and unhealthy or pathologic levels of anxiety. The subjective experience of anxiety typically has two components: physical sensations (e.g., headache, nausea, sweating) and the emotions of nervousness and fear. Anxiety disorders, when severe, can affect a child's thinking, decision-making ability, perceptions of the environment, learning and concentration. It raises blood pressure and heart rate, and can cause a multitude of bodily complaints, such as nausea, vomiting, stomach pain, ulcers, diarrhea, tingling, weakness, and shortness of breath, among other things.

    Etiology of Anxiety Disorders
    The etiology of most anxiety disorders, although not fully understood, has come into sharper focus in the last decade. In broad terms, the likelihood of developing anxiety involves a combination of life experiences, psychological traits, and/or genetic factors. The anxiety disorders are so heterogeneous that the relative roles of these factors are likely to differ. Some anxiety disorders, like panic disorder, appear to have a stronger genetic basis than others (National Institute of Mental Health [NIMH], 1998), although actual genes have not been identified. Other anxiety disorders are more rooted in stressful life events. What the myriad of anxiety disorders have in common is a state of increased arousal or fear (Barbee, 1998). Anxiety disorders often are conceptualized as an abnormal or exaggerated version of arousal. Much is known about arousal because of decades of study in animals Acute Stress Response

    rather than anxiety . Anxiety differs from arousal in several ways (Barlow, 1988; Nutt et al., 1998). First, with anxiety, the concern about the stressor is out of proportion to the realistic threat. Second, anxiety is often associated with elaborate mental and behavioral activities designed to avoid the unpleasant symptoms of a full-blown anxiety or panic attack. Third, anxiety is usually longer lived than arousal. Fourth, anxiety can occur without exposure to an external stressor.

F or years, social anxiety disorder, also known as social phobia, has been underrecognized and undertreated. That situation is beginning to change, however, because recent research has shown that the disorder is highly prevalent, chronic in its untreated course, often associated with comorbid mental and substance-related problems, and capable of disabling those who have it. We now know more about recognizing social phobia and the types of interventions to which it is responsive. Clinical Features Social phobia is an intense, irrational and persistent fear of being scrutinized or negatively evaluated by others. Everyone experiences anxiety as a normal reaction to threatening, dangerous, uncertain, or important situations. Psychiatric medicine classifies anxiety as normal or pathological. Normal anxiety can enhance some people’s function, motivation, and productivity, such as the person who works well under pressure. People with generalized anxiety disorder (GAD) experience pathological anxiety, which is excessive, chronic, and typically interferes with their ability to function in normal daily activities. Generalized or "free-floating" anxiety is distinguished from phobia because it is not triggered by a specific object or situation.
