Abdominal Aortic Aneurysm and Low Back Pain
Norman Engel, MS, DC, DABCO An aneurysm, by definition, is a sac formed by the dilatation of the wall of an artery, a vein or the heart. The chief signs of an arterial aneurysm are the formation of a pulsating tumor and often a bruit is heard over the swelling. Sometimes there are symptoms from pressure on contiguous parts. The aorta is the largest artery in the body. It arises from the left ventricle, ascending and bending over the heart, descending anteriorly to the thoracic and lumbar vertebrae. It splits at L-4 into the common iliacs, femorals, tibials, and dorsalis pedis arteries, supplying the lower extremities. The average diameter of aorta is 2.0-2.5 cm. More than 90 percent of abdominal aneurysms are associated with atherosclerosis and can cause leg pain (claudication), numbness or fatigue. An artery may be sclerosed (hardened) but if the lumenal diameter isn't altered, there is no aneurysm. Most abdominal aortic aneurysms do not produce symptoms, but some people feel a pulsating sensation in the abdomen. These silent (asymptomatic) aneurysms are often recognized by careful physical examination. Abdominal aortic aneurysm is most likely to occur in people older than 60 years, and it affects men more often than women.

Ardith Eastlund had no idea she had an aneurysm until her twin sister died from an aneurysm rupture. From Debra Goldschmidt CNN Medical Unit ANN ARBOR, Michigan (CNN) Ardith Eastlund was 66 and felt great. She was enjoying life, spending time with her family and traveling with her husband, Dale. They were the best of times. Then last year, while on a cruise, she received news that threatened to topple her happy existence: Her sister had suffered an aneurysm and was in a coma. Her twin sister. The aneurysm, a blister on a blood vessel on the brain, proved fatal for Ardith Eastlund's sister. Like 40 percent of people who suffer an aneurysm, her sister never awoke; she lingered for a week. A new procedure called "coiling" fills the aneurysm with tiny metal coils.

But two new devices, approved by the Food and Drug Administration Tuesday, may offer a new way to repair this very dangerous bulge in an artery. An AAA is a weak area that develops in the wall of the main artery that brings blood flow into the abdomen. If the aneurysm breaks, it can cause almost instant fatal internal bleeding.

Chlamydia pneumoniae Causes Abdominal Aortic Aneurysm
Antibodies to Chlamydia pneumoniae are rarely found in children less than five years old; the primary infection usually occurs between ages 8 and 16. Reinfections occur throughout childhood, and the rate of infection is higher among men than women. In non-elderly people it causes mild respiratory symptoms, but may cause pneumonia in the elderly. Indications of the presence of Chlamydia pneumoniae have been found in the majority of studies which have looked for them. C pneumoniae was found in 11/25 aortas, 5/9 iliac arteries, 2/5 femoral arteries, and 1/2 iliac veins, by PCR in AAA specimens. It was also seen in vascular tissue from 3/6 "ostensibly normal" liver transplant donors, which showed some atherosclerotic changes. "The figures are perhaps conservative as it is possible that more specimens would be found to be C pneumoniae positive if a larger amount of tissue was examined." Open repair of Abdominal Aortic Aneurysm Surgery
An aneurysm is a swelling or dilation of a weakened wall in an artery. Arterial aneurysms have a tendency to grow and burst causing life threatening bleeding. Treatment of this disease has been ongoing for many years through open surgical repair of the dilated artery. Using an abdominal incision, open surgical repair of abdominal aortic aneurysms allows complete removal of the aneurysm with replacement using a piece of Teflon tubing. Within the last five years a new approach to treating aortic aneurysms has been developed. This is a minimally endoscopic invasive approach that was pioneered by Surgical Care Associates in this area.

Abdominal aortic aneurysm
While low back pain rarely radiates to the abdomen, abdominal disorders can often extend to the back and be felt as acute low back pain. This process happens in an abdominal aortic aneurysm, which represents an enlargement of the aorta (large artery) in the abdomen. In the elderly population this is not uncommon and rarely is symptomatic. However, if the enlarged blood vessel begins to leak blood or ruptures it can cause severe, unremitting abdominal and back pain.

Preview : Surgery is the only effective treatment for an abdominal aortic aneurysm. Unless serious contraindications prohibit it, repair should be undertaken in all patients with a known aneurysm while it is still an elective procedure. Complication and mortality rates increase dramatically when an aneurysm ruptures and surgery becomes an emergency procedure. In this article, the authors discuss what is known about formation and rupture of aneurysms, including patients at risk.
