AIED defined
How common is it Diagnosis Treatment Education Index Autoimmune inner ear disease or "AIED" consists of a syndrome of progressive hearing loss and/or dizziness which is caused by antibodies or immune cells which are attacking the inner ear. The classic picture is reduction of hearing accompanied by tinnitus (ringing, hissing, roaring) which occurs over a few months. Variants are bilateral attacks of hearing loss and tinnitus which resemble Meniere's disease , and attacks of dizziness accompanied by abnormal blood tests for self-antibodies. About 50% of patients with AIED have balance symptoms. The immune system is complex and there are several ways that it can damage the inner ear. Both allergy and traditional "autoimmune disease" such as Ankylosing spondylitis , Systemic Lupus Erythematosis (SLE), Sjoegren's syndrome (dry eye syndrome), Cogan's disease , ulcerative colitis, Wegener's granulomatosis, rheumatoid arthritis, and scleroderma can cause or be associated with AIED. Biochemical studies demonstrated that RA patients had antibodies to both EQRRAA/ESRRAL sequences.
Further molecular studies identified a second similarity sequence IRRET, present in Proteus urease which showed molecular mimicry with the LRREI sequence found in type XI collagen, a common component of hyaline cartilage. Since erosions of hyaline cartilage are frequently found in RA patients, especially in the small joints of the hands and feet, this could explain the localization of the pathological lesions in this disease to these sites. These results are consistent with the hypothesis that RA is an autoimmune reactive arthritis following infection of the upper urinary tract by Proteus mirabilis microbes.

General Discussion
The conversion of ligamental tissue to bone usually extends along the sides of the vertebrae of the spine. (This may be called flowing calcification.) Also, DISH is associated with inflammation (tendinitis) and calcification of the tendons, especially at the points at which the tendon attaches to the bones. When this happens, the patient is said to have developed bone spurs, especially in the heel and ankles (heel spurs).
DISH affects three or more vertebrae that are most often located in the chest or in the spine between the chest and pelvis. It is a disorder of older patients, more often affecting men than women ages 50-60. The disorder is often found in association with diabetes, high blood pressure, heart disease and obesity.

Gina: Would you tell us what the different types of juvenile rheumatoid arthritis are and what the symptoms are for each of them? Dr. Lehman: An equally common group is polyarticular (means many joints) JRA, and [this is composed of] children at almost any age up to 16, because if you are over 16, it is not called juvenile [rheumatoid arthritis] anymore. They start off with a large number of joints, five or more, and often they are much thicker at the beginning. [Medical editor's note: That is, the joint tissue, the synovium, is more inflamed (synovitis) and swollen, which can be felt on examination.] They have trouble opening their hands; it frequently involves the small joints of the hands and feet, as well as having troubles with the hips or the knees. They are stiff when they wake up in the morning [and] they often have difficulty walking. [It occurs] often [in] the older children, and you'll notice that not only do they have trouble when they are walking in the morning, but if you take them for a long car ride, they'll have trouble getting out of the car and they'll walk like a little old man, and then in five or 10 minutes, it will get better. These children have much more difficult to treat disease, and often disease that continues on the later life.

DEFINITION: Ankylosing spondylitis is an autoimmune disease that causes inflamation of the spine and sacroiliac joints and can cause inflammation of the eyes, lungs, and heart valves. It varies from intermittent episodes of back pain that occur throughout life to a severe chronic disease that attacks the spine, peripheral joints and other body organs, resulting in severe joint and back stiffness, loss of motion and deformity as life progresses. A dramatic loss of flexibility in the lumbar spine is an early sign of AS. Although most symptoms begin in the lumbar and sacroiliac areas, they may involve the neck and upper back as well. Arthritis may also occur in the shoulder, hips and feet. As with many of the autoimmune diseases, many body systems can be effected in extreme cases.
AT RISK: Ankylosing spondylitis typically starts as an insidious onset of back pain in the late teens or early twenties, and seems to effect men more than women. It is a genetically determined disease, susceptability to which is related to the presence of the HLA-B27 antigen in the patients' cells.

About the AS Family Genetic Study

This is an exciting time in research for ankylosing spondylitis (AS) and related diseases. To keep the momentum going we need the help of people like you to participate in research trials. By volunteering, you are becoming an advocate for continued research into treatment for AS. Visit our AS Family Genetic Study Site to see whether you are eligible to participate in the study. The real excitement is the discovery of regions on at least two chromosomes implicated in ankylosing spondylitis (AS), stated John D. Reveille, MD, the study's Principal Investigator, during the American College of Rheumatology Scientific Meetings in 2002. "We've known for some time that there were between two and nine additional genes involved in the predisposition to AS," Dr. Reveille said. "Without any question, the leader of the pack is HLA-B27. We've just picked up two more in line, but where they stand (as far as importance) is not clear. We believe that HLA-B27 is the biggest factor, these other genes influence the condition, interacting with each other to affect susceptibility.

References The synovial membrane (SM) of affected joints in ankylosing spondylitis (AS) is infiltrated by germinal center-like aggregates (foci) of lymphocytes similar to rheumatoid arthritis (RA). We characterized the rearranged heavy chain variable segment (VH) genes in the SM for gene usage and the mutational pattern to elucidate the B lymphocyte involvement in AS. Cryosections from an AS-derived SM were stained for B and T lymphocytes. B cells were isolated from different areas of a focus. The rearranged VH genes were amplified by semi-nested polymerase chain reaction (PCR) using oligonucleotides specific for the six different VH families and heavy chain joining segments (JHs). PCR products were cloned and sequenced.

MAIN AREA OF RESEARCH Our research involves investigating genetic predispositions to autoimmune disease as demonstrated in ankylosing spondylitis, as well as septic arthritis which may predispose to tissue disorders. BIOGRAPHY Dr Field graduated from University in London in 1977, undertook a clinical career initially in Oxford and subsequently joined the Charing Cross and Westminster University Medical School in London working as a lecturer in the Kennedy Institute of Rheumatology. Dr Field joined the Centre for Rheumatic Diseases in 1991. PROJECTS Septic Arthritis This clinically based project has assessed septic arthritis because this is one of the most rapidly destructive arthritic diseases known to man. We have been investigating mechanisms of this joint destruction and predisposition to infection.

Recent Developments in the Treatment of Arthritis
Published by SMI Publishing, Ltd
Jul. 1, 2001
Arthritis is a debilitating disease which largely affects older individuals. There are actually three distinct forms of arthritis: rheumatoid arthritis- a familial, autoimmune disorder; osteoarthritis- a disease which affects the weight-bearing joints; and arthritis related to pathogens which is a temporal disorder which arises as a result of injury or exposure to viruses, bacteria, or parasites. The incidence of arthritis is on the rise due to the aging of the population and also due to an increasing percentage of individuals who are overweight. This marketing report will address the progress in arthritis therapy that has been made in the last two years. Arthritis has recently been recognized as an autoimmune process and treatment for arthritis has changed accordingly. New therapies are in development which will treat the underlying cause of arthritis, rather than the symptoms. This report will address the trends in arthritis research and will prepare the healthcare executive to deal with them. This report will include such features as:
  • background on the epidemiology, etiology, and pathogenesis of all three types of arthritis
